seen its conclusions. our special correspondent lucy manning reports. the families of those who died at grenfell, and the survivors, have waited more than two years for this report, which heavily criticises the london fire brigade. it says its preparation and planning for a fire such as grenfell was wholly inadequate. it concludes that if the fire brigade had made an earlier decision to evacuate the building rather than telling residents to stay put, it's likely more lives would have been saved. the report praises the extraordinary courage of firefighters on the night, but identifies many failures. there were serious deficiencies in command and control, inadequate training, problems dealing with 999 calls, basic information on the building was missing and the communication systems didn't work properly. the london fire brigade commissioner, dany cotton, is personally criticised. the report says her evidence that she would not change anything

about the response on the night showed remarkable insensitivity, and suggests the fire brigade is at risk of not learning the lessons of grenfell. the report also concludes the cladding on the outside of the building failed to comply with regulations. the second part of the enquiry will investigate the failures of the building materials and refurbishment. the london fire brigade said it would be inappropriate to comment until the full report is published tomorrow. our special correspondent, lucy manning reporting there. and in a few minutes we'll be speaking to the fire brigades union. two british people have been seriously injured in a shark attack in australia. the men, who are thought to be in their 20s, were in the water off the coast of queensland when it happened. let's get more on this from our correspondent phil mercer, who's in sydney. some pretty grim details, what can

you talus? it was a terrifying attack on two british tourists aged 22 and 28. we understand they won a snorkelling trip near a place called who could passage off the whitsunday islands, very popular place for international visitors in northern queensland. the shark attacked one man and came back to attack the other, we understand. the older man has lost a foot and both british tourists were taken by boat to land before being transferred to hospital ina very before being transferred to hospital in a very serious condition. tracy eastwick is from the queensland ambulance service. it is understood that the two patients were on a day—cruise vessel out at airlie beach, and they were snorkelling at the time. both patients are male and both in their 205. they are believed to be british backpackers visiting the area. one of the male patients was attacked first and the shark is believed to have returned and come back and attacked the second patient.

we know in the last year there have been five shark attacks in that region around the whitsunday islands in queensland, so clearly this is a political issue too. there is much political issue too. there is much political discussion in queensland about how to protect surfers and snorkellers from shark attacks and clearly the two men are still in hospital, still suffering those very serious injuries, and the issue of shark controls will intensify after the hideous attacks on these two visitors. thank you for the details, phil mercer. boris johnson will today make his fourth attempt to trigger an early general election despite mps rejecting a similar attempt last night. he's publishing a bill that would only need a simple majority to succeed, not two thirds as required in previous votes. well, let's talk to our political correspondent chris mason who's in westminster.

on the top floor of the tower. and he called the fire services on a number of occasions that night. he was calm, collected, because he was told to stay where he was. they knew where he was and they were coming to get him. and then, eventually, the smoke came into his flat and he... ..he died, you know? you know, they didn't do enough, i feel. could the fire service have done more? normally fires in tower blocks are contained in one flat. residents are told to stay put because it's safer than trying to get out. your full name? danielle amara cotton. the public enquiry was told there was an early need for a total evacuation. but at grenfell the stay put policy was kept in place for almost two hours. i wouldn't change anything we did on the night, i think... the head of london's fire brigade stunned residents when she told the enquiry she couldn't have

planned for the fire and wouldn't have done anything differently. do you stand by everything you said at the enquiry? so clearly a lot of things i said at the enquiry were based on what we knew on the night and our response on the night. speaking at the london assembly two weeks ago, dany cotton said lessons had been learned. today if there's a fire in a building with cladding the advice is to get out and stay out. those buildings need to be evacuated immediately. and by the time we get there, the wait and watch people should be taking the steps and we go and assist and evacuate. if you had your time again, you would have abandoned stay put earlier? so if i was able to know about the building what i know now, clearly we'd have to. we didn't know that about the building. no building should ever have behaved like that. it let the people of grenfell down. but there were warnings from history. most recently at lakanal house in 2009, a refurbished tower and south london where fire spread up the outside. again, people were told to stay put and died. we all compare lakanal house because there was a public enquiry,

it made recommendations, scotland listened and scotland implemented those recommendations but we didn't. and, had we have done, we probably would never have been in this situation we are in today. the public enquiry‘s first report tomorrow will focus largely on the response of the fire service, questions about cladding, the local authority, early regulations will come next year. some are worried firefighters will be made scapegoats. every month near grenfell tower there is a silent march. the most poignant moment when residents pass the local fire station. there is no doubt firefighters acted heroically on the night. but the fire service does have questions to answer. graham satchell, bbc news. matt wrack, general secretary of the fire brigades union, joins us now from our london newsroom. resort a little bit of him in that report. thank you very much for coming on the programme —— we saw a little bit of him. i know you have

signed a nondisclosure agreement so cannot discuss certain things, but i would like to talk around the report if possible. the leaks are highly critical of the london fire brigade, i wonder what your reaction is to the planning about this type of incident, saying it was wholly inadequate? a girl it is unfortunate somebody has leaked this, it will cause a lot of dismay to firefighters and the local community and we are complying with the agreement we reached with the g re nfell tower agreement we reached with the grenfell tower inquiry. the first frustration firefighters will have is that, as we want from the start, the inquiry is back to front. we are looking at the night of the fire instead of what led up to it. the truth is that fire spread in the way it did because it was wrapped in clu b it did because it was wrapped in club —— in flammable cladding, that isa club —— in flammable cladding, that is a key issue but it is not being addressed. the firefighters turned up addressed. the firefighters turned up after the building had, in reality, already been turned into a

death trap. firefighters' reactions, which were remarkable in the circumstances, are being scrutinised. nobody is trying to avoid scrutiny but we think the ordering of the inquiry is com pletely ordering of the inquiry is completely back to front. do you think some of those firefighters, who we noted heroic things that night, feel unfairly scrutinised ? who we noted heroic things that night, feel unfairly scrutinised? —— who we know did heroic things.” think there is a sense of injustice. the report mentioned lakanal house, the clarinet la ka nal the report mentioned lakanal house, the clarinet lakanal house wrote to various people, including the london fire brigade, but also eric pickles, a government minister. the people getting away in all the scrutiny scot—free are the people who set policy which created the regime that allow buildings to have fire safety undermined. nobody is questioning them. we raise the question of state purchased several years ago following the deaths of two

firefighters, we raised it directly in relation to grenfell at least as early as last december. the inquiry was asked buyers to issue an interim recommendation regarding a review of state perch at that time. they did not act. we wrote to government ministers, they did not act. people in very high positions of authority have failed to do anything serious about the risks facing people in residential blocks of flats, but firefighters are now being held to public scrutiny, i think it is somewhat unjust. explained the significance of lakanal house to us? the core problem grenfell tower is the breakdown of what we call compartment station. these buildings we re compartment station. these buildings were designed by the architects and we re were designed by the architects and were safer when they were built but they are now, meaning the fire should be contained within the compartment, or the flat or room of origin, because of fire safety design into the walls, ceilings, the floors, the doors, the windows and

so on. floors, the doors, the windows and so on. that is all compromised by wrapping the building in flammable cladding, but we find a whole list of failings of fire safety provision within grenfell tower. firefighters work on the assumption that the building will act as it is designed to act, clearly at grenfell tower, it was not operating in that way. the warning sign and lacking our was there was another fire were compartment agent broke down and the coroner alerted the secretary of state, eric pickles, of the need to address those issues. 0ur concern is that was not followed through thoroughly. one side of this is theirfamily, friends, thoroughly. one side of this is their family, friends, residents desperate for answers and desperate forjustice. 0n the other side of that, you represent the firefighters who feel have been unfairly criticised, and the spotlight being put upon them. can you take us back

to that night? in terms of failure of communication, where you and others are where things were not being done correctly? the firefighters stand in solidarity with the community. including the right of people to ask difficult questions, nobody is trying to judge that. firefighters have known for a long time, i think members of the public were quite shocked, at the communications equipment. this is not a failure of the system, it is a failure of equipment, the higher up the building firefighters went, the less likely they were to be able to communicate with control systems on the ground. people trying to ensure the ground. people trying to ensure the safety of cruise on the upper floors loss communications with those firefighters who were in the most dangerous part of the building. i think people would be shocked that is the situation facing firefighters in the capital city. those lessons have come out of earlier incidents

and accidents we have had to investigate, that is not particularly surprising. the issue is systemic, firstly how do we allow buildings to see fire safety com pletely buildings to see fire safety completely compromised ? secondly, how is it the systems we have in place are not actually applying the lessons, because some of these were known, why are they not applying the lessons across the country? it is not a london fire brigade issue, it isa not a london fire brigade issue, it is a national issue. there are buildings with this type of cladding all over the country and buildings with other breakdowns and fire safety all over the country. that is a national matter and one that government should have addressed immediately after the grenfell tower fire. instead, they had told us they are waiting for this inquiry. we might be waiting years to get to the bottom of this if we allow it to continue. i know you have been with usa continue. i know you have been with us a long time already, but we heard in the report, graham satchell asked

jonny clayton about what she said at the time, she said she would not have changed anything and she seems to have slightly changed position. do you think london fire brigade and others have learnt enough about what happened at grenfell, so it would not happen in the same way? —— graham satchell asked da ny not happen in the same way? —— graham satchell asked dany cotton. the people running the london fire brigade have to apply the lessons of this fire and previous fires. we have alerted government ministers of the need to act quickly on altering or reviewing the issue of state perch, for example government ministers have failed to act on that. let's scrutinise london fire brigade and its management, but why is nobody holding to account the fire minister or the home secretary all, for that matter, the prime minister, who was previously in charge of the london fire brigade himself, because. really good to
