it's five o'clock. we're live at westminster — where the house of commons has observed a minute's silence — in memory of the victims of the fire at grenfell tower — which claimed the lives of 72 people in june 2017. today saw publication of a report on the fire — calling for the government to treat its response as ‘a national emergency‘. the report is critical of the london fire brigade‘s response — and says the tower did not meet building regulations. the london fire brigade said it was ‘disappointed' — by some of the criticism of individuals — followng calls from mps and campaigners for resignations at the most senior levels of the capital's fire service. more from westminster in a moment — but first our home affairs correspondent sarah corker with this report. operationalfailures by the london fire brigade. a tower block that didn't comply

in a fridge freezer, and the flames engulfed the tower block because it was covered in flammable cladding. yeah, hello, hi. there's a fire in grenfell tower. this was the first 999 call of the night. there were hundreds more. quick, quick, quick! and for nearly two hours, residents were told to stay put. and while firefighters were commended for their extraordinary courage, the report concludes the brigade‘s planning for a fire like grenfell was gravely inadequate. today, the head of the fire service, dany cotton, personally apologised. we will never, ever forget the tragedy of that night. the 72 lives lost, were just the worst thing ever and london fire brigade are truly sorry we could not have saved everyone‘s life that night. the commissioner was criticised for her evidence in the enquiry when she said she would not have done anything differently that night.

the report now, in black and white, shows what went wrong. so it's time the commissioner of the london fire brigade takes ownership of that, of those failings. the report says important steps must be taken to improve fire safety without delay. and it calls on the government to develop new guidelines for evacuating high—rise residential buildings. i ask colleagues to rise to observe a minute's silence. today in the commons, mps remembered those who died. we promise them the truth, we we re died. we promise them the truth, we were then the truth and to the double country, double world is finally hearing the truth about what happened at grenfell tower on the 14th of june 2017.

does draw that conclusion, it it. so you do not accept that fine. you get to just retire in six months with a pension pot that is estimated to be £2 million. as you know the systemic failures that have been recognised are things that have been going on the number of years. since i had been commissioner, since 2017, i have been taking steps to look at areas we can address and do differently. obviously i have been a firefighter for 32 years and committed to protecting the people of london for 32 years and will retire after that knowing i will do my very best to pass on the legacy that we will continue to develop london fire brigade. do you think it is fair that the fire brigade are facing this level of criticism at the moment? what i find is really upsetting is that my individual firefighters, control

officers and officers are being named and facing this criticism when truly what we want is to prevent it happening. but we think phase two that talks about the building and fire safety regulation, that really needs to happen to understand what happened on that night. some of the criticism i find is not fair for my firefighters. do you now regret saying that planning for something like this was akin to planning for a rocket to land on the shard? what i was trying to explain was that london fire brigade is continually planning, exercising and training for risk in london. we reflect the risk we know about so we do a lot of training now for things like terrorism but for us that building should never have behaved in that way. the way it failed and the way it behaved truly let down the people of grenfell. do you accept there were institutional failures and systemic failures and that the planning was unacceptable? i absolutely accept that london fire brigade has failures and we have lessons to learn

and i fully intend going forward from here, not only the work we are already doing, but we have a detailed look at the recommendations of the phase one inquiry is a priority for us. how has grenfell left you personally feeling, how has all the criticism affected you? i think i have been left with the feeling of utter sadness and devastation that any community should have faced what they did on that night. the loss of the 72 lives will live with me forever but equally the heroic efforts of my firefighters time and time again who put their own lives on the line to save those people will live with me forever. 0k, thank you very much. thank you. at times both head of the london fire deep think it is here that the london fire brigade has been scapegoated in this report is.
