double the number of americans that have to take statins, dr. siegal, let's start with who could be affected by this. >> welcome back. first of all, jamie, i don't like guidelines in general. i go into the office and get this question every day, should by on statins? they are cholesterol-forming drugs that block the amount of enzymes that are in the liver. they are used to prevent second heart attacks and strokes. so everyone agrees those who have had a heart attack should be on statin drugs. but the question is, should you be on it if you're at risk for heart disease? previously the american heart association and the american college of cardiology have said mainly, let's go by the numbers. if your cholesterol is too high, we'll put you on a statin. now they are moving in the direction of, what's your risk factor? >> diabetic?

family history, maybe? >> are you male? do you smoke? are you obese? do you have diabetes? smoking being probably the biggest, these are the things they are focusing on now. but you know what? i still say it has to be a doctor and a patient. and something we have emphasized top show here, and david will get into this, too, what are we doing first? you may be at risk, and let me explain that. what do i mean by risk? we snow statin drugs retard plaques in the coronary arteries. if you have heart disease, it will prevent you from having a second heart attack. but people at risk for heart disease are already forms those plaques, but they just don't know it yet. the first way to address it is to improve diet, the mediterranean diet we talk about on the show, get your blood pressure down, lose weight. you do all of those things and you're not successful, then i'm thinking statins. >> that's great that you address that, dr. siegal, and maybe you can address this, dr. samadi,

welcome back. >> thank you. jamie, there's going to be a whole change and shift in the way we're going to be treating cholesterol in this country. what they are really saying is that so far there's been a campaign about numbers. so we wanted your cholesterol to be lower than 200. your ldl to be there and your good cholesterol below 60. now they want him to change. and think about the campaign for risk. what does that mean? if your ldl, the bad cholesterol, is over 190, you're automatically going to go on some of these medications. if you have a heart disease or stroke or history of this, which is where the history of physicals comes in, jamie. you'll be 45 to 75 and at risk

for diabetes, you're automatically going to go on it. and the biggest, important part we need to know is something called risk stratification which mark was just talking about. what does that mean? your risk of heart attack or stroke, if it is more than 7.5%, there's going to be a calculator. what's in the calculator? general etics, race, all of them go into the normal gram and will spit out saying, this guy should be on statins. >> i want to ask you about the side effects because we hear a lot of problems about leg cramps and deterioration of your muscular tendons. >> it can also increase the risk of diabetes, but marc will tell you overall it's a safe medication. >> here's the thing, the muscle problems are the most common, but i can go with that. if i decide you need a statin and are having muscle aches, i may be able to change you to another statin. those are pretty minor and easy to adjust to.

diabetes, there's a risk of diabetes. i believe those studies. minor risk, but jamie, the risk of having a problem from diabetes causing heart disease without the statin is much, must have greater. so diabetics should get statin drugs. >> let me just say one other thing, this partly comes from cardiologists using medications like this, for example, to lower ldl, but it doesn't reduce the risk of stroke and heart disease. but the big thing is, is it really the cholesterol we are going after or the component of information. and some problem in the vessels. and now statins reduce the clotting and the formation. that's the big news. >> they have shown angry plaques, and that's what led to this kind of thinking. but i also want to say, 190 is too high. you have to look patient by patient. if you don't see your doctor and he's looking it up, you want a doctor that looks at you the way we do. works with the patient and personalizes it.

>> good information. >> besides statins, another issue is right mines. apparently there's no issue to show that vitamins can prevent heart disease or cancer. so taking my flinstones and so forth, no use? what about this? >> this is also big news. for years we have said vitamins are great for you to boost your immune system and reduces cancer and heart disease. now all of a sudden we're going to change all of that. the truth is the u.s. task force has looked at 24 major studies, eric, across the board. is there real data to show there's a reduction in heart disease and stroke? and unfortunately the answer is no. we have said many times on this show vitamin a, c and e reduces cancer. fish oil reduces heart disease. i have to tell you that it doesn't work. so without getting into

portions, i truly believe this would help. >> there's a homework assignment here, i see. >> kidding aside, vitamin d and calcium reduce colon cancer. that's what i want people to know. personally, i love l-argenin. >> l-argenin? >> yes, and i will post this on my facebookment and the other one is, tell me, you have the prescription. >> it's my prescription. we are going to post it. >> the vitamin vegetable, vitamin fruit, vitamin vegetable, this is biotene. these are the bad sivitamins, b i want to keep the hair that i currently have. these are the ones we don't like. vitamin e, we found out this increases prostate cancer.

be betakerotin causes cancer. these are drugs, talk to your doctor. the be careful. >> i'm still going to pop my vitamins. >> first of all, i agree with your point to check with the doctor because these are chemicals. vitamin a, vitamin d, these vitamins you actually can get to a tragic range. they were showing on the video there of vitamin d 3 and b-12. you should take patients that come in the office for vitamin d, which is more like a hormone than a vitamin. you have to make sure you're not low in vitamin d, especially in climates where you're not getting enough sun. inadequate absorption. vitamin c before i got pregnant with him. the key here today is multivitamins, unless you have nutritional ze efficiencies. do not reduce your rate of

post them on their facebook pages. dr. siegal, when you see patients sometimes, is that a question you ask, were you born prematurely? >> absolutely. it's something you see in the early effects of life. i want to give a shout-out to the march of dimes that has been around since 1938. they have done more good than any other organization i have seen. they are focusing this month on the issue of 37 to 39 weeks, and they are saying, look, we may be saying that 37 weeks is full-term, but do not have your babies between 37 and 39 weeks, because you still could have problems with breathing, with development, with maturity. i don't want to scare anyone out there, but 37 to 39 weeks, don't go scheduling your c-section or having an induction. granted, most of the problems, jamie, are way before that, but at 37 wreaks you see issues with

prematurity and some of the things you see with developmental delays and problems with breathing and ending up with other issues. by going for your check-ups as soon as you know you're pregnant, by taking the prenatal vitamins we talked about, and if you're having problems like cramps or bleeding, anything where you're not feeling like yourself, get in to see your obstetrician. >> dr. >> the truth is that it's really ten months. and you have to go, if it's a marathon, the pregnancy would be the last month at 40 weeks. the closer you get to that, the better it is for the child. and the whole kangaroo mothering is extremely important. the more times a baby stays in the uterus, the better it's going to be. one-third of brain activity is formed between 35 weeks and 40 weeks. and that's why it is so

important, as marc mentioned, don't schedule the c-sections because you're ready, that doesn't make sense. that's when the baby is going to get all their nutrition the last few months in the brain, and that's important. and this foundation has helped tremendously. we have about 500,000 kids that are premature. and hopefully with these kind of segments we can help a lot of people. go for prenatal testing, education, et cetera. >> don't drink or smoke. >> exactly. >> i was a preemie myself. >> well, if you quit smoking, we'll get to that in a second. >> sounds good to me. we'll talk about smoking coming up next, the problems with smoking, and if you would quit, does that really mean it's fine? the doctors will tell us.

back now with "sunday housecall" and should i back to "sunday housecall." and should i worry? the first few ask, i stopped smoking a year ago, should i still worry about my heart and lungs? another viewer writes, i don't smoke, but my husband does. should i worry? dr. samadi, are they in the clear? >> hopefully this is the same person, so i'm going to combine the questions. first, i congratulate the person for stopping it. then i would say i'm disappointed your husband is still smoking because that's secondhand smoking and you're still in trouble. but when you quit smoking, you know, you have the nicotine, that's going to be out of your system for a couple weeks or so, so that's the good news. most doctors from medical school, remember the effects of smoking will stay in your system for years. some say for up to 20 years. so the sooner you quit, the

better it is. i don't have to, you don't have to be a doctor to know smoking is not good for you. besides lung cancer and heart disease, which will increase it by 25%, bladder cancer is one of the things we see in my practice all the time as a result of blood in the urine and there is a polyp in the blooder and that is not good. put pressure on your husband to put pressure. now. that can change your lives. >> dr. siegel you quit smoking and you thing you are okay. >> i that will anyone to go to anatomy level, the first thing we see is a black lung and i cannot believe it is a lung that is how bad it is. if you quit, again, i congratulation you the risk of heart disease goes down 50 percent the first year. the risk of lung cancer goes down and emphasize goes down. if you are stuck in a house with a spouse smoking you have more problems because 45,000 death as year from people who get heart disease because their partner was smoking. the smoke that is in the air can

dr. samadi could remotely operate on someone in another country? >> good question. first step is you send doctors. better, if you can do it by the internet, by the web, by video conferencing, we have devices that take your ekg and beam it across the world and you can be in an operating room. david will probably some day soon be able to operate remotely from leader and help someone over there. >> you remember that a couple of years ago when i was this we did our teleconferencing and i was here, we were giving licktures and operating and they were able to see it. from there we went to telementorring so on our ipad we can draw it and they can see it and the final stage is telesurgery and the whole idea of doing this from 10' away is coming through. >> it is amazing patients will have the benefit of doctors world white. we appreciate the benefit of having both of you.
