hello. i'm arthel neville. time now for "sunday housecall." >> and i'm eric shawn. welcome as always. joining us is dr. marc siegel. professor of medicine at nyu's langone medical center and the author of "unlocking the secret code of sibness and health." >> and dr. david samadi, chairman and professor of urology at lennox hill hospital and chief of robotic surgery. hello. >> good morning. how are you? >> great to see you. >> good to see you. i want to start here today. there's a disturbing new report about measles so the centers for disease control revealed that the u.s. has seen more measles cases this year than since the

disease was eliminated back in 2000. dr. siegel, i'll start with you. why the resurgence? >> the cdc put out a chart which we can show that shows 288 cases already this year. as you said, in 2,000, there were no naturally occurring cases in the u.s. as all, but the problem is the issue regarding vaccination, okay? arthel, we have about a 90% compliance with vaccination here of the mmr vaccine which is a live virus vaccine, do the not have thimerosal in it and never associated with autism though fear mongers have gotten into this. this vaccine has helped to stamp out measles worldwide. we had over 500,000 deaths from measles worldwide back in 2000 n.2010 it was down to 100,000. this vaccine saves lives. measles is a respiratory virus. it's easy, easy to transmit it. can you cough it. you can sneeze it. can you transmit it before you even know you have it because that rash you get you get later on in the game. you don't even know that you've

transmit it had because the rash comes later. so if we don't vaccinate people we're going to end up with a resurgence. now, this is about something called clusters. if you have it in one area, you're not going to get it remotely on the other side of the country. here's what happened this year. some amish mercenaries went to the philippines with this 34,000 cases over the past six months, and they hadn't been vaccinated because they don't believe it in. they brought it back, and so in ohio we have almost half. cases. i want to ask dr. mad is' question. >> i would like to ask him a question. >> go ahead. >> you're talking about getting vaccinated. what if i got vaccinated 30 years ago. should i get the measles shot again? >> that's an excellent question and the big message out there is if you're not sure whether you got vaccinated or not and you're about to travel to one of those countries, arthel, go ahead and get another vaccination. the other -- >> what if i'm not going to that country and staying here at home? >> if you've gotten the vaccination then you're fine, then you're okay. the vaccination is recommended for children between the age of

12 months to 15 months, and the second boost is between the age of 4 to 6 years, but if you're planning to go to those countries over the age of six month, go ahead and get vaccination. that's what cdc is recommending. when you think of measles, what comes to mind, a young child, right. 50% of these measles, it's a month to even 65 years old. even adults are getting these. how do you know you have it, runny nose, fever, headache, acting like a flu except a couple days after you'll have a red rash all over the body and then some lesions in the mouth, et cetera, but i agree with marc that this vaccination is important, and 90% of the people who got this didn't get the vaccination. now, here's a big question, and you guys have been in the midst of this discussion is the vaccine safe or not? does it cause autism? personally i've said there's no scientific data to back it up to show that there's a risk of getting autism.

since they took it out ten years ago, it's -- >> i want to clarify, you're talking about the mmr vaccine? >> the mmr doesn't have thimerosal in it. other vaccines which also caused fear because of the mercury issue, they have never been shown to cause autism either n.africa there's a 74% vaccination rate with mmr. in india it's 75%. you know what? guess what, arthel? there's a ton of measles in those countries. here it's over 90%, but what worries me is the vaccination rate is going down. everyone out there has got to get their kid vaccinated before school. >> i remember getting vaccinated, polka dot marks all over your face and it kind of hurts, and you talk about missionaries who went over and come back with it, and if you had this shot when you were a kid, as you say, is it now a lifetime protection? do i have to worry about getting another measles shot? >> you don't need another

measles shot because you have the antibodies and you're covering and the point that marc is bringing up is very, very important. so the pockets are in ohio, orange county, california, 28 cases in new york, and that's because those people don't believe in vaccination, and i think celebrities are not doing a good job here. jenny mccarthy came and has become the face of autism and has not done a favor to some people who do not know whether to get this or not. this vaccine is safe. when you talk, arthel, about autism, there's a lot of environmental toxins and other things that could be doing this. the message to a lot of people is get vaccines. it saves people. 1 out of 1,000 people can die from this. there's no reason. >> can die from the measles? >> yes, sir. >> i'm much more afraid of the disease than i am the vaccine. fear is an operative here. be afraid of the disease. >> what besides the obvious of staying away from someone who has the measles, how do you prevent, if you could, from

getting it in. >> you have to be aware. this show is going to help today because you have to be aware that you can actually get the respiratory problems. one more thing we didn't say, red eyes. red eyes is very characteristic of measles and it's very early in the game. if you're getting sniffles and headache and fever and you get the red eyes, got to be isolated. to your point, arthel, we really don't have a treatment for it. we only have that vaccine. >> you know something, your point is well taken because if you're in pre-school and some patients are going to listen to some of these celebrities, saying, you know what, i don't want to put my kid in danger, your child is now at risk so you can't tell them to wear these masks or anything like that. this is a preventable one. if you're traveling you should absolutely get the vaccine and it's treatable. >> great advice. thank you. >> ever go to the supermarket, the grocery store or a restaurant and they say, yes, we have gluten-free pasta. is it a fad or something that's real to be concerned about? coming up here on "sunday housecall" we'll talk about the

mmm. amazing. yeah, i get that a lot. alka seltzer heartburn reliefchews. enjoy the relief. and back now to "sunday housecall." it's gotten a lot more popular lately, people going gluten-free. you've seen those signs and the things on the boxes that say, yes, it's gluten-free. what exactly is gluten? do you really have to give it up? dr. samadi, everyone is talking about it, a restaurant near me, big sign, new york's number one gluten-free restaurant? it's all over. >> and it's packed. >> it's everywhere. >> is it a scam or is it real? >> i get all the hard questions. gluten is a protein that you find in wheat, barley and wheat so basically the purpose -- it has a purpose in food, supposed

to keep everything glued together. now, gluten-free, obviously if you're really allergic to it. people who celiac disease so gluten is known as like a, you know, foreign protein. your immune system is going to react to it and it affects your small intestine and then you have all the bloating and fatigue. >> how do you know you've got it? >> every time you have some food, you'll feed tired and you won't get the nutrients through the small intestine so you'll have osteoporosis and you'll lose hair and be fatigued so your liver and bone is going to be fatigued, so for celiac disease, people who have a diagnosis of gluten allergies, done by blood tests, 3 million out there, stay away geleten and go ahead and have gluten free, that's all gout. we're worried about a $16 billion industry, people are saying i had gluten-free, i feel

great, why don't you. marc is going to make more comments. >> dr. siegel, what if you're not a celiac and you're like i'm going to go gluten-free because it's so popular. could you possibly miss something out of your diet? >> the word that you can miss is something called fiber because when you take gluten out of your diet in rye and barley and wheat you often lose the fiber we need in grains. now, the reason that this is happening is because it's all in the media, and our job here is to give corrective information so i have one word for you guys out there today, instead of placebo it's a no cebo effect. if you take it out of your diet because everybody else took it out of your diet then it's helping you, and a study in australia, the guy in australia who originally said there's something called gluten sensitivity, not celiac, the same guy who in 2011 started this craze because with celiac disease, again, i check blood in my office, if i find an be a

normity i have an endoscopy done and look to see if the intestines is in trouble and if the crips that you need for on sorpgs is gone. and there's another 23 million we wonder if they have an allergy and the guy in australia said maybe it's a problem with sugar. maybe it's not geleten. maybe it's hard-to-absorb sugar and did a same study but it looks now that a number of people we thought had gluten sensitivity and have glucose problems so 11% of american household are gluten-free, i say that is too much because they lose the fiber. >> let's say you go gluten-free for a short period of of time, maybe you want to lose weight, cutting out bread, and how long is okay for little bit of a weight loss had. >> if you're doing it because somebody told you to do it it's not good enough because as marc mentioned, when you take gluten out, the companies have to put something else to really stick the food together. what do they put in, they put

sugar, they will put some fat and take the fiber out and over time we'll have the same problem of gaining weight, not losing weight. that's number one. number two. this australian group who brought gluten-free to the surface in 2011 now recently this week they came up with another study saying, you know what, there could be another player, not gluten alone, that could be causing this, something called fructan, this is a sugar and when you eat it, in an apple, would you know that apple has that kind of sugar not good for you, milk, cheese, a lot of foods that you eat, the bacteria in the gut can ferment them, cause gas and bloating and the big message for people, if you have celiac disease, go gluten free, if you're not don't even think about it because it won't even help you. >> i don't want to eat anything now. >> i'm sorry, dr. siegel, i've got to go. >> if your bowels are not looked at get them checked by your

time now for our should i worry with our weekly segment about everything that worries us. one of our viewers wrote in saying i've been losing a lot of weight over the past two months. should i wore? dr. siegel, kroon if it's a male or female, but does it matter? >> first of all, no. it's the same for both. i worry if any of my patients are losing weight. now what do i mean by weight? five to ten pounds over several

months is what i look for, and the first question i will ask you are you depressed? are you eating what you usually eat? is the amount that you're getting in going down, that's number one. number two what, medications are you on because many medications can cause this as a side effect. then i look for thyroid disease. >> medications like what. >> just to give you an example of what happened recently. i had a patient on a chemotherapy for cancer and they said i'm lose weight, must be the cancer. no, your tests just showed that the cancer gone. it was actually the chemotherapy that they had taken so many medications, you have to look and your medication to see if it causes weight loss. many do, but amphetamines do, speed does, cold medications do, thyroid medications can if they are not adjusted properly and thyroid disease can cause you to lose weight. the big "c" is what everybody is worried about. rule out the other things first. bowel infections can cause

weight loss. >> asking about a parasite, what are they and do you have to go from a jungle? >> eating a lot of fish. >> i don't want to scare people eating sushy. >> don't you sushi. >> uncooked pork can give you tapeworm. >> how do you know if you have a parasite? >> if you're losing weight and having g.i. symptoms, i'm going to check your stool for parasite. >> speaking of stool and g.i., there are people who take laxatives for various reasons. i don't know, am i off base here? >> no, you're absolutely on the money. and i'll send all the patients with parasites to dr. siegel so i can concentrate on the patient with laxatives. before we get to that, as a cancer doctor, the first person that comes to mind seeing massive weight loss the first thing that comes to mind is does this guy quietly in my mind does he have cancer, e soft gus

cancer. >> pancreatic cancer. >> pancreatic cancer. these are really red flag. you want to make sure you really do all the studies, maybe get a cat scan to find out what's going on. you're right, unfortunately a lot of teenagers are using laxatives to lose weight. and that's quite dangerous. so that's a question in the history of physical we do. hyperthyroid, too much thyroid it increases your metabolism, you could lose a lot of weight, you could lose hair and start panicking, et cetera. and medications mark talked about. for example, diuretics, somebody has high blood pressure, go on diuretics, lose a lot of salt and water quickly. be careful. and crohn's and constipation, diarrh diarrhea, back and forth, et cetera. but as a surgeon we see a lot of lap bands.

don't forget these are new things and the patient is like 300 pounds and a few months after lost a hundred pounds. >> i want to emphasize two more. one is stress can cause you to lose weight because you're becoming hypermetabolic. your metabolism is zooming up. diabetes can cause you to lose weight because you're not absorbing food as well. and i want to emphasize one david mentioned, colon cancer is a huge problem in the united states. but we can screen for it. so get in there and get checked. >> while you look for parasite. it's true. look for parasite check for blood in the stool too. >> that's right. >> so important. you're right, doctor, absolutely. well, do you have an overactive bladder? coming up our doctors are going to talk about that. and we'll explain all the trips to the bathroom late at night.

it's a condition that impacts more than 30 million americans, but if you suffer lying overactive bladder, you down. doctor, tmi alert here, i actually, you know, probably go to the restroom like two or three times a night. i know my reason is because i have thyroid -- they push on the bladder. >> got to go, got to go, got to go. >> yeah, it's annoying. people who have it they know it's annoying. what causes it other than fibroid. >> you give the diagnosis away.

in your case, even though you're not my patient, fibroid's push against the bladder and cause some of that. anything in that area that causes inflammation. in women we see mostly cystitis, inflammation of the bladder, occasionally urinary tract infection or after pregnancy the whole area of the pelvic muscles are out of sync and they can have overactive bladder. sometimes we have no real diagnosis. ideopathic. men with large prostate the bladder is trying to overwork to get the urine out. they may have bladder cancer, bladder stone or urinary tract infection. typically what i would do in something like this is check the urine first. simple thing come to a urologist, check the urine, send it to sightology to rule out cancer. and then at some point scope the patient to find out what the bladder looks like. we get tremendous amount of information by looking in the

bladder and come up with the diagnosis. at the end if i can't really come up with a real diagnosis, we have a urine dynamics. it's a computerized diagnostic tool we use to find out is it prostate, bladder, myoma -- >> dr. siegel, for men we're told is it a myth or real? as we get older we're going to start going to bathroom at night. >> that's true because your prostate is likely going to be getting larger. the big question we ask is what's the change in trend? how many times you go in now and how many times did you go in last year? as an internist i want to rule out diabetes before i get -- make sure it's not a medical condition. >> it makes you go more? >> yes, because of the amount of sugar. it could be adjacent areas. might be a problem with the bowels pushing on the bladder. problem with the prostate pushing on the bladder. what do i want people to do about it? number one, cut down on caffeine. number two, cut down on alcohol. number three, increase exercise

and lose weight. that's regardless of what the causes are. that will help. there are medications that we use, david uses a lot more than me, detrol, they relax the muscles. >> i think this is a good point because i think we overtreat -- i'm glad you brought this us. we overtreat patients with medications. if you ask the patient, well, i drink ten glasses a day, i have beer, all of these diuretics can make you go often. so if you rule that out, then there's medications. also i want to make sure something, biofeedback for women and men works really well for a lot of people that have overactive, biofeedback is -- they will teach you how to do exercises. >> after pregnancy you rebuild the pelvis -- the muscles in the pelvis with those exercises. >> i didn't say i didn't like them. i just had a thought we're not going to discuss here. moving on. you're not getting us into
