got nothing. dr. sammed ay, what's first on the list? >> obviously this is a very serious problem because it is a leading cause of death among men and women. we have 720,000 deaths and heart attacks as a result this. i'm glad we are all wearing the red color as of bringing awareness in this situation. i always talk about knew monics because i think it's always easy to remember exactly what we're talking about. i think a -- and i'll post this on line. a is for ask your doctor whether you need aspirin or not. aspirin is a big topic i'm going to ask marc who should be on aspirin and who should not. i think a third of the country are suffering from metabolic syndrome. what exactly are we talking about? metabolic syndrome is blood pressure, close troll problems and obesity which leads to stroke and heart disease. know your blood pressure.

we are not health care people. what do i mean by that? we need to change the behavior of people, ar they will. that means as a child we need to educate our people. >> it's hard, you talk about exercise it every week. you don't always exercise. i walk a lot. >> yeah, because you live in new york city, but if you didn't live here you probably wouldn't walk. >> get in the car, go to the mall, walk down at the mall -- it's hard for people. >> you are coming to the heart of it. at a time when heart disease deaths are going down, obesity and diabetes is going up. we are talking about three tiny arteries on the hart. i wish you could see how spall they are. they are so small. they get clogged you have got a problem. and it's easy to get them clogged. what can we do? prevention is key. patients come to see me, i want to know cholesterol, blood pressure, and when they are kperizing, and what are you doing when you are not exercising? are you sitting on your duff in

front of the television? that's a huge rediktor of heart disease and stroke. we want to know the blood pressure, how many fruits and vegetables you are eating. we talked about the medtarian diet. that's key. get rid of all the processed food with the should goers and the salts in them. i want to talk about aspirin for a second. i have to identify you as being in a high risk group as we just said, high cholesterol, high blood pressure, you know, if i'm treating you for cholesterol and high blood pressure and you have a family history of heart disease and you are a smoker -- number one risk of heart disease is a smoker -- i might into decide to put you on a baby aspirin a day. of course if you have had a heart attack before, i definite will he will. >> dr. marc a lot of people don't know when they've had a heart attack. is that true? >> especially. i want to point that out. especially because it was go red for women yesterday.

women especially don't necessarily know. we all know about the crushing chest pain like an elephant of the sitting on your chest. did you know you could have a heart attack just by being fatigued or or being nauseous or vomiting or you have a sensation in your left arm or jaw or your neck or left shoulder. that can be the only warning sign. >> or the feeling like an elephant is sitting on your chest and the pain is radiating to your left and you have to call 911. >> if you think you are having a heart attack, should you pop an aspirin? >> at that point it may prevent some of the dissolving of the clogs and prevent the at of the clogs to go up? >> if you are having chest pain radiating down your left arm and you are short of breath, puffing, you have to have an aspirin. >> let me mention something. we keep talking about all of this stuff, but the question is whether we are getting through to people and they are learning from this. i think the only way you are going to change the health care

is to go back to when the kids are 4 years old. we are looking for a lot of quick things. we are basically getting a lot of fast-food. when you add sugar salt and fat what do you get? fast-food, processed foods, and that's what you are eating. unless we start educating and prevent the disease yes we can chop the legs for diabetes, yes we can give you viagra for sexual dysfunction, yes we can do all these things but we are going going to make a difference unless we go for prevention and wellness. that's the message this month. >> question for you on this, a man, is there a warning sign for men in men's health to say they have heart disease in terms of the your logic function? >> this is a very important point that you are bringing up. the very first sign of heart athat can in young men is sexual dysfunction because the same arteries hare bringing the blood to the heart is bringing it to the peen i will tissue or the sexual organ. when i see a 40-year-old obese

especially african-american who is at high risk of cardiac, and he says doc by sex life isn't the same i send him to marc and say get your ekg, get all the labs and make sure this guy is not going to have a heart attack. and by losing weight and everything else we can help him. >> one final point we didn't cover, stress. i can't prove this to you but i think stress, stress, stress increases your risk of heart disease. try yoga. >> have you noticed how much he is smiling more because he is sitting next to you. >> it's not you, it's me. whatever. no, i did know that the women are really unaware a lot of times they are having heart attacks because i did work for go red for women in the harlem chapter and i learned that from many of the survivors. >> i coment you for also wearing the dress. it is symbolic and brings a lot of awareness but they are important points that people need to talk about. >> one point about women, which is after menopause your estrogen

shows secondhand smoke still affects one in four nonnoek smokers. what's more, they say it's the most vulnerable members of our society who at the most risk from second hand smoke. >> rates are alarmingly high among certain populations. there is currently 58 million americans including 15 million children that continue to be exposed to seconds hand smoke. >> the cdc says there is no safe amount of second hand smoke. children can develop illnesses. when they reach heart disease, strokes and lung cancer. worst of all, they are likely to roll model their parents, and become smokers themselves. doctor suzanne mccally, chief of pulmofollowing at a children's hospital in chicago has many years of experience treating sick children who are victims of second hand smoke. she teaches families ways to overcome the problem often sharing an emotional experience she had when caring for one

child with cystic fine roeszis whose grandfather was unwittingly subjecting him to cigarette smoke. i said to him i know that you know smoking affects your health. but did you know that it affects your grandson's health? >> and he was not aware of that. and as soon as he learned that, he made a commitment to quit smoking. he quit, and he has been smoke free for over ten years. >> dr. sammed ay you hear about secondhand smoke but you donkno can affect children. >> that's certainly -- i'm glad that marc is bringing this to the surface. it was a great report you've done. i think it's absolutely important because in adults it's causing the same things that we spoke about, which is heart disease and lung cancer. about 7,000 cases related to second hand smoking. 41,000 cases of heart and lung disease. which is significant. >> from second hand smoke, not even from you smoking? >> exactly right. but the good news is that the level of the chemicals in cars

jens is actually decreasing over the last decade. we started in 1991 about 80%. 2007 was about 40%. now in 2011 it's about 25%. all of these ads about the lady with the hole here, all these ads are encouraging people to walk away from smoeg. make sure you don't smart smoking and educate your children to look out for second hands smoking. we've heard about sids, it is a real problem. i'm glad you are spending the time bringing this to the surface. >> i have is a couple of additional points. mccauley told a personal story. she says there was a child she was taking care of with cystic phi brosis and a gran father who was smoking smoking smoking and didn't realize because he is an older generation that he was impacting this kid. the kid was coming in with asthma, exacerbations getting

worse. she tells the gran father and he quits smoking because he didn't want to hurt his grand child and the kid got healthier and has just graduated high school recently. there is a message there. and we have to educate people. 26 cities have bans on public smoking -- i'll sorry, 26 states have bans on public smoking. that means that over 20 states don't have bans on public smoking. 700 cities have bans on public smoking. but there are many cities that don't. what about the two-family dwellings the skrrks dc was focusing on that. someone smoking like a chimney that lives next to you, it's coming through the walls. >> if you 134e8 that, is the smell enough to potentially impact your health? >> you are smelling it it's impacting your health. any a. secondhand smoke whatsoever can increase your risk of developing heart disease long term, of developing lung cancer long term. and short-term, exacerbating your asthma and landing you in the hospital. third hand smoke, which the it

lives on the furniture. that's been studied as well. >> a lot of times you can smell someone's clothes. >> absolutely absolutely. >> what's going on is there are certain categories of people are at higher risk. so children between the age of 3 to 11, construction workers, blue collar workers, african-americans, and also the salary, the amount, the poverty level plays a big role. and we haven't made a huge impact in those categories. overall the level secondhand smoking and smoking in basketball is way down. so quit smoking, look for restaurants, places that really ban this kind of stuff. if you are not going to go there by force, they are going to change toor their policies. if you are in the car and somebody is smoking it's going to affect you. so it is a real risk. >> absolutely. two final points. one is that 40% of children are stillics posed to second hand smoke. 70% of african-american children are exposed to secondhand smoke. and you know what the worst problem with secondhand smoke is? we haven't mentioned it yet. if if your parcht or someone

around you is smoking you are very likely to do-it-yourself. then you become the smocker. >> so teaching your children not to start and be a role model is exactly what you were trying to bring in. you have a comment. >> if you do smoke, you may have a residual effect. you could have stopped smoking 20 years ago and still ends up with lung cancer. can that happen with second hand smoke. >> once you quit it can take ten to 15 years before the effectiveness wears off. we talk about cardiologist but we see urine diseases and other diseases. >> here is a stat particular. it wears off but not completely. if you smoked 20 years ago i have to check. >> i'm saying if you didn't smoke. >> it doesn't wear off. none of it wears off but it diminishes over time. here is a dramatic statistic. the risk of heart disease is one

half of the yethe year after yo than it was while you were smoking. >> i know there are e cigarettes. don't go to e cigarettes as a segue to quit. that's going to take you to a whole different thing. e cigarettes are still controversial. we still don't know the data on that. don't get into the whole e cigarette thing. >> i have found that it works in some smokers. >> we can debate about that. >> just don't fight. >> i won't. i choked him last time >> remember chocolate cigarettes when we were kids. >> and the bubble gum cigarettes. >> i agree with him on all cigarettes. they can be problematic. researchers uncovering a better way to detect high risk prostate cancer. we'll tell you all about it. and why they say this new method is a lot more accurate. diabetic nerve pain.

not. the way we often did it was random biopsy. i would put a probe near the prostate, look at the prostate and take 12 or 14 samples to find out if there is any cancer. but the key word is random. we didn't know exactly where we were going. the new way is called mri guided. we are taking a picture, an mri of the prostate and fuse them with our ultrasounds pictures and find out where the hot spots are and just target the diagnostic tools to go over the areas where it looks suspicious rather than randomly. and we are finding out more aggressive cancers those are the ones that need to be treated as opposed to low risk ones. so it helps us in that. it is a great way to distinguish who should get biopsy and who shou should not. it is a great help. >> biopsy you take a clip of the tissue. how do you go in and get night usually i have the latest model

euro and a half in new york city. this is like the mri where i give lidocaine around the prostate. it takes 10 to 15 minutes. fairly simple. it sounds awkward but it's easy to do. and the risk of infection is 2 to 3%. so it is a fairly benign process. >> marc, when you go to the doctor and they give you the examine that's important, too. >> i think it is really important. and whenever we do a your logical topic and i review something like this i can't believe the numbers. i can't believe there is a debate here. because 27500 men die of prostate cancer every year. it is the number two cause of death from cancer in men after lung cancer. everyone knows you want to catch lung cancer early. i think you need to catch prostate cancer early. how do i do that as an internist. you mentioned one that a lot of internists have gotten away from. you have to do the internal rectal examine and feel for a ridge on nodule. we get to feel about 30% of the prostate.

if i feel abnormality i send the patient to the a urologist. also the psa, i follow a trend. if there is an an normal friend and i can't figure out something else, sends the patient to a urologist and consider a biopsy. mris are getting more sophisticated. the more accurate they are the more they help guided biopsies. >> we are going to take a break and talk more about the psa and what it means when we come back. , when we come back. can this decadent, fruit topped pastry... ...with indulgent streusel crumble, be from... fiber one. fiber one streusel.
