time for "sunday housecall." >> and i'm molly line. welcome. joining us dr. david samadi chairman and professor of urology at lenox hill hospital and chief of robotic surgery. >> and dr. mark siegel author of "unlocking the code of sickness and health." >> thanks for being here. >> gentlemen, our first topic here is as the drug dubbed female viagra gets the green light from the fda is likening that pill to its male court parent. is that misleading? let's start with dr. samadi, is it misleading to say that this is similar to viagra? >> yes. would i certainly not use the term female viagra, because as a urologist we prescribe a lot of viagra, stylies and levitra.

for men we're talking about the mechanical problem and below the belt. basically viagra brings blood flow and they can have great sexual activity and it works very well. take this medication as needed, when we prescribe viagra. this is a totally different medication and that's why i would not call it female viagra. sex among women is very complex. it's also mental and what's up there, the self-image of women and how they feel about themselves and the partner and this particular medication i would add was approved after two rejection periods by the fda works on the brain, serotonin and dopamine and originally prescribed as anti-depressant. the question is does it make them feel better and that's why there's an increase in libido t.only works for 10% of the time and it's on pre-menopausal, very important, not post-menopausapo. pre-menopausal women who have

this particular disease means hypoactive, low libido sexual drive disorder and that's what i want marc to describe because it's a very controversial diagnosis. >> how does it work? goes to the brain. does it do anything like viagra does in men? >> absolutely not. viagra is below the belt and works on increasing blood flow to the area and in fact can work in some women as men. women on anti-depressants i find off label it tends to work sometimes there. totally different concept. here you -- >> what does off label mean? >> off label means the fda has approved it for use in men, but you can use it for certain women. this time we're talking about a libido drug, first ever. i would call it a pioneer drug, the first ever libido drug, and eric, here's your million dollar question. it will be in stores on october 17th. >> okay. >> exact date. you always ask when is this going to be available, october 17th? the problem with this drug is it only has a very minor effect. it increases sexually satisfying

experiences by about one a month. if you have a very low libido that may be worth it, but the side effects, low blood pressure, fatigue, sleepiness, nausea and as molly was saying before we came on here, you know, alcohol. if you drink alcohol, you increase the chances you're going to black out or having a fatigue problem. well, alcohol is the usual, you know -- >> sometimes one has a glass of wine when you're out to dinner with your husband. >> to get you in the mood. how can you do it without alcohol and for doctors to be able to prescribe this, molly, eric, they have to have taken a course, online course, because of the side effects. they literally has to be a special certification, for certain people but not for everyone. >> when you look at the side effects of viagra, and when viagra came out in 1998, all the doctors, so wary of what's going to happen to these people, do you have cardiac disease and if you take it with food, with alcohol, but same thing almost with viagra and vitra, have it with food it's not going to

work. can it lower your blood pressure, sure, and if you have heart disease you should not be touching it and there are side effects but this particular drug was the significance of like a sex debate. why do we have 20 different treatments for men and never have this pink pill and that's why this company pushed the fda for whatever reason, maybe for good reason or bad reason, they jumped on it, and there's a lot of debate out there, whether people should be doing this. if it's only going to help very few people for one event a month, look, it can save a lot of marriages. people have taken this. can save a lot of marriages. >> one event per month. >> it could be a big deal. >> totally want to add it's very expensive. $200 a month and the question is going to be whether insurance covers it because in this case as opposed to viagra. viagra you take as needed. this case you have to take it every single day. >> someone comes into your office and says i want, it what are you going to say? >> if i'm certified to give it, if i've taken that online course, i'll look at their situation and think whether it may be worth it and then i'll

read the list of possible side effects. david is wright. every drug has a side effect fairly prominent and i'll go over the side effects and make an informed decision. patient-by-patient basis. >> a lot of doctors, really what they do is sexual health, and they should be involved in this particular thing because with all due respect maybe me and marc, we're not the experts in female sexual function which is extremely complex. is it hormonal, like really mental, something that has happened? maybe the woman has gained a lot of weight and all of a sudden she doesn't feel good about this, so to put them on these kind of anti-depressants may be problematic and -- and a lot of medication conflict. i also say that i think the insurance companies will cover this because if they are going to cover viagra they have to probably cover this, but it's a prolonged period, and we have to wait and see exactly what happens. >> one more point on this is that it's a ground breaking drug, a pioneer drug because it will lead to research into other drugs. >> open up other once.

patients who receive common operations in the daytime, well, they claim they fare no worse in the short term if the physician worked that hospital graveyard overnight shift the night before than patients whose doctor did not. so dr. siegel, does this make sense to you? >> well, first of all, it's a first of its kind study, and it looked at almost 40,000 patients in onterio and almost 1,500 physicians and looked at one day. you know, your doctor that day had the graveyard shift from 12:00 to 7:00 and next day had an elective procedure like a hip replacement or knee replacement or spinal surgery. how did he or she do in the complication rate was not greater, even if the surgeon worked overnight and the death rate was not higher and readmission rate was not higher. but, eric, the author of the study points out these are not trainees. we're talking about attending surgeons, like dr. samadi here, people who have experience. people who also have control of their schedule works can say i want to take a break in between surgery. i want to spread that out. it's really surprising finding,

but i think it's very, very informative, and i would ask dr. samadi whether it matters, i want to ask him one quick question. >> dr. samadi does a billion operations, i mean, what do you do? >> look, number one, you have to be very honest with your patients, and if you don't feel good or if you're tired and you've had some trauma or something that happened the night before and you have to tell them, and this has happened to me. a few years ago, i went up to my patient, just happens that he was a gynecologist, and i said today is not a good day. i don't feel good and i'm not going to perform your operation, and they respect you more. when you're honest with your patient and you tell them the truth. of course at the beginning he was upset because he was all psyched up for surgery, but it turns out not only did he amazingly well, we still laugh about that moment and he's doing very well. this study is important because years ago the same thing happened to the residents and fellows that stay overnight. they weren't doing so well so there were more complications, and the office of graduate medical office basically said you can only work for 24 hours,

and you only work for 80 hours a week and you go home. that's what happens to our residency program. today if they are at night working, the next day they go home. this particular study from canada about 40,000 patients, 1,400 doctors, is for doctors, attendings where you can actually say, you know what, i don't feel good today. i'm going to change my schedule so you have more control. basically the message to a lot of patients is when you interview the surgeon find out what does he have his own team and own anesthesiologists? when you do alo of these cases for a long time, for example, last week i had 15 major surgeries, that's a lot of surgeries so your body, just like being in the army as a surgeon, your body gets used to it. you build up the stamina. >> a couple of points, we train as doctors to be a little bit on the sleep-deprived side and still do our best and we're not talking about chronic sleep deprivation. nobody is recommending and the you had aor is very clear, nobody is recommending surgeons get four hours of sleep on a

regular basis. regular sleep is very important for pilots, surgeons and a surgeon who has done a lot of the same operations over and over it seems like at least a short-term outcome is just as good. >> you always try and you do this as a surgeon for your patients. you always get your six, seven hours of sleep, but your body, you're right, very right. your body gets used to it and you can perform. >> if you have an operation, do it like early in the morning because i think the doctor just woke up. most wide awake at like 7:00 in the morning. >> don't put it out there because a lot of patients will say i want the morning one. depends on your surgeon and how good and afternoon surgery and morning is basically the same. also play the music. remember we talked about last week. >> i asked you that and you said your afternoon operations are just as good as your morning ones but i think the music is part of it. >> he did do it blindfolded. >> coming up, new research may be saving you a trip to the gym.

not a fan of the gym. a new trial showing that exercise and dieting is no more effective than just dieting all alone because when people exercise their appetite actually increased and they eat more food so the only way to lose weight is to eat fewer calories. let's bring in dr. samadi. a lot of people will be interested to hear this because they don't want to go to the gym so much. >> or they don't have time and busy schedule don't allow and said how do i lose weight without going to the gym? the first thing is make a diary of what you put in your mouth and what you eat all the time. when you write down and see what's going on, that's already a great biofeed-back. get a nutritionist, it's important to know about calories, educate yourself about this. never skip breakfast because when you eat breakfast in the morning, you jump start your metabolism, that's important. start with a lemon water and also add green tea and coffee, that's going to increase your metabolism, that's huge. and also get your sleep.

now, something else we can do in the kitchen, get rid of all the flakes, get rid of your fork and knife and take a smaller portion dishes. i promise you, eric, this works. >> wow. >> because people have used chop sticks to just basically strug federal eating. >> so much work. >> wait for this, the biggest message is to chew your food and go slow. >> right. >> that's really important. and of course, mark is going to talk about alcohol and getting rid of all the junk, but if you do this, of course exercise is good for you, of course we encourage exercise, i promise you to lose weight, following some of the tips. >> eat less. >> well, that's part of it. the other part is more fruit and vegetables, it's fiber, it's all that substitution, if you have fruit and vegetables instead of cheeseburger, you're going to lose weight. more water, i don't agree with the experts who are saying exercise doesn't work. if you jog a mile, you're burning 100 calories, if you eat a muffin, you're eating 500, i agree with that. but if you exercise properly and

follow-up with a lot of water and say to your brain, i don't want to throw that away. look what i did with my day, started off on a treadmill or elliptical and bike, people that do that that exercise in the morning then don't immediately gorge themselves -- >> when i exercise, i crave water and fruit. >> exactly. and those are better for you. and david's point, use the chop sticks, smaller portions. get rid of the soda and fruit juice. >> be realistic and set up a reasonable goal for yourself. when you go for 30 or 40 pounds, you're not going to make if it. so i think just follow those tips, alcohol is a big-time calorie. and i think exercise is very difficult. you have to be having discipline, you run far mile, you only lose about 100 calories. that's nothing because you gain it from one bagel, et cetera, but follow this because when you drink a lot of water. it already sends a system that your stomach is full. green tea has a lot of antiox dants, increases your

metabolism. we've talked about coffee a lot. i'll post this on social media for people out there. >> about the hormones and et brain, exercise puts the right anti-hunger hormones in your brain. also some of the dietary supplements. if you eat sugar, you start to crave it and you have the smaller portions, you crave less. >> that's another big one, lack of sleep. >> and lack of alcohol. >> no muffins, you're going to make a lot of fun. >> greek yogurt is great. but use real fruit, not the additive. >> yeah, i know. >> all right. well coming up, the way you decorate your home could say a lot about you. there's something else in the home that could speak volumes, and that's the dust. the doctors will be in sunday house call when we come back.
