>> earlier today the feds released a report on the suspicious 2019 death of jeffrey epstein in a new york city prison, jonathan hunt has the latest. jonathan a lot of interesting questions tonight. >> for sure, sean. it is a damning report that was released today and raises as many questions as it answers about jeffrey epstein's death. the report by the inspector-general reveals for instance that epstein was left alone in his cell with extra bed linens that he ultimately used what has been officially been ruled a suicide. the report points out nearly all the surveillance cameras in his jail unit were not working that he wasn't check on regularly despite a suicide attempt two weeks earlier and the guards simply weren't doing their jobs. >> the combination of negligence, misconduct, out

right job performance failures documented in today's report all contributed to an environment in which arguably one of the most notorious inmates in the bop's custody was left unmonitored alone in his cell with excess prison linens there by providing him the opportunity to take his own life. >> epstein was found dead in his cell august 10th, 2019 while awaiting trial on sex trafficking charges. his cell mate was transferred out the day before that, despite warnings from prison psychologists that epstein not be left alone. while the report details how the suicide happened, sean, it does little to explain why so many mistakes were made on one night, allowing the accused sex trafficker with so many ties to the rich and powerful to effectively silence himself.
