this might be a longer game for north korea looking at biden second term and generate 2025 or perhaps a new administration. arthel: lots of calculations going on, on the part of kim jong-un own. so what do you think is going to happen in the immediate future? will he contin continue with the missile testing? what where do you see this going right now and then six months from now? and once we have a new president perhaps? >> north korea has tested their 100 missiles since he started 2022 which is a lot. they are reportedly potentially considering a nuclear test as well. all of this adds a little bit of leverage to north korea. but also they are serving china's interest and rushes as well because they are distracting from or they believe they are distracting u.s. focus from the war in ukraine. from iran's nuclear program back to the korean peninsula which actually serves their patron

24 million people are expected to see heat indexes above a 100 degrees. a new study finds the heat waves like this will add a billion dollars in health cost across the country each summer. meanwhile, there are those smoke coming in from canadian wildfires. that is set to reappear in the upper midwest. this is happening as the research at rutgers university say smoke is as bad for the lungs as secondhand smoke was in bars before smoking was banned. it may be worse than pollution from car emissions. let's bring in dr. janette nesheiwat medical news contributor. let's top start with the heat will get to the smoke later. anyone of course is at risk of heat injury. but who is most vulnerable? and how would you even know if you have a heat injury? >> hate arthel, the most vulnerable just like we saw in covid the elderly, people have

underlying medical conditions. but they keep the very young children are vulnerable, not as we saw in covid. but really come arthel come anyone can be impacted from this high heat. it is the number one a weatheweather-related killer ins country. we have about 235,000 hospital visits every year during the summer because of the extreme heat. most recently i have taken care of patient to have come in, they have fainted on the subway. they have been spending all day outside not staying hydrated. they came in with headaches and dizziness. i used to work in the er in arkansas we would take care of young college athletes who are very healthy but would be running outdoors in the prolonged heat a would come in with severe hydration and severe muscle cramps and seizures. they would we would work hard to cool them down reverse the organ failure they were suffering from. even though we have groups most vulnerable it could impact anyone. that is why it is so important

to take precautions. for example if it is hot outside, if it's approaching 85 or 90 degrees you want to try your best to stay indoors. go inside where there is air conditioning. make sure you're saying hydrated as well but not just with electrolytes. when you sweat you lose sodium, potassium and you want to replenish the electrodes as well but for some people they have to be outdoors. and they work outdoors so it is important they take breaks and wear loose clothing. sunglasses, hats, try to stay in the shade when possible. and also very important, arthel, to make sure especially if you are uncertain medications you avoid excessive alcohol and avoid extreme exercise activity outdoors in the heat. arthel: is there an optimum temperature that is best for us to be outdoors? >> it is a great point you bring up. our body is like a human while that's consulate generating lots of heat. we function best at about

70 degrees fahrenheit. once you go be on that is when we start seeing symptoms of headache, dizziness, muscle cramps, extreme thirst. some people even lose consciousness. if you're starting to feel any of the symptoms you want to get indoors right away. sip on some cool water. take a cool shower. if you do not see improvement that is when you went to seek out medical care so that you are symptoms of heat injury, of heat exhaustion do not progress to a heat stroke which can be deadly. arthel: absolutely. let's move onto the canadian wildfires now but what kind of impact, dr. janette, will be smoke and pollution have on all of us? >> a study published in nature found the wildfires smoke is one of the worst sources of pollution. especially when it comes to our respiratory system. we have highest level of hospitalization from pollution, secondary to wildfire smoke. it can be a serious, especially

of underlying medical conditions like asthma, copd, emphysema, that sort of thing. my patients who came in from the canadian wildfires come through new york they were coming and with coughing, wheezing, chest pain, shortness of breath. and their eyes were irritated as well. that is the acute symptoms we see. but long term similar to smoking cigarettes. if you smoke for 30 or 40 years you are more prone to deve developing, excuse me emphysema, copd, heart disease and even cancers. the same situation applies here. you've got to take a look at what is in the wildfire smoke? we've got the carbon monoxide, got benzene's, dioxins, you have formaldehyde and the particulate matter. everything is being burned. burnt wood, burn plastics and chemicals up or you're breathing all that in and it can cause inflammation in your body. it can cause hardening in your arm arteries, your blood vessels can lead to cancers and heart disease and lung disease in the
