palin was "dancing with the stars" over in los angeles, the tango and the cha-cha. meanwhile, here in phoenix, where sarah palin was kicking off a book tour. sarah palin signing books here for several hours. now, bristol came in third behind jennifer grey from "dirty dancing" and a short time ago, i had a chance to ask sarah palin if she was disappoint by the "dancing with the stars" reports. >> not at all. she -- she's had the journey of, you know, the challenge, and the improvement and the work ethic. this has been all about that, and perseverance. i couldn't be prouder. she's happy. she's very happy. >> reporter: work ethic, hard work. sarah palin is clearly working hard right now, working for something. the question is, for what? >> we will now reveal that the couple in third place is --

part of sarah palin's reality. >> sarah palin has perfected the reality show candidacy. not just the tlc series, which isn't all that real, but her books, her tweet iing. it all enables her to use the main stream media. >> reporter: she is everywhere, including tonight in phoenix, arizona, signing copies of her new book, which among serious policy discussion, actually including a section that deems, literally, with reality tv. this time, "american idol." she calls the bad singers on that show victims of the cult of self-esteem. no one they've encountered in their lives, from their parents to their teachers to their president, wanted them to feel bad by hearing the truth. her comments are about self-awareness and responsibility. instead of facing tough economic reality, americans are convinced, she says, that someone else will provide for

the other leading republican presidential contenders combined. >> the main why the establishment has mixed feelings about palin is, they can't control her. she goes around them, she goes over their heads. >> reporter: most of palin's reality is on sarah palin's terms. this book signing tonight is very tightly controlled. >> no bags, no books, no purse, no cameras, no cell phones. >> reporter: the big question, with all of this, the book tour, the reality shows, is, why? what does sarah palin want out of this? is she running for president? after all, her book tour includes stops in the key states of iowa, south carolina and ohio. she told barbara waters -- >> i'm looking at the lay of the land now and trying to figure that out if it's a good thing for the country, for the discourse, for my family, if it's a good thing. >> if you ran for president, could you beat barack obama? >> i believe so. >> reporter: but could she? in the latest telling poll of primary voters, palin is in the

lead. but in a head to head matchup with president obama, she fares the worst among the leading republicans, trailing by 8%. mitt romney is actually polling ahead of the president right now. >> when it comes to independent voters, those people who decide elections, she's less popular. >> reporter: what about all the twitter followers, all the eyeballs on "dancing with the stars"? >> we know what she's doing right now is helping her get more media attention and raising her profile. but we don't know that that's necessarily a good thing. >> reporter: her biggest obstacle right now? according to the latest abc news poll, 67% say she is not qualified to be president. and this reality might not help. it should be noted that ratings for "sarah palin's alaska" plunged after the first week. >> right on. >> reporter: right now, palin's support network is a group of informal advisers that communicate mostly by blackberry. it is not a typical campaign structure. >> it's clear that she's going

from the young to the old, the new dos, the redos and the extreme. the admissions and the down right shockers. it's plastered all over reality tv, from "dancing with the stars" to "the hills." >> i got a slight eyebrow lift. >> reporter: do the kardashians. across the country, nearly 10 million procedures were performed last year. 300,000 breast enhancement, 300,000 liposuctions and 2.5 million botox injections. if plastic surgery has a pecking order, dr. garth fisher of beverly hills tops the a-list. his roster of high wattage celebrities is endless. he's hollywood's dr. fix-it. he's fixed bruce jenner's facelift, boosted kourtney's and kendra's breasts. he's there to help the megastars sculpt their look.

what percentage of hollywood has had some work done? >> probably 70%. >> reporter: they are taking out their plastic for plastic. and the key to survival in this industry is to skal calpel your through. i've heard that you are sort of the michelangelo of plastic surgery. what makes you the go-to doctor? >> i'm a perfectionist. i know what looks good and what doesn't and i'll stand there until the end making it work. >> reporter: dr. fisher is known as a breast man. his special technique for natural looking cleavage avoids the fake circular look in favor of the tear drop. there's a list of celebrities walking around with fisher breasts. kendra, holly madison, kourtney kardashian, katie price, to name a few. all publicly boasting his

maybe, earlier and wish they hadn't done it. the probe is having surgery too early and too aggressive of the surgery and there wasn't a lot of balance in the work. just because you can do something doesn't mean you should do something. >> reporter: heidi montag became the modern poster girl for the addictive nature of plastic surgery, after going under ten procedures at once. >> she looked great before the surgery. if she came into my office, i'd hate to second guess what happened there but i probably wouldn't have offered those things to her. >> reporter: how often do you tell a girl, you know, honey, don't do it? >> we turn down patients all the time, 40% of the people that walk in here. >> reporter: 40%? >> yeah. we turn down a lot. stars are used to get what they want and it tuts a lot of pressure on, i think, the plastic surgeons. they come in your office and here is somebody that you've idolized all your life and they are telling you what you want to do, and they're like, can i get your autograph? you have to protect them, no matter who they are. and think about the medical side

of this. >> reporter: heidi sat down with cynthia mcfadden for the first time about her regrets. she has a few. >> i do not feel like i was prepared enough for this. maybe i should have known but how can you know when your doctor is saying, it's a little this, a little that. it really becomes a lot. i think that doctors should really walk you through all aspects of it. not just the glamorous side of it. doctors, it's like they are selling you cookies or something. >> reporter: the 24-year-old reality show starlet says it's not all that it is cut out to be. >> surgery is not glamorous. i would say plastic surgery is anything but glamorous. it's really hard and then it becomes a burden because you feel like you need to keep up with it and you don't. if i could take it back, i would. >> reporter: and heidi is far from alone in her regrets. take jennifer grey, she's publicly say that her nose job almost cost her her career. and though she's the new it-girl as tonight's winner of "dancing
