you all. i'm t.j. holmes. >> and i'm tai hernandez. we begin with health officials scrambling to figure out how a nurse in texas got ebola. >> the young woman wore protective gear when she cared for thomas eric duncan who died of the virus last week. investigators will be looking into whether that gear was incorrectly removed. we begin with tahman bradley. >> reporter: this morning, the first known transmission of ebola here in the united states. a nurse in her 20s who cared for ebola patient thomas eric duncan has contracted the disease. the hospital says the nurse was wearing full protective gear. a face mask, two layers of gloves, shoe covers, and leg covers. somehow, she got infected anyway. cdc officials say the woman was exposed due to a breach of protocol at the hospital. she may not be the only person. >> unfortunately, it's possible in the coming days, we will see additional cases of ebola. >> reporter: the nurse started showing symptoms on friday and drove herself to the hospital. this morning, officials are retracing her steps, trying to

determine who she had contact with. one person, as a precaution, has been placed in isolation. >> definitely, it's scary being in dallas. but then when it's like, right here. >> reporter: the same hazmat crews who decontaminated duncan's apartment is cleaning the nurse's. and people were advised in person and by phone. >> please be advised that a health dare worker who lives in your area has tested positive to the ebola virus. >> reporter: the cdc has issued guidelines to the hospitals, some health care workers are confused. >> when the nurses become infected they're blamed for not following the protocols. that is not going to work. >> reporter: this morning, health officials have ruled out ebola for a passenger that became sick on a flight that landed in l.a.x. yesterday and a patient in boston just back from west africa. t.j. and tai? >> all right, tahman bradley, live for us in washington. thank you, as always. of the six patients treated for ebola in the u.s., thomas eric duncan was the only one not

treated at one of the specialized units in several hospitals around the country. >> abc's dr. richard besser says this latest case may change the way patients are treated. >> every hospital has to be ready to receive someone with ebola. they have to ask about travel. if they're suspicious, they have to isolate and test. but at that point, if they have ebola, they need to think about transferring to one of the four hospitals in america who are trained for this. there is one in nebraska, one in montana. one in maryland, and one in georgia. this is what they do right. the cdc says they're thinking about a new change. i think that's a good idea. >> at one point, dr. besser was the head of the cdc. you'll hear more from him coming up on "good morning america." also developing this morning, after weeks of quiet, we're now tracking new tropical storms in the atlantic. fay battered bermuda over the weekend with 65-mile-per-hour winds and heavy rain, knocking out power for 30,000 customers. the storm moved out over the

ocean -- open ocean and strengthened into a hurricane. an advancing cold front is expected to neutralize fay later today. meantime, much farther south, puerto rico is bracing for tropical storm gonzalo. flash flood watches and warnings have been posted across the island. it currently has winds of 45 miles per hour. it's expected to swing out into the open atlantic, missing the u.s. east coast. on the u.s. mainland, a huge storm system is stretches across much of the midwest. >> jim dickey joins us now with the latest. good morning, jim. >> good morning, t.j. and tai. a couple of systems we're tracking. one storm is departing bermuda no longer an issue. now we're tracking gonzalo. this is headed toward the leeward islands, and going to track toward puerto rico and hispaniola. as we head through the early to middle part of this week. going to bring heavy rain fall. likely, it will strengthen, moving into an area that would be favorable for development. this is not expected to impact the u.s. mainland. we'll keep a close eye on it.

it's expected to make a turn to the north and could impact bermuda in the upcoming weekend. meanwhile, a powerful storm system moving out of texas this morning. dangerous storms headed through dallas, through oklahoma. and then those storms expand toward the mississippi in the afternoon and evening. gusty winds, large hail, and torrential rain. t.j. and tai, back to you. >> thank you, jim. iraqi leaders are pleading with the united states to send in ground troops. some 50 u.s.-led air strikes against isis positions in the past week have failed to stop their relentless advance. america's highest-ranking military leader now admitting the air strikes may not be working at all. and that isis is changing tactics. >> the enemy adapts. they'll be harder to target. they know how to maneuver and use populations and concealment. >> general dempsey says he expects to see isis start firing into baghdad. from nearby locations. for now, no immediate plans for u.s. ground troops in iraq.

breaking news overnight. an angry confrontation in hong kong as pro-democracy protests continue. hundreds of people charged barricades set up by protesters to block main roads near the city's financial district. police intervened. older men and taxi drivers demanded demonstrators reopen the roads, calling their occupy movement illegal. here at home, more marches are planned today after the weekend protest of a police shooting of a young black man in missouri. police used tear gas and pepper spray. police arrested 17 demonstrators blocking a convenience store. they were protesting last week's shooting death of 18-year-old vonderrit myers, who place say shot at them. test results on possible gun powder residue could come back as early as today in the shooting. thousands gathered outside police headquarters in ferguson. rallying the crowd against the shooting of unarmed michael brown. his mother led a march calling for the arrest of the police officer that shot her son.

ten people were hurt in a ferry accident in san francisco. the boat hit a piling. injuring some passengers. all were taken to the hospital as a precaution. no word just yet on what caused the accident. also in california, hillary clinton got an unpleasant surprise yesterday during a speech in san diego. the former secretary of state was just minutes into her address of the annual meeting of america's media -- pediatricians when a heckler blew a bull horn. [ horn sounds ] millions marched. [ applause ] >> as you saw there, clinton barely missed a beat. the heckler was whisked away. coming up, a security breach for an app used by millions. what the company is saying. what went wrong? school scandal. students arrested and now suspended. part of a football hazing case dividing an entire town. it was supposed to be a fun hayride. but it went horribly wrong.

well, on average, we're paying a lot less for gas than just three weeks ago. 12 cents less, that's according to the lundberg survey. the average is $3.26. further decreases are expected. prices have been falling for nearly four months now. the stock markets are open

this morning, despite columbus day after a wild ride last week. the dow had swings of 100 points or more on four days. wiping out all the gaines made this year. the s&p 500 have had fluctuations. hundreds of thousands of nude notes and private individuals are showing up online after haerks accessed a snap chat database. those photos are expected to disappear after ten seconds. there are apps that allow users to save them. snapchat is blaming those apps for the breach. "gone girl" stayed at number one in the box office. $27 million over the week end. "dracula untold" was second. and the movie "alexander and the terrible horrible no good very

rocked one suburban new jersey town. >> hundreds of people turned out for an anti-bullying rally. the football season has been canceled at the sayreville school after the hazing allegations have been surfaced. >> kids who didn't do anything are suffering. >> one or two kids who stepped up and did the right thing. they prevented one less child from being bullied. >> seven students face sex crimes charges. they were arrested over the weekend and last night they were suspended from school. it's not clear if any of the coaches have been found responsible. a teenage girl is dead after a hayride accident in maine. the jeep towing the wagon went down a hill and struck a tree.

investigators suspect the jeep's brax failed. the harvest hill farm will reopen today but not the hay rooid. two toddlers injured at another fall fast value. their inflated bouncy house was picked up by the wind and carried 50 yards away. one of the to thes is in critical condition. one of the kid's fathers watched helplessly. >> we see the bouncy house come up off the ground and slam right back down in the place it was. then it takes off about 60, 70 feet in the air and travels about, maybe 50 to 150 feet away. and slams both of our children into the ground. >> the bouncy house was still being set up and wasn't yet tethered to the ground when the two boys crawled inside. the owner said he had blocked off the entrance. others say the kids were given permission to use it. three swimmers trapped in a

cave were rescued. they had to be plucked to safety with ropes and a crane after jumping from a cliff apparently not realizing how rough the water became. all three swimmers are expected to be okay. now a bizarre play to show you. at an nba preseason game between the lakers and the warriors. >> rodney price lost a shoe during the game. instead of putting it aside or back on, he decided to throw the shoe at the opposing player. >> you do what you have to do at times. this is kind of an attempt at defense. it got him a technical foul. time for more sports, including a dramatic late win in the n.l.c.s. >> for that, we turn to our man at espn. >> welcome inside "sportscenter," i'm steve levy. to many casual fans, kolton wong

will be remembered for being picked off last year to end the world series. he ended game two of the n.l.c.s. last night with a 370-foot walkoff home run. giving the cardinals a 5-4 victory. game three, tuesday afternoon in san francisco. john lackey against tim hudson. now to the nfl, big matchup, the cowboys in seattle. fourth quarter, less than five minutes less, cowboys down three. third and 20, romo spinning out of trouble. great play to get out of there and look at the catch by terrance williams. our cris carter called this the best catch of the nfl season. tiptoeing through the tulips indeed. demarco murray. over 100 yards in every game. richard sherman can't keep him out of the end zone.

dallas wins in seattle. the boys are 5-1. "monday night football" the 70th edition. it's the 49ers and rams. little known fact, both head coaches played for our mike ditka in chicago. all starts with "monday night countdown" at 6:00 p.m. eastern time on espn. for more, check out a live "sportscenter" from studio excellent, 9:00 a.m. eastern time. a great time of year with sports, football, baseball, hockey. but, the north american wife carrying competition is our "play of the day." jesse and christina finished third the last couple of years. this year, they won. >> they get to share her weight in beer, 96 pounds. they also share five times her weight in cash. so almost $500. but, you know, just the pride of

from the transition road up to the toll plaza, a long line of cash-payers moving through. we will see if something is going on there. the rest of us are accident free. 24 minutes westbound from tracy to dublin with antioch, over to san francisco, 17 minutes. 4:29. oakland police are investigating a shooting that last a woman dead and a man in the hospital. police think boat victims were in a car that appears to have been shot up. the woman who died was hit several times and the man with her was hit only once. he was able to run from the scene and taken to the hospital where he is in stable condition.

the coast guard is waiting for drug and alcohol test results after a packed ferry crashed into the pier in san francisco. amy hollyfield is at pier 42 with the latest. amy hollyfield? >> yes, the investigation is focusing on the crew. that is why they will look at the drug and alcohol test to figure out why the crew backs into the piling. ten people were hurt yet, ferry that backed into the fixed piling. there were 232 passengers. the ferry was headed to sneak and pumped into the piling at pier 41. a man's wife and nephew were taken to the hospital. >> a big gash on his eyes but they are trying to do everything
