back now to our dream. a cover story in black maternal health week started by the black mamas matter alliance to raise awareness around the inequities . black mothers face nationwide and joining me now is dr elizabeth carroll practicing o b. g. y n is also senior vice president and chief medical and health officer for the march of dimes. good morning. thank you for being with us. she said she'd rather be delivering a baby in the hallway, then on tv right now. several studies show that maternal mortality and severe maternal morbidity among women in this country is going in the wrong direction. the surgeon general i talked to him last week. he said that c section rates are a concern and even the new york state birth equity project has announced new initiatives should try to bring down the c section rate. 5% talk to me about the concern there. so first. thank you. thank you for highlighting black maternal health week. um and really important to be talking about experiences of black women

versus experiences of white women. and if we aren't doing that, we're not really talking about women's health so really appreciate being here and the statistics are all going in the wrong direction. we've got mortality rates that are up 89% we've got preterm birth up by 4% and, yes, c section rates all going in the wrong direction. despite really doing big effort . we are seeing big disparities when it comes to black and brown women in this country we talk about those disparities for black and brown women, what is at the heart of it, it's a really complex problem. but it's really it's fundamental. there's racism. there's bias their structural and social issues we need to address and one of the biggest ones in the march of dimes is addressing is about access to care. yeah no access to care is one of the biggest barriers to healthy moms, healthy babies and families. and so when you think about access to care, i think about just getting in for that prenatal visit. um and if you don't have

insurance, or you don't have access to maternity care, desert is someplace where there is no obi provider. there's no hospital or birth center to deliver. and we're seeing a rise in those across the country. and i bet you're thinking that's all rural. actually urban. it's actually 30 to 50% of counties, right? exactly and so how do we solve for that? i would say the marginalize we're here talking about it. we're here educating. we're trying to do a lot of implicit bias training for providers. we're trying to get to women and get them educated, were also really trying to advocate and do research and we're doing we actually have a health equity center at the university of pennsylvania. it's our inaugural. center for health equity and really trying to look at doulas and other outcomes and innovations. elizabeth howell fantastico bgn chairman. they're really pushing this that we support and then we do a ton of advocacy. um we know the postpartum period is really, um, something that we as women six weeks later, yeah, you're not done right. it's not wrapped up in a bow, and we're not on tv.

what we are really trying to do is advocate to extend postpartum care for a full year because we know mental health. we know that cardiovascular doesn't end after six weeks. neither does diabetes anho conditions need to be looked at, and it's in that period that we're seeing that most maternal deaths happen a week to a year after a woman gives birth, i want to bring up this graphic that the new york state department of health actually just recently came out with these lists of patient rights and things that they're going to try to do better at number 11 on the list is making sure that patients go home with postpartum education and knowing the warning signs. how crucial is that for the health of a mom and her baby? it's incredibly incredibly crucial. these are times when you can still sees after birth, just having complications or readmission for your hypertension. the most common time is within those 1st 16 days, but then you need to be continued to be monitored and followed, and it does like i said, doesn't end at 42 days doesn't end at six weeks. it

every person you meet can be someone along the way that can help you. and i think people especially when you have internships, they usually do their internships and they move on, and they forget about those people because they're moving on to the next. but let's build those relationships. i think that's so interesting because the idea of networking is has always been really intimidating to me. but building real relationships feels much more natural. so it's nice to hear you say that. what about people who are looking to further their careers whether they are unemployed right now? or or liquid looking to pivot who may be dealing with imposter syndrome. i love this question. so i believe that if you're in the room, you belong there and a lot of people. when they get in the room, they question it. they're like well am i am i deserving? am i smart enough, and i really the person who should be here. but what they should be thinking is what can i do now that i am here? what impression can i be making? i love that and you say it's really important to celebrate your wins. but you say it's important to do it elegantly. what do you mean by that? wellpa
