7:49 am
>> reporter: through a series of public records requests, the investigations unit obtained state inspection reports revealing they have failed to properly maintain critical signals and in of the trains. the report details hazards that could lead to derailment and trains colliding head-on. in a report of 2013, vta knowfully and willfully violated procedures, and vta does not have the capability to repain and maintain safe vehicles. one shows in one unannounced viet that month, multiple wheel defects and outtread on four trains was found. what safety risk does that pose? >> it's a risk of derailment. >> reporter: paul king, the state's deputy director for rail safety. when the inspectors found the problems, what did they instruct the vta to do? >> not run those vehicles, and we found that they did run them

month made over the last year, i don't think they are going to be ready. >> reporter: and getting enough trains into compliance is not the vta's only problem. did they have multiple indents last year with trains headed towards each other on the same track? >> no, but we operate single track inspections so there are places where trains have to wait for a train to go opposite direction if that makes sense. >> reporter: said on multiple occasions there were concerns about signals being crossed in those trains potentially colliding head-on. are you saying that's not true? >> we do not have that concern. >> reporter: inspectors had that concern because of this, multiple incidents in 2012 and 2013 where inspectors found problems with signals that allowed trains to head towards each other on a single track in potential head-on collisions. >> there would be signal between the trains, other place for the train to stop or where the train is required to stop. >> reporter: should that happen
7:52 am
at all ever? even if there were signals in between, should two trains head towards each other? >> no. >> reporter: there's reasons for the failures of electronics to confusion for the vta controllers. one said they were concerned the trains were both traveling at a high rate of speed, 55 miles per hour and 45 miles per hour towards each other before they stopped. it happened here on the line, a better look for the side, a one and a half mile stretch of track going north and southbound trains share. if they continue to travel at those speeds, they would have collided in less than a minute. >> there's never been an incident where trains are in an accident, a head-on collision. >> it disturbing me he's minimizing the problem. they are just lucky. >> reporter: russell quinby, an expert spending 20 years as an
