>> on december 2, 2013 i was diagnosed with ovarian cancer. i led an extremely healthy lifestyle. i was starring in "motown" on broadway. i wasn't having symptoms. >> at 34 she went to her gynecologist for her annual visit. a doctor noticed a cyst on her ovary had grown larger and was confirmed to be cancer. >> knowing how small it had been initially and the growth that it did within a year's time was very significant. the first time i had a pelvic exam it was uncomfortable. i will tell you it is not the worst thing in the world. it's fairly easy compared to what it could be. >> she is now cancer free and is featured in a psa encouraging women to be their own best health advocate. >> to know your body is really the most powerful gift to give

to yourself. >> an assistant professor and practices family medicine at georgetown university medical center. welcome. >> hi, ladies. we have you on opposite ends of this controversial decision that came out today. tell us your points of view. why do you think people should continue to go? >> i think one of the most important reasons that pelvic exams are important for women is it gives a baseline sense of a woman's anatomy each year. if i do a pelvic exam if there are changes from year to year much like you heard in the prior film i will be able to feel it and detect new findings like fibroids, ovarian cysts, pelvic pain, the list is lengthy. >> makes sense. >> if she hadn't gone the next year.

>> this is a terrible story. i could have brought a story of a woman who had a finding who went on to have surgery and had a lot of complications. there are harms to the exams. so they should not be discounted. the number of women who actually have pathology that is meaningful clinically and will save their lives if we find it early is very small. >> the guidelines mention very few studies on this at all. so really there is not a lot of data looking at pelvic exams in women and the long term consequences of what has been found. >> and if they had not gone for a mammogram. and to me those statistics mean nothing if it is somebody you love, it is your mother, you, your best friend. i think this is all about money. that is my personal feeling of it. >> i agree with you. i don't think it is about money.

when it is your mom or any individual this is a terrible situation. however, we can't make clinical decisions based on anecdotal situations. those people who did the study looked at 60 years worth of studies. there are not a lot out there but looked at more studies. >> there were zero studies on benign findings on pelvic exams. >> the down side, they say it is costly. number two, it is uncomfortable. it seems like both of those things are sort of a yawn. if you end up getting a diagnosis for something and you do surgery you don't need you see that issue. why not really? >> there is not much you can really find on the pelvic exam in a woman who is completely asymptomatic. the things you can find that if you find them early make a

difference in extending her life -- >> find it early. >> i don't agree with that. i have many women who have no complaints when they come in. they don't vocalize any issues. i will find benign things possibly and they will say i feel a 5 centimeter fibroid. it is an important part of education and counseling. >> do you think it will change anything? >> i hope it changes some of the practices. i think this is a ritual that is not evidence-based. i hope the practice changes and i hope we get to do fewer pelvic exams. >> how many pelvic exams do you do as a family practitioner? >> i deliver babies and do thousands a year. she says she finds a lot. i don't find a lot. listening to her or me is not the point. the point is listening to the science. >> i think there needs to be more research. >> healthy debate.
