clarity as steve kornacki gives us his perspective. thanks for starting your week with us i'm frances rivera. >> new hope the u.s. may soon see the approval of a coronavirus vaccine. cases and deaths continue to surge. as of today there have been over 14.5 million infections and over 282,000 fatalities according to an nbc news tally. the latest data comes as most of california enters a new lockdown here's nbc's meg fwan fitzgerald >> reporter: valerie, good morning. millions of californians are waking up to some of the most restrictive measures we've seen during the pandemic while they do all they can to stop hospitals from being overwhelmed. 33 million californians, hours away from a lockdown people told to stay home, stop

gathering, no more in-person dining and reduce retail as hospitals try to save people in crisis >> bottom line is if we don't act now, our hospital system will be overwhelmed. >> reporter: fear turning to frustration for restaurant owners. >> i'm losing everything everything i own is being taken away from me and they set up a movie company right next to my outdoor patio. >> and for so many other workers forced to close down, desperation. >> we've been taking one for the team and there's nothing left to give like nothing left. >> reporter: in some parts of southern california, defiance. the sheriffs of orange and riverside counties saying they won't enforce the state's new stay at home orders. >> riverside cumulative sheriff's department will not be black mailed, bullied in the enforcement of the government's orders. >> reporter: even as the virus surges to unprecedented levels,

conflicting messaging and patch work enforcement causing confusion. >> it adds confusion to who's right and who's wrong. >> desperate pleas for americans to take this seriously >> i want to be very frank to the american people, the vaccine's critical but it's not going to save us from this current surge. only we can save us from this current surge and we know precisely what to do. >> across the nation, no region's spared. more than 100,000 americans now hospitalized new orders out to contain the crisis eight states adding restrictions in the last week alone families like barbara zieman who lost loved ones know far too well how serious the virus is. the pain of losing her father will never go away. >> a lot more families will be in my shoes and it didn't have to be this way. >> reporter: officials say it

takes two weeks to see the effects of new restrictions that are put in place here in los angeles county it's been 12 days since outdoor dining was banned, so it's likely we could see the effects of that by the end of this week. >> meagan, thank you. president-elect joe biden is already -- readying, i should say, his key allies to battle the pandemic the president-elect will nominate california attorney general javier becerra to lead the health department and roichelle wa len ski will lead the cdc. let's get to nbc's tracie potts for the latest on stimulus talks. time is running out for a new relief bill. >> reporter: they're running out of time, frances the house is supposed to leave on the winter break this week. the senate next week they are rushing to try to get this covid relief bill done.

on the table is a $908 billion compromise that will deal with some of the most immediate needs, people who are hungry people who are facing evictions at the end of this month and millions, up to 12 million who could lose unemployment benefits the day after christmas. so those negotiations are on a fast pace. also we are watching very closely the fda panel that meets on thursday to decide if the pfizer vaccine can be approved for emergency use. if so, they're going to turn that around pretty quickly and try to get the vaccine out so people can start getting shots on friday. >> we have a vaccine but we will not all have the vaccine in our arms before may or june so we need to be very cautious and vigilant. >> i want to be very frank to the american people. the vaccine's critical, but it's not going to save us from this current surge. only we can save us from this

current surge. >> reporter: president trump has announced that his lawyer, rudy giuliani, who's 76 years old, has tested positive for covid-19 his age puts him in a high risk group. he's been seen meeting recently with lawmakers in michigan, in georgia, in arizona in some cases without a mask the arizona capital times reports the legislature there is shut down after giuliani met with 15 current and former republicans just before the announcement that he tested positive. >> now he's being treated at the hospital there in d.c. tracie, thank you. this morning we are getting our first look at the covid vaccine arriving at hospitals in the u.k. and by this time next week the same thing could be happening here in the u.s. here's sarah harmon with the details. >> reporter: frances, valerie, good morning this is a day that has been

months in the making the vaccine is finally here, and on tuesday people across the u.k. will begin receiving the very first doses, including here at st. thomas's hospital in london after much anticipation, they're here the first doses of the pfizer vaccine have arrived in british hospitals. >> to know that they are here and we are amongst the first in the kun troy receive the vaccine and therefore the first in the world is just amazing. i'm so proud. >> reporter: staff are working overtime preparing to give shots on tuesday the u.k. has ordered 40 million doses, enough for 20 million people but only 800,000 will be available next week. >> i don't have any concerns i have concerns about covid and i would rather have a vaccination than not. >> i want one as quickly as possible so i can get on with the rest of my life. >> reporter: meanwhile, in the u.s. the fda is considering

emergency authorization for both the pfizer and moderna vaccines. the pfizer decision is expected on december 10th. >> within 24 hours we'll ship to all of the states and territories that we work with and within hours they can be vaccinating. >> reporter: initial 40 million doses is expected in the next month. >> we're going to focus on those most vulnerable and those most on the front lines treating people with covid with the initial 40 million doses in the next month, then we'll progressively keep adding more and more people. be thinking in the february/march time frame you'll see more general vaccination by next year we'll have enough for every american that wants it. >> back in brittain more demand than doses the 94-year-old queen expected to be among those vaccinated in the coming weeks, but she won't get priority treatment >> reporter: now there have been

logistical hurdles along the way. initially the plan was to offer the vaccine especially to vulnerable nursing home and staff, however, transporting nursing home residents to these vaccination sites safely has proved a huge hurdle in fact, it's thought the first people who actually get this vaccine will be people over 80 who already had in patient appointments at the hospital frances? >> our thanks to sarah harmon with that report. a covid vaccination could be approved this week that's just the beginning. the next challenge is getting the vaccines to rural areas. dasha burns has more. >> reporter: hospitals and health departments are in overdrive preparing to roll out the covid vaccine as soon as it's approved. the race is on to get the ultracold freezers in order to store the pfizer vaccine and

make a plan for distribution it's a problem for rural states like south dakota. our team spent some time there and got exclusive access to one hospital's storage units and their plan to vaccinate the rural population in south dakota hospitals hit hard. >> we're definitely in a crisis situation. >> reporter: rural regions facing a lack of access and resources now faced with the daunting task of distributing a vaccine. for sanford health, the largest health system in the region, cold storage is the key. here in sioux falls, south dakota, they have two freezers one for the fooifz and one for moderna. they have eight ultracold freezers strategically placed to reach the rural communities. each costs 10 to $15 million and can hold 75,000 doses. >> they're double doors. the other door with the latch on

it, you can lock this and then inside here is where the actual shelves store it you see it's very cold >> reporter: how critical z having this technology >> it gives us a lot more control how we can distribute and vaccinate patients. >> reporter: dean webber is responsible for getting the vaccination. >> this is a considerable effort to distribute the vaccines our drivers put on 11,000 miles a day. that's something like 3 million a year long distances. >> reporter: but not all rural health systems can meet the challenge. worries about storage in kansas. >> there are only about 10 different hospitals in the entire state that have that capability. >> reporter: and distribution in wisconsin. >> there will be hubs around our state and we'll partner with our state and our county health departments to distribute those.

>> reporter: respiration with a dose of hope. >> things could look very different come spring and summer it's been a long haul. these people in the hospital have been working long shifts, really hard. >> reporter: in many of my conversations with doctors, nurses, front line workers across the country, when they start talking about the vaccine, people often become emotional after so many months of feeling helpless in the face of this virus. there's finally some hope around the corner and a light at the end of the tunnel. frances? >> dasha, thank you. a nor'easter brought heavy rain, strong winds and snow to parts of new england over the weekend. more than 20,000 people lost power in massachusetts, maine, and new hampshire. by late sunday power had been restored to more than 50,000 people in the region central maine power, which accounts for many of the outages in that state, said their crews are working as quickly as possible to restore power to customers. >> let's hope they get that, too.
