good morning. welcome to today on a wednesday morning. it's already a hot one. we're glad you're with us. >> it is a real busy one, too. we have several important questions are being asked, including this one -- will masks once again be necessary? the highly contagious delta variant is surging in pockets of the country where vaccinations are down. we're going to talk about that and more with the director of the cdc. plus, nearly a week after that devastating condo collapse in florida, the question remains, what happened? how did it happen? new pictures taken 36 hours before that tragedy appear to show what may have happened. morgan chesky is there live with more on that. meantime, it's a little after 7:00 in the morning. and guess how hot it is in new york already? 81 degrees. millions are asking this question -- how long will this heat wave last? >> and it's a wet heat, okay? we're going to get that answer real muggy out there. but first, nbc's stephanie gosk in central park with more on the problems being caused by this dangerous heat, how people are coping.

good morning. >> reporter: what is that old adage, it's not the heat, it's the humidity? it is like a hot soup out here. when the temperatures top 90 later today, which they will, this will be the second official heat wave in new york city and it's not even july yet. across the northeast, people are waking up to another day of brutal heat and punishing humidity. >> it's so hot, i feel like i could barely go outside. >> reporter: heat warnings across pennsylvania and new jersey with feels-like temperatures well over 100. boston and philadelphia declared multi day heat emergencies. in revere, massachusetts, crews battled house fires causing at least one firefighter to suffer from heat exhaustion. >> i'm planning on going in the house and staying under the air-conditioning. >> reporter: adding to the misery, in philly, only 70% of the swimming pools can open because of a lifeguard shortage.

>> i don't know if we'll be able to open up beyond that. >> the weather service warning people to be on the lookout for heat stroke which is exactly what emergency responders have been up against in the northwest. in oregon and washington, more than 1,800 people have been treated with heat-related illnesses since the start of the heat wave. temperatures are finally cooling near the coast, but the staggering heat in moving inland. >> it's nice to get out when it's this hot because inside it's like an oven. >> reporter: temperatures approach 100 degrees in eastern washington late tuesday as rolling blackouts continue across the region. >> the weather in the pacific northwest, i've lived here all my life and it's changing. >> reporter: 35 cities tied or set record temperatures this week and scientists say climate change is creating conditions for more extreme weather. >> we're going to have events like this popping up more and more often. more and more places. >> reporter: here in new york city, they're scrambling to get cooling centers up and running. but a lot of the places they use for cooling centers are facing covid restrictions or being used

for vaccinations. >> you have to love new yorkers. they're out there running in this oppressive heat. a lot of people are wondering when this is going to break. >> this is putting a real strain on power grids all across the country and here in the northeast. in fact, we were getting some indications that they were dropping the power here in new york city in some places. 5 million people still under heat advisories. heat watches and warnings. in the midwest, new york, and the northeast, 46 million people are being impacted by this system. this intense heat with the jet stream way up to the north as you move here into the east, look at this, the heat index it's going to feel like 93, state college, p.a. lynch burg, 101. same in norfolk. 95 in ocean city. 102 in new york city and 100 in boston. the good news is, we are going to see a break. there's going to be a front that pushes through and that's going to drop temperatures by friday in detroit to 77. 79 in charleston. new york city, by saturday, 75

degrees. boston, 68. but out west, we're going to continue to see that intense heat away from the coastline. the coolest locations, that's where it will be, but back to the west, look at this. we are talking about temperatures, triple digits for spokane into the weekend, boise, idaho falls, winnemucca, medford, oregon, will see triple digits into the weekend. guys? and now to new covid concerns re-igniting the debate over masks. the outbreaks of the highly contagious delta variant spread. some health officials in some communities are reinstating safety recommendations even for those who are fully vaccinated. we'll talk about that with the director of the cdc in a moment. but first, miguel almaguer has our report. good morning. >> reporter: savannah, good morning. officials are warning the more transmissible variant could be the dominant strain by the fall. especially states with low vaccination rates. that concern is leading the

country's most populous county to reverse course and tell residents don't ditch all pandemic precautions just yet. this morning, the dangerous delta covid variant renewing the debate over face masks. >> it's definitely been kind of chaotic and there's not really strict guidance. >> i think it should be your choice if you want to wear a mask or not. >> reporter: l.a. county is urging everyone to keep wearing masks inside public places like grocery stores and movie theaters whether you're vaccinated or not. the county's public health agency saying, "until we better understand how and to who the delta variant is spreading, everyone should focus on maximum protection." that messaging in line with the world health organization, which continues to recommending face masks and social distancing indoors. lester with dr. fauci. >> do you expect the cdc to change its mask wearing guidelines for vaccinated americans? >> no, les, i don't.

we know from experience now that the vaccines that we're using in this country do very well against the delta variant. >> reporter: moderna announcing a new lab study found its vaccine shows promise in protecting against virus variants, including delta. still ahead of the busy fourth of july weekend, many are encouraging caution. >> you should just bring your mask with you and keep safe. >> reporter: the new concerns, especially tough for parents with kids too young to get a shot. >> the pros and cons, do you want to get out and socialize? it's just a constant double edge sword to me. >> reporter: another debate among doctors, should americans vaccinated with johnson & johnson consider getting a booster shot? there are multiple clinical trials in the u.s. on booster shots with various combinations of the existing vaccines, but some doctors are already adding a shot of another vaccine to j&j. dr. angela rasmussen initially got the j&j shot but recently

took a dose of pfizer. >> i thought it was wise to have as much protection as possible. >> reporter: still, she touts the overall effectiveness of the vaccine. >> the j&j vaccine does provide full vaccination. they need to keep you from getting really sick, they need to keep you out of the hospital and they need to keep you from dying. >> johnson & johnson says its vaccine offers durable protection and there is no evidence for need of a booster dose. dr. fauci told lester the cdc and fda will need more time to study scientific data before making any formal recommendations on boosters. savannah. >> miguel, thank you. dr. walensky, good morning. director for the cds cs cds. it's good to see you again. >> good morning, savannah. thanks for having me. >> let's talk about this confusion. i think a lot of people are going to be scratching their heads. you have l.a. and the state of illinois saying, look, even if you're vaccinated, if you're indoors, put that mask back on. the w.h.o., the world health

organization, taking it a step further and says you should not only be wearing your mask if you're vaccinated, you should do the social distancing. where does the cdc come down on this? >> i think this is a really important thing to work to clarify. first, we know that the w.h.o. has to make guidelines and provide information to the world. right now, we know as we look across the globe that less than 15% of people around the world have been vaccinated and many people of those have really only received one dose of a two-dose vaccine. there are places around the world that are surging. so as the w.h.o. makes those recommendations, they do so in that context. here in the united states, we're fortunate. we have three vaccines that we know are safe and effective. we have two-thirds of the adult population that is fully vaccinated. and, really, quite protected from the variants that we have circulating here in the united states. that said, we have always said that local policymakers need to make policies for their local

environment. there are areas of this country where about a third of people are vaccinated. they have low vaccination rates. and there are areas that have more disease, that are -- have more disease in the context of people not being vaccinated. so in those areas, we have always said please look, make suggestions, but those masking policies are not to protect the vaccinated they're to protect the unvaccinated >> so bottom line, and all that context is actually quite helpful, but the bottom line is the cdc still says if you are vaccinated, you don't have to wear a mask. >> that's exactly right. what i would say in that context is if you are vaccinated, you are safe from the variants circulating here in the united states we would suggest that you look to your local policies because we would like our policymakers, even if you're one of the one-third of people vaccinated in the area, to adhere to the policies to protect -- the policies that are made to

protect the unvaccinated and then, of course, as we have always said, if you have an immunocompromising condition, if you might have had a transplant and you're vaccinated, everybody should consider their own situation if they would feel more comfortable wearing a mask. >> another big issue that's come up, the delta variant is sweeping through this country quite quickly, as you know it's very concerning for health officials such as yourself now, the two-shot vaccines, the pfizer and the moderna that we have here in the u.s., have been shown to be quite effective if you have both doses. johnson & johnson, the single dose vaccine, is somewhat less effective against this delta variant. i think that the science is showing that so far. a lot of private doctors are

telling their patients who got the j&j one-shot dose, go ahead and get a booster shot go get a pfizer, go get a moderna. is that okay with you? do you think that's a good recommendation should people do that? >> thank you, savannah so let me clarify this point as well right now, we have no information to suggest that you need a second shot after j&j, even with the delta variant. here is what we know so far. we have seen less data with j&j and the delta variant and those data are forthcoming however, we have every reason to believe, based on how j&j is performing with our variants of concern, and that is quite well, and how its sister vaccine has performed against the delta variant in other countries, which has done quite well there, as well. so generally people are agreeing that they anticipate that j&j will perform well against the delta variant as it has so far against other variants circulating in the united states >> to put a fine point on it, is there any harm in doing that let's say it performs well, but not as well as the two-shot

regimens if a doctor tells you, go get another booster shot to be safe rather than he sorry, is there any reason not to do that? >> well, it's unstudied. just as we don't have any data to inform and say we believe the j&j is going to perform well, we don't have a wealth of data to say how well you'll be protected by a second vaccine, how -- what side effects would occur with that vaccine so if you choose to do that, the one thing i would ask is to enter a study in doing so so we can learn more >> dr. walensky, always a lot to talk to you about. thank you so much. we appreciate it >> that you can. stay well. a lot to get to including the latest overnight in that tragic building collapse >> it is day seven of that painstaking search and rescue effort, a an operation that is still unfolding around the clock. no one has been rescued since last thursday. this morning, the command for
