report all contributed to an environment in which arguably one of the bureau of prisons most notorious inmates was provided with the opportunity to make his own life. what happened? what were the failures they failed to do inmate counts. they failed to do 30-minute checks on inmates, which they were supposed to do. they failed to provide him with a new cell mate. his cell mate was transferred the day before he died he should have never have been left alone, but he was and they didn't search his cell the day before he died and they don't know, in fact, when the last time his cell was searched and they he died they found he had been hoarding bedes fo linens and had torn them up and made nooses they found in his cell >> there was a theory that ebstein did not die by suicide, and does this report put those

theories to rest >> it tries to, craig. i'm not sure anything will if the inspector general agrees with the fbi, that it found in criminality involved in this situation that there were three inmates that had direct line of sight in epstein's cell where they didn't see anyone come in and out that night the one video camera that did record video was trained on the stairway into the tier where epstein was kept no one went in and out of that tier that night. and also they found that there were no defensive wounds on his body no signs of a struggle turned up on the autopsy and most interesting of all, they found out that two days before he died, he signed a new will that is something the prison staff didn't find out until after he died. >> lots more to get to, including the latest on the health alert doctors urged to be on the look out for malaria after five case

and texas. blaine, good morning >> reporter: craig, good morning were found in florida and texas. blayne alexander joins us now with everything we need to know. good morning >> reporter: well, craig, good morning to you that's right the cdc says this is the first time in 20 years that malaria has spread state side. the good news is officials say the risk of actually contracting malaria are extremely low. but it is a troubling sign as millions of americans prepare to spend their summer days outside. they are the pesky part of summertime fun, from kids camps to backyard barbecues, mosquitos are often the unwanted guests. but this morning, the cdc is sending a warning about mosquito transmitted malaria cases. it comes as health officials identified five locally acquired cases in recent weeks. four in florida. one in texas all transmitted by mosquito bites. >> what was different about this

is things happened domestically. >> reporter: officials say there is no evidence the cases in the two states are linked. in florida, all four cases were detected in sarasota county. now the entire state is under a mosquito-borne illness warning symptoms can be similar to the flu, fever, body ache and chills as well as headaches and nausea. with an early diagnosis, malaria is treatable with the help of prescription drugs now the cdc is urging doctors to be on high alert. >> we need physicians to understand that they should have some suspicion for malaria it is not difficult to diagnose, and it is not difficult to treat. >> reporter: the key protection that includes wearing insect rebellion, covering up with long sleeves and pants and getting

the in-laws of seven-time nascar cup champ, jimmie johnson, and the location of a heartbreaking murder-suicide involving the couple >> it is one of those cultural community shocks. >> reporter: early monday night, the police department says they received a 911 call from a woman saying there was a disturbance at a residence involving a gun before she abruptly hung up. when the officers arrived, they found one person dead in the front hallway. that's when they say a single shot was fired from inside the home police eventually found two more bodies, later identifying the victim as 69-year-old jack, his wife, and their 11-year-old grandson police believe terry made the initial 911 and then shot and killed her husband and grandson before taking her own life. >> it was traumatizing to find out that a long-standing family who had made so many contributions to our family were involved in this type of incident it was more bone-chilling when we found out there were children
