verbalizing at that time before the surgery. after surgery she was under anesthesia so it's hard to say whether she was in a collotype state. but we light and up the anesthetics and in examination we were very happy and optimistic because of essentially over the course of the evening she was able to follow simple commands. i get its very early in our course we don't know what's going to happen what her deficits will be in the future anything like that. we can give you those updates as we go. we've been happy with the hospital course, the blood transfusions of worked well, she's not quite empathic. were all very happy at this stage. with that interest. all open up the floor for questions. >> it's been simple commands how she verbalize that all?
9:09 am
she didn't recognize her husband last night? >> we can get into much more detail but i can tell you that right now with this type of surgery her she can open her eyes at this point from a mechanical standpoint and she's also on a ventilator so she can't speak at this time. >> what is the simple command? >> please squeeze your hand, shows two fingers very simple but one thing i want to emphasize as we take the simple commands for granted but they apply a high level functioning in the brain. >> he did mention, this wasn't all little grazing will through the brain this was a devastating wound that travel the entire length of the brain on the left side. they saved their lives than a resurgence save to these patients' lives. with the function that she had before the surgery tells us how much injury the bullet did. the fact that she had a lot of preservation before surgery is what made the
9:10 am
situation as well as it can carry and of course, as time goes on the a fax from the bullet is going to have some degeneration of the brain cells a matter. we have to see what happens later on. >> making it through the first 24 hours as that of vital signs a good sign? >> love a ticket through sometime points one most important is how she was before the surgery. in brain injury particularly gunshot wounds we have a primary injury that's the damage done by the bullet as a reverses. secondary injury that's the injury caused by the swelling in changes. that we can try mollified. i again, if she comes and is able to follow commands that appliance the brain circuits are working. the more we see them working after the surgery between 24 is 48 to 72 hours a much more encouraged we are. >> it went from the left
9:11 am
side from basically the back to the front. the left side and most people the left side controls the right side strength, a sensation. in most people it also controls their speech functions their ability to understand or make speech. that includes the ability to understand simple commands. >> how long was the surgery? >> about to our store is an extension at the end of the case we had one of our occupied plastic surgeons perform a decompression because one fracture was affecting the i saw it. >> prognosis and time of recovery how long my congresswoman have to be in the hospital? is a lifetime of recovery? >> we don't speculate on
9:12 am
the degree of recovery because we've seen the full gamut. it's really not productive. in neurosurgery would talk about recovery on the order of months to years ago even close the book on it until were 70 years out. the faster you recovery this year recovery will be but again that's a general rule of thumb that does not play apply to all cases. it's not uncommon for these entries to be in the icy you for a least a week may be in the hospital for two to three weeks and no doubt there will be rehabilitated phase and that could take weeks to months. beyond that setting in actual fast date is not productive. >> it's going to be how her brain functions it's important for us to get a clean assessment so the drugs were used are very manageable and very short lasting so we can't get a good assessment. it's
9:13 am
basically resting her in between the time. send them away crop for the examination, how she responds is what will be able to base our prognosis on. i know dr. just did the surgery and everybody's got to be cautious about overcalling it. i'm optimistic and i was optimistic yesterday when i saw the case and i saw the brain. overall, this is about as good as the squad to get. when you get shot in the head in the bullet goes through your brain, the chances of the living are small. the chances of your waking following commands its is much smaller than that. this is but a great situation. hopefully it will stay that way obviously we don't know which direction she's going to go. it is still precarious at this time. >> we always do cat scan in fact we had one before the surgery that tousles the surgeons were ripped a focus our efforts. i can't go into details for obvious patient
9:14 am
privacy issues, the cat scan showed pipit the bullet went through the left hemisphere. it was not true some of those critical trajectories that what made recovered much more difficult. >> so normally in the i c u state when the rain swells is like when you have a sprained knee and fluid accumulates in there and gets tense and there's a tremendous amount of pain. the brain will go through the same situation except when it gets real tense and their the blood flow to the neurons and brain cells get compromised. in this kind of situation doctor took this fall-off and kept it off. so right now half of for brain doesn't have this call it's preserved in the hospital right now and again reimplant that later on. that's part of the progress we've made in your surgery. also the last decade we've
9:15 am
learned from the military about letting the brain after penetrating tron us well and not be constrained within the cavity it has helped a lot. it played an important factor here is well. it >> if run into trouble the next few days we have tools we can use medical agents to try of the brain. we lit to the center of the brain to drop so it to measure pressure and believe it if necessary. -because she looks good right now there's no need for that. brain swelling is the biggest threat at this point. >> the bullet went in the back left hemisphere where did it come out? >> from a forensic prospective doctors are very leery especially later on to be for sure. with the experience we've had a look fairly clear to me that most likely to go from the back to the front. >> she's not an o'clock? >> she said in a medical,
9:16 am
we've induced her, we very frequently wicker does your progress to make sure something catastrophic does occur. >> we turn off the sedation, that allows her brain to kick in and function of its ties level. it puts you in to a medical,, it but she does the buck and anesthetic you could turn it off and within minutes will be able to examine. she is now suffering in the bed from being operated on and also been on a ventilator. >> i do want to give you
9:17 am
live and let the experts tell you. >> because this borders on speculation for recovery i want to go into detail, and their obvious areas of the brain that are less tolerant to intrusion, it does not mean were using 10 percent of our brain, doesn't mean the other parts of the brain aren't important. it means the brain is able to recover from those injuries as opposed areas that would call eloquent. without use your going i don't want to go down the speculation road. at the same time we're cautiously optimistic. >> with the penetrating brain injury it's always surprising that you think any bullet going in any brain is instant death. there's a lot of capacity that the human body has and the ability for us to compensate. >> i attribute that to five
9:18 am
things. one, this dr. kevin stated the paramedics got to us quickly. they didn't do a lot of intervention. we have a system in place this place works like a finely tuned machine. everything at that time carried worked well. they got her here quickly dr. rainy was a trauma surgeon and did everything right. we got our quickly lined up and assessed the new would injury she had. we got our quickly scanned into the operating room as fast as we can. the anastasia's staff of the sleeping got ready for the surgery, the neurosurgery personnel. all of these things including the post top care the nurses in place everybody gets part of the credit everybody gets the credit. the other part is locked. in where the bullet actually hit when she first got here. >> there were a lot of other victims that came in.
9:19 am
>> you're hearing from the experts and doctors, neurosurgeons at the university of medical center in tucson who treated both gabrielle gifford congresswoman and of course the other dozen victims that were heard. what we're hearing actually some of the advancements we have and brain trauma treatment that allow them to help gabrielle gifford. it also the fact that she was lucky that the damage to our brain could of been much much more serious but she seems to have cautiously optimistic survived. >> because of what they are optimistic worse in the next 72 hours will be critical. ♪ let's go out to the dmv
