in a high rise condo. now police believe they have the suspected arsonist. they have a picture of him, now they just need a name. the man looks inconspicuous. the images were taken at the time time there was a fire in the area. he hopes the release of these surveillance pictures taken on two separate occasions last week will pre prevent any more fires from igniting in his neighborhood. a firebug has started as many as one dozen fires over the last week. some have been at construction sites in the area, others newsstands and dumpsters. >> he said call 911 there's a

fire. >> reporter: that's him running out the door. he ran to put out two fires set on his building. >> without the fire i probably would have ran out to the street. >> reporter: the arsonist most dangerous fire yet inside a high rise condo increasing concerns. >> much more, yeah. we have our car, our property, our bikes. yeah i think it's very bad. >> reporter: those images were taken just last wednesday and thursday. both of those days firefighters responded to two suspicious fires. reporting live tonight here in san francisco, i'm heather holmes, ktvu channel 2 news. fire investigators in union city are trying to find out what sparks a blaze that killed one man today and injured another. it happened just before 3:00 this afternoon at a home in the 3300 block of san carlos way. firefighter say clutter inside the home may have helped the

flames spread. today's winds were also an issue. a viewer shot this video as the house was burning. he says neighbors rushed to help but the fire was too intense. >> i can tell you that the house, the house was completely engulfed with flames on the side of the house. which would be the entry level where the house is at. >> reporter: the man who died in the fire was a senior citizen who lived in the house with his adult son. oakland police say a woman in her 20s was shot and killed this evening and a man with her was wounded. police say the two were found in the 600 block of douglas avenue at kerry avenue. that's off eves avenue south of the oakland coliseum. it's a residential area. when officers arrive they say the woman was dead and the man was taken to the hospital. his injuries are not life threatening, the woman is oakland's 42nd homicide of the year. the investigation is still in its early phase. so far police have no suspects or motives. continuing coverage now.

the ntsb report on asiana flight 214 and the factors that contributed to the crash at sfo almost one year ago. today the national transportation safety board released its final report on the asiana crash. it found the cockpit crew relied too heavily on the crew's isolation. they decided to abort the landing too late. we get more now on the proceedings from ktvu's jaqueline fell from washington. >> reporter: the national are transportation safety board said the pilots did not understand the complicated automated system on the plane. >> look at that one. >> reporter: the national transportation safety board found the pilots on asiana airline flight 214 mismanaged the landing causing the plane to come into sfo too low and too slow. the massive jetliner smacked down on to the runway creating havoc on the ground. >> oh my god, it's an accident. >> reporter: at the end of the day three people were dead, 49

injured. >> we found some things the pilots should not have done. >> reporter: the pilot did not have adequate training for a visual approach at sfo. and pilot fatigue likely played a role. investigators also said the complexity of the boeing 777 auto pilot and auto system contributed to the crash. boeing disagree that is the auto pilot system the contributed to the accident. >> the auto system worked as designed but the pilots did not fully understand what the system would and would not do. >> reporter: emergency staffing helped save lives but there were also accidents like when a

fire truck ran over a teen. three passengers were ejected as the plane broke apart. two of those people weren't wearing seat belts. investigators say had they been they likely would have survived the crash. in washington, jaqueline fell, ktvu channel 2 news. the ntsb released a new animation of flight 214 breaking down just what happened in the cockpit in those critical moments before the crash. about 5 miles from the runway the plane was still about 400 feet higher than it was supposed to be so the crew began to slow down the airplane. the pilot switched off the auto pilot and moved the level to idle. at 11 seconds from impact an alert sounded because the plane's air speed was too slow. you can see on the profile view that black line shows just how low the plane was compared to the green line which is the target glide path.

then four second later the pilot monitoring advanced the thrust levers followed by stick shaking acceleration but these actions were too late and the plane hit the sea wall at 122- miles-an-hour. in a statement asiana airline says it's enhancing flight training and evaluation and that quote the recommendations made by the agency can help ensure such incident does not happen again. there were also problems after the crash on the ground where a teenager was run over by first responders. in our next half hour, san francisco's response to today's report and the changes officials say they've already made. new at 10:00, residents of an apartment building on san francisco's knob hill tell us they're angry about a broken elevator. they say the elevator has been broken since friday and they've been told it may take up to six weeks to fix. that's one issue. residents also say the owner of

the prado group is refusing to have someone operate the building's freight elevator. >> the woman told me, prado can't afford that and i was furious. >> reporter: we're told some of the people who live there are elderly or in poor health and have trouble getting up and down the stairs. we reached out to prado and have not heard back. the san francisco building inspection department has put prado on notice for violations. police in oakland are asking for the public's help tonight in finding a missing teenager who is considered at risk. 15-year-old boston marion fortenberry was last seen yesterday. he is described as 6'1" and weighing 190 pounds. he was wearing a tie die shirt and blue shorts. the teen is considered at risk

at the oakland airport. ktvu's amber lee is live at highland hospital where he's being treated, amber. >> reporter: the injured firefighter is a 28 year veteran. city officials are not yet releasing his identity but a source tells me he's a devoted firefighter and husband. the california highway patrol and cal osha are now investigating what went wrong to cause the firefighter to roll over the vehicle he was driving during a drill at 11:43 this morning. the fire chief says this was a monthly exercise on how to respond to plane crashes at oakland international airport. a source tells me the firefighter had been participating in this type of drill for the past seven years or so. >> right now the department has been shot. like i said this firefighter was very well known and liked with all the members within the organization. >> reporter: the firefighter was alone in what's called an aircraft rescue fire fighting rig. it's equipped to carry 3,000-

gallons of gallons and 420 gallons of foam. it's unclear how fast he was driving. the vehicle rolled during a turn in front of the oakland maintenance center which used to be the united airlines hanger. >> we're not sure what happened so we're, going to have to wait on the results of the investigation. >> reporter: members of the oakland fire department have arrived here at highland hospital to show their support. a source describes the firefighter as a truly wonderful person who was devoted to his work serving the public. we've learned he's married with no children and considers his wife his best friend. one source tells me the veteran was just a few years from retiring. >> we are really praying for him, you know for his recovery. we're praying for the strength of his family. we will be staying at the hospital until he gets into a more stable condition. >> reporter: chp investigators hope to talk with a firefighter either tonight or tomorrow as soon as he's well enough.

reporting live in oakland, amber lee, ktvu channel 2 news. a robbery suspect is under arrest tonight after a quick thinking neighbor took action. investigators say 51-year-old james murdoch broke into the bolders apartment complex in walnut creek through a front door and filled a grocery bag full of jewelry. but an alert neighbor intervened. >> we're on the second story and i chased him down. i caught him about 2/3 of the way down just by his collar. i head him in a head lock and he was reaching at me with a screwdriver. >> reporter: the neighbor was stabbed in the leg with a screwdriver. grant's father has filed a lawsuit seeking unspecified damaging for the death of his

son. oscar grant iii was shot by a b.a.r.t. police officer. his father is serving time for first degree murder. morgan hill police are searching for three people wanted for attempted murder in a drive by shooting. they are identified as francisco diaz, 21-year-old nicholas young and deja marie ayala. diaz and young had an argument with a man then got into a car driven by ayala. as the car passed the other men shots were fired. one person was wounded he is expected to survive. the three suspects should be considered armed and dangerous. levi stadium is set to host their first event.

return to classes. >> reporter: jimenez remembers seeing something about sexual assault on the application. >> it must have been on sexual harassment or drinking or something. >> reporter: do you remember take ago class? >> no. >> reporter: and she was not the only one. universities do not ensure that all faculty and staff are sufficiently trained. the report says universities risk having their employees miss reporting assaults without that training. and coaches must learn how to respond when a student comes to them with a problem. >> obviously there's still a lot of work to do. we need to expand our training and educational programs. >> reporter: and make those programs mandatory. uc davis and ucla already do that. berkeley doesn't yet. >> some campuses actually do have a gate in place so that if you don't complete your

happy with the precidio location that started it all. there was a real worry, a big museum would cut off access to this spot on the bay. >> deeply grateful that we still have access to this beautiful venue for playing. >> reporter: los angeles was the other city to lose out in a statement today mayor ed lee expressed his disappointment with the decision in san francisco ken pritchett, ktvu channel 2 news. the latest ktvu poll shows governor jerry brown with a large lead in the governor's race over his republican challenger. the poll finds 52% of likely voters supporting governor brown. 32% favor his republican challenger neil kashkari. the field poll finds brown's support is broad based while kashkari has 32% of the

support. right now there are no cannabis clubs in unincorporated areas of the county but there are about 80 in san jose. city leaders worry those clubs will want to move into urban areas. they want to have regulations in place in case that happens. >> what the board asked for is study the ban to make sure we're taking care of compassionate use and access. what we want to do is balance so that children don't have access and people who need medical marijuana do have access. >> reporter: the county says it received 10 inquiries a year from people who were interested in opening dispensaries in unincorporated areas. returning now to the asiana214 flight crash. the ntsb also addressed troubles on the ground including the 16-year-old girl run over by rescue grews on the

police released a sketch. the man entered a business on south san mateo drive and robbed an employeen at gunpoint. the suspect is described as 6'seven -- 6'and 7" tall and weighing 275 pounds. the suicide of audrey pott of s era toga prompted the legislation. she took her own life after photos spread on the internet of her being sexually abused while she was pass out at a party. the bill now calls for rehabilitation instead of a mandatory two year sentence for teens who were convicted of sexual assault while a victim is incapacitated. however it includes an enhancement for those who share photos through text as a crime.
