resignation of the boston latin head master in june. now the results of a far- reaching more comprehensive review into racially charged allegations dating back to the 2012 school year. the district witty office investigated 115 reports of race-based incidents. of those, it identified 17 instances where district policy was not followed. in six cases, administrators were found not to policy. teachers and substitutes didn't follow policy in four cases according to the report, and in 7 cases, students were found not to have followed district policy. 31 allegations were not formally investigated because the office of equity felt they lacked sufficient evidence. all of this coming amid a federal investigation into accusations of harassment and discrimination at the prestigious school.

12 days later, investigators announced it was a cigarette. you. >> know, everything is very dry. the wood on the porches, and all it takes is just a little, you know, flicker of a spot. >> reporter: news of the cause came as salvage crews gave the charlestown mom the ultrasound picture she saved of her baby in the womb and a scrapbook scooped up from the remains of her fire-ravaged home. >> just shows once a starts, it can really affect a lot. >> reporter: scenes like this, another fire in dorchester that forced 10 people out. last month in manchester, two adults and two children killed in a fire, also started by a cigarette. >> for a cigarette to cause that much damage, that's crazy. >> reporter: jen mart sip an amazon delivery woman and

the cause is still under investigation tonight. the driver of this chevy tahoe is in the hospital after this suv crashed into a tractor trailer. policesay they tried to pull it over on route 20 about 11:30 but they wouldn't stop. tire spikessed them down. police think that the driver of the suv had a medical problem and that person is hospitalized. after his bike hit a police cruiser about 1:30 this morning. policesay the bike didn't have headlights and there was another bike in the area but that took off. a wake just ended for 7- year-old kyzer willis who drowned at a city-run summer camp, his family held a vigil last night. the boy accidentally drowned in the water about 20 yards off carson beach.

tomorrow. a school is settling sex abuse claims. dozens of people came forward to say that the teachers and staff at st. george's in middletown abused them. the claims go back to the 1970s. the school admits it didn't report any of the allegations. no word how much the settlement is for but a mediator is determining how much each student will receive. and the search is on for a suspect who robbed two last night. policesay the suspect is a white man a gray sweatshirt, jeans, and a camouflage hat. a live look at the boston skyline, and this is the last time this year -- slightly depressing -- that the sun sets after 8:00 p.m. >> no. a sad moment. and eric, the signs that the summer is coming to an end.

new numbers show problem in massachusetts does not seem to be slowing down. >> over the past 5 years, the number of overdose deaths in massachusetts has more than tripled. last year alone, more than 1600 people died from an opioid overdose. as massachusetts continues to be on the forefront, some cities and towns are using a new type of narcan in hopes of saving more lives. >> and this requires 1 dose, not 2. bill shields has a
