>> they have been notified of these violations over and over and over and over again. >> instead, it's putting them at risk. only on 5, we investigate complaints of a shelter in shambles. they are facilities tore people with serious mental health issues. licensed to operate by the state. from the outside, a majority of them look like regular homes in your average neighborhood. but in some cases, behind the front door lies a different story. news 5's kristin volk has been investigating adult care facilities for months. kristin, these are vulnerable residents in the worst conditions. >> it certainly is disturbing danita. it's happening right here in our own communities all while your tax dollars line the pocket of the landlords. >> this is the fly droppings on the mattress cover. that's what we had to sleep on.

you cringe. >> this is all mold. >> reporter: and so diss video. >> when i went to bed, the bedbugs were so horrendously awful, i could not sleep in either bed. i actually slept on the floor. >> reporter: but all of these bugs, all of this filth, that was daily life for ed bricker. life here at care circles three homes on east 89th street. >> that would the morning. every morning. >> reporter: care circle was supposed to be a place for ed to get back on his feet. a place where he could get help for his mental health issues. >> i would lay on the floor and cry for hours. >> reporter: severe depression. bipolar, a.d.d., anxiety. he never imagined he would hit a new low. >> i actually considered going

>> this is definitely one of the worst that we've seen. >> reporter: disability rights ohio, a nonprofit that acts as the state's protection arm for people with disabilities, has its own pictures inside care circle. >> why isn't this being cleaned up? >> reporter: these photos taken just four months ago with the organization's investigative team showed up to care circle here in an announced. it was the team's fifth visit in the past year. each time, they report back to the little improvement. >> this is a facility that has been given too many chances. they have been given too many opportunities. >> reporter: mia, i would like to talk to you. i'm kristin with news 5. >> reporter: we gave care circle an opportunity to respond, but they did not. >> that's awful. >> reporter: sanitation problems date back to 2011. according to this state inspection report. ed moved in three years later,

funded by cuyahoga county, and each year the owners rake in hundreds and thousands in rent payments. >> reporter: i would like to know why you continue to take taxpayer money while refusing to clean up these homes. would you live in a home like this, with these photos? >> reporter: they repeatedly refused it licenses care circle. ed is no longer at care circle. he now lives in another facility after he says he complained enough times to county officials. but he worries about others just like him who suffer from mental illness, yet unlike him, they can't advocate for themselves. or they don't know how. >> there are people with mental health issues that really need

advantage of. >> reporter: these records show that a year ago the state did initiate administrative action against care circle. that included a threat to revoke the home's license, and a hearing was supposed to be held. but the state tells us there is no hearing scheduled, saying, quote, at this time the department has been conducting ongoing work to help care circle correct its deficiencies. in the studio, i'm kristin volk, news 5. there are new questions tonight surrounding a north ridgeville man accused of murdering his girlfriend. despite having a history of domestic violence convictions, randy hamilton was allowed to have a gun. news 5's derick waller live tonight outside a elyria district court. >> reporter: in 2010 temporary orders leave a lot of discretion to the judge.

deadly weapons ban. well, that did not happen here. our investigation uncovered a history of domestic violence and assault convictions dating back decades, including a 2010 temporary protection order filed by hamilton's ex-wife on behalf of herself and her two children. >> it appeared that they were arguing. he pushed her. she fell backwards, split the back of her head open and had eight staples. >> reporter: it was filed in the same judge's court filed a temporary restriction order. hamilton told a dispatcher he just shot his girlfriend. >> where is the gun at? >> i threw it in the bed radio. i will throw it in the garage right now. >> reporter: that order has long since expired. but in may an "on your side" investigation found that judicial discretion constitutes

battling drug and alcohol addiction say issue 35 would save lives. so far this year, 80 people have died in lorain county from drug related overdoses. now the lorain county alcohol and drug services agency is asking voters to support a levy that would allow the agency to hire more social workers and offer more programs. elaine george told me the agency is so busy, addicts are dying on waiting lists to get treatment. the agency estimates 20,000 people need help with dr county. the five-year 1.2 mill levy would generate $7.2 million. >> we're paying for it already. our jails are gore crowded so we're paying for the cost of people in jail. there's higher emergency room costs, which will probably roll into higher insurance premiums for all of us.

your future? >> reporter: in about five minutes, yes, there is. the center of the baseball universe has moved on from the corners of carnegie and ontario to right here, the corners of waveland and sheffield. game three of the world series tomorrow. we all know in big events like this, priority number one is to keep everybody safe. chicago police say there will be more than 1,000 law enforcement officers from the police department, the sheriff's department, fbi, homeland security, all eyes right here just like in cleveland, everybody is going to have to go through security check points. they are going to be two blocks from the stadium. a lot of the roads around wrigley field are going to be shut down starting tomorrow but to keep a huge crowd with no particular from gathering around, the police department is encouraging people to watch the game elsewhere. >> if you don't after ticket to the event, we encourage you to gather with friends, order some food, and celebrate at home in your neighborhood, as traffic

that i've ever heard about the crooks didn't take anything. here's what they did, according to police. they broke into a doctor's office and performed some sort of medical procedure. news 5's bob jones tells us the who and what are baffling investigators. >> reporter: the bath township police chief told me he has never seen a case quite like the bizarre crime committed at this doctor's office. it leaves many unanswered questions including who would break in and then perform a who's the patient? and where are they now? >> who does this? i mean this is so strange. >> we have no idea. that's what we're trying to get to the bottom v. >> reporter: photographs captured the evidence that someone used machines and conducted a medical procedure after the break-in at this family practice. instruments left in bowls, a surgical glove tossed in the trash along with medication wrappings. even a purple gown left on a table. >> it's very bizarre.

medical procedure. >> reporter: based on the machines and instruments used the chief believes an ear procedure was dunn. he says whoever broke in disabled an alarm. the chief says none of the employees used the practice during that time. >> you would certainly be concerned about the person the procedure was performed object. was there sterile instruments used? was it performed properly? >> reporter: carolyn calls the situation scary. she owns the boutique across the street. >> it's mccabe. it's right out of a screenplay. >> reporter: in a statement, the university said there is no confirmation at the moment about the misuse of equipment or supplies. but police say at the least this was breaking and entering, and apparently much worse. >> i would assume there's some type of charge out there for performing a medical operation

we're just trying to determine who the suspect is. >> reporter: in bath township, bob jones, news 5. with less than two weeks to go until the election republican donald trump took another day of the campaign to make a swing through ohio. he just finished up his second rally of the day with supporters in toledo. earlier he was in springfield where he kicked off his day long trip around the buckeye state. trump who has been adamant on the trail that he is going to had a slip, and his supporters didn't let him get away with it. >> if we win on november 8th -- when. we don't want to take it for granted, but we are wing in a lot of statements. >> and in less than an hour, trump is holding a campaign rally in ashtabula county, geneva. let's get a check on our

>> we've got clouds that are trying to thin out, but we will be hard-pressed to see any glimpses of clear. our current temperature is 50 degrees. that is right along the lake shore. inland, you're much cooler than. that we're tracking a few sprinkles coming in off the lake. notice, though, north shore beginning to see some clearing. so some of you will see a few glimpses of stars overnight, but notice our hour by hour stl isolated sprinkle through much of the overnight and even carries the sprinkle threat into ashtabula county where most of us are drying out. mansfield, you could clear out before sunrise. but i'm thinking tomorrow we will see quite a bit more sunshine breaking out. there you go. there's 7:00 a.m., 8:00 a.m. tomorrow with that isolated sprinkle over in ashtabula county. right now, though, look at the temperatures. we are in the 40s just about everywhere you go, except right
