david felix vasquez was the train's operator a 28-year veteran of the mbta. >> adam: investigators say the train wasn't receiving signals when it took off. >> kim: we have nightteam coverage from the passengers on board to the latest on the investigation. let's head out to the nightteam's ryan schulteis in sub. this is where the train spent most of the day as investigators are trying to figure out how this happened. a wild ride for commuters on the red line a runaway train with passengers on board with no one he controls. >> the train picked up speed and went from five miles to like 30 and kept going and going. >> reporter: investigators say human error likely led to the six-car train taking off from braintree with 50 passengers on board and without the operator. "t" workers noticed it within minutes and cut power to make it stop but not until it had gone six miles and through several stations.

going investigation that involves folks from the mbta from dot and from public safety. from that we can tell at this point, it a poors to be an isolated incident. >> reporter: the riders are thankful they could get out of the train safely. >> there is a god. a lot of people could have gotten hurt this morning. >> reporter: the 50 passengers were not hurt, the "t" operator was treated for injuries and released from the hospital. live in south boston, ryan schulteis. >> adam: tonight the investigation is focusing on the train operator and if there is something he did or didn't do that put the train on the dangerous track. tim caputo continues from southie with more on the investigation. >> reporter: the department of transportation trying to figure out if the powers they currently have in place are enough or if this operator made an error or a combination of the two. the dot says it wasn't a problem with the train. >> operator error is the current focus of the investigation.

transportation ceo wouldn't say what the operator did wrong, the red line train had a sig fal issue. the operator got permission to leave it and pull the switch to bypass the signals. >> the requirement is that the train be put in full service brake and a hand brake which is a manual brake. before the operator exits the cab to put it in to bypass. >> reporter: authorities won't say if the brakes were deployed. here's what a red line train operator area looks like, that is a lever. >> reporter: put in to full service break and the motor person is required to walk off to the side three feet from the operator's console, another large lever with a chain. that deploys the hand work on the train. >> reporter: there is a report of a passenger seeing a cord wrapped around the lever but they can only say they are

>> that will be part of the focus of the investigation. video the dot cleared other trains on the tracks ahead of the runaway train. and in bypass mode the collision avoidance system is disabled. it wouldn't have automatically stopped when it got close to another train. and there is nothing the passengers on board could have done either. >> there is no access for them to get in the operator area, as a matter of security. >> reporter: a multiagency investigation is underway to prevent this from happening again. >> we have procedures in place and training in place to ensure that our passengers are safe, and feel safe. and we failed our passengers today. >> the kept of transportation has in the said whether or not that operator has been disciplined but this morning's incident he has been placed on administrative leave. tim tim caputo. >> kim: we are hearing from

that red line train. we are heading to braintree now with their stories. >> reporter: you can imagine the reactions were all over the board, understandably. one man said he knew there was problem from the beginning. an early morning commute turned scary for about 50 "t" riders on the red line on thursday, some resorting to drastic measures. >> people were trying to break the windows so we could get out. >> reporter: she was in the second car trying to stay calm. and wait. >> reporter: one passenger on his way to work said the taken took off from the platform too quickly and it got worse, blowing by stops. everything was dark. everything would stop. and we were trying to open the doors and we couldn't. >> reporter: once they realized there was this conductor some tried to call the mbta.

and this is an active scene as they try to figure out what happened and look for the shooter. >> he was like out of breath. help, help. >> reporter: neighbors say they heard fighting and a scuffle and desperate cries for help from the first floor apartment, when they ran down the stairs they saw a man lying on the ground. we saw a man on the ground, lying face-up at the front dr and bleeding out. >> reporter: sources say one man and a woman in her 30s are dead and a third person a man, was shot in the face. neighbors describe a frightening scene. >> very bad. really not a beautiful thing at all. >> reporter: and back here live you can see police are continuing their investigation, neighbors tell us it all happen on the first floor apartment and that is where the man in his 60s ved and the toe people who were visiting were visiting that person and that is two people who died and one person who was
