into trucking. this time after repeated questions the federal motor carrier safety administration says not so fast. september 2012 two employees of the systems incorporated died when their rig blew a tire and ran off i-24. a tractor trailer twice in greens bureau and keeps going. a witness snapped photos and linked that hit-and-run truck to brake systems. >> i think it's fair to say they were scared they were going to die. >> representing the elderly couple in the car the woman died several months later after the incident left her too afraid to even get in the car.

>> the worst safety records in north carolina. >> a lot of red flags. >> a retired member of the state highway patrol works as a consultant on trucking safety points to this timeline. in 2013 reporting two drivers and one truck with an interim registration. that same year record shows a lead of drivers and 20 trucks. they received a permanent dot number and within six months the company was on dot radar receiving a warning letter for unsafe driving. by august of last year records show nearly all of assets transferred to 50 drivers and 30 trucks. >> they could possibly be carrier. >> and the company history warrants investigation. >> if you got a reendarn ated character or bad equipment unsafe drivers a safe vehicle

life in danger. >> this past august the case took a mainly turn. the feds received a complaint of not caring about dot regulations making drivers start routes at night for proper pretrip inspections and a visit to headquarters. they found multiple problems including false reports from drivers. use of vehicles that weren't inspected and multiple on the road violations like following too close, speeding and failure to yield the right of way. inspectors also found them operating out of the same wake forest location. a week later investigators determine the two carriers were in fact the staple and ordered them to merge under the name. a bumpy history also transferred. >> they are putting everything into one lump category to make the investigation complete. >> we reached out to the family that runs elite and trucking

the head of the family who transferred official ownership to his children years ago told me he was planning to sell the company and get out of the trucking business. no other comment. >> that the company has failed to address the under lying problems. >> with little or no improvement and safety they say a shut down may be the only solution. >> instead it just seemed to keep switching identification numbers, switching owners but otherwise doing business as usual. >> back to that deadly kentucky crash the state industrial commission last year determined they had no worker's comp insurance and ordered the company to pay nearly a half million dollars for funeral spences for both of the driver's families. keep in mind with the on going continue to operate. >> cullen how wide spread of a problem is this. >> it's really hard to say and for one reason. it's hard for the federal government to prove all of this

here's the water vapor imagery and a couple things to note abundant moisture through the south eastern part of the united states. this is not the whole story because you look back in here and rotation this is a very strong system in the upper atmosphere that's going to link up with this area of moisture and spawn the development of a pretty strong area of low pressure that is then going to intensify and track up west of the mountains and try to scower out the shallow layer of cool air and replace it with a windy warm moist unstable air mass as we head into wednesday afternoon and evening and if it succeeds in doing that the ingredients will be there and severe thunderstorms and perhaps a couple of tornadoes. the two main ingredients are severe weather potential involving wind sheer and instability. wind sheer basically refers to how the wind is changing direction and speed as you go through the atmosphere and even though many times we like to

research has shown numerous papers have shown that tornado potential it's most important to look at the wind sheer and the lowest half mile of the atmosphere so looking at that here 7:00 tuesday night and overall that pattern is quite favorable with increasing values of wind sheer as we head on into wednesday and through the afternoon and on into the evening. so that scenario or that rerecycling exit if you will is there almost all of the time but you need the instability to go along with it so let's take a look at how that looks. at the same time the sheer is already impressive tuesday evening. no instability to speak of. so we don't have anything to worry about tuesday night into wednesday morning there isn't a lot going on except down here in far south eastern counties but as we head into the mid day and afternoon hours the instability increases dramatically and at the same time the wind sheer in a favorable mode so both those

potential again for severe thunderstorms and perhaps even a couple of tornadoes on wednesday afternoon and wednesday evening. that will be our focus for the next couple of days. all right so 8:00 this evening temperatures holding steady from where they are right now with periods of mainly light rain and the same thing overnight with temperatures dropping back to the mid 40s. tomorrow looks like a cool clammy day low 50s south and east of raleigh and north and west perhaps holding into the 5040s best chance of measurable rain in the morning and a misty type environment into the afternoon. then on wednesday we may very well start much like we are going to finish tuesday sort of cool and clammy and all of a sudden during the afternoon it becomes windy and temperatures rocket up towards 70 and showers and thunderstorms begin to develop and at this point we can hope they are plain old showers and thunderstorms to make people aware. >> we know you'll keep an eye
