named versions of a medication really identical? dr. joshua joins us this morning with answers to that. good morning. good to see you, as always. when can a generic version of a brand named medication become available to the public? when does the fda say it is okay to have a -- >> well, a patent is provided for 20 years, protection. but that patentish ud before the medication reaches the stage where it can be available to the public. generally the development phase takes about ten years. so you are talking about having brand named medication available to the public for somewhere around 7, 8, 9, 10 years. >> all of the other -- companies get -- >> after that, once that pantent runs out, then general air can become available. >> we assume generic medication is identical to brand named medication. is that true? >> this is where a lot of people may be surprised here. what the fda requires is something called bio equivalent. es enly what that means is that the generic medication needs to
11:43 am
enter the bloodstream and come to roughly the same levels in the person's blood and as the brand named medication does. it does not have to be exactly the same. there is actually a range so that the medication can have about 20% less in your blood than the brand name or up to 25% more. so you are talking about a fairly wide range. now they have done studies that have found that in general, generic medications differ from the brand named counterparts by about 3.5%. that's the way they are in general. that's not the requirement. requirement is that range which i think will surprise a lot of people. now the fda is not unique in having this range. this is actually the range that sort of is standard for most countries around the world. >> many of our insurance companies, though, will pay for the generic but not for the brand name. >> exactly. lot of insurance companies because it is a cost savering to them, they encourage people to go on the generic medications. in general, generic medications
11:44 am
are perfectly safe and they are going to be fine. but one thing people really need to be aware of is that there can be multiple manufacturers of a single generic medication. take, foinstance, the medication prozac. the generic version of prozac has at least ten different generic manufacturers. so each of these medications, generic medications, may have a slight difference between them because there's differences in the fillers and binders and so there might be a difference in the amount of medicine reaches somebody's system. it is possible somebody could take one generic version of prozac which has 125% of what they are used to with the brand name. switch to another generic and it could have 80% of what you are used to with the brand name. 45% difference in the amount of medicine they are take. >> what about side effects? do they differ, too? >> side effects could differ as well. >> how is a person to know what to do? >> it is tough. what you really need to do, i think is you need to make sure if you do switch to a generic,
11:45 am
which i advocate, i have lots of patients on generic medications, i think you just need to make sure whatever generic medicine you are taking you stick with. what i suggest is when you go to your pharmacy, and you get your medication filled, make sure that that generic medicine is the same one from month-to-month because the pharmacy may change who they purchase that generic from without telling you. you get home and you open your bottle and the medication looks a little bit different. it is the same active ingredient but it might respond in you a little differently than the one plier. you need to open that bottle of medicine up in the pharmacy because you need to tell them hey, this isn't what i got last month. because once you walk out of that pharmacy with your bottle of medicine you can't bring it back. >> what about if you take the generic and you react? you feel a difference than you felt from the brand maim which you may have started with. >> issue may be that maybe you are falling into that category where only 08% of the medicine is getting into your system. you need to talk to your doctor and explain that this generic does many seem to be working the
11:46 am
same way. interestingly enough they did a survey that found that only 17% of doctors actually understand what i just talked about. they don't know the fda approval process. you may actually need to educate your doctor and then they say oh, okay, we need to go up on the dose of the medicine or down or maybe we may switch from one generic version of the medicine to another generic of the medication. >> maybe we have doctors watching and they got a lesson. >> i hope so. >> thank you. always great to see you. next wednesday, we will be answering viewer questions. if you have a question just send an e-mail to -- he is a psychiatrist and a physician. you may want to ask him any questions with regards to your health. he will be a part of news4's health and fitness this weekend. walter e. washington convention
