end. i know that all this is a lot, but what are the highlights? >> reporter: rep there were major communications for responders. getting new details about that. a 911 transcript and go ahead and show the video. there were problems communicating with the radio panel for d.c. fire and rescue. some had to use cell phones to communicate in that smokey tunnel. we are finding out back on january 8th the fire department notified metro that there were major problems with communications in the metro station. this is highlighting that metro knew there were communication problems four days before the event happened. it shows that two people self-evacuate and that's a dangerous situation. the third was in operation when they got out of there. they were in fear for their life and they went ahead and got out. a lot of other people stayed

inside the metro train. this transcript shows that people were frightened and calling 911 and coughing and saying that people were suffocating in the train and begging for dispatchers to rescue them. very fearful. when you read through the transcripts, you see how frightening it was for people stuck inside the train cars in that smoking tunnel and as we see from the video, the smoke was filling up in the train cars. many were fearful they were not going to make it out alive. the 911 tribute transcripts were showing how scary it was for the folks. this is a 37-page report by the fire department just this morning. we were digging through it looking for the new information for you. a live report in the next half hour. we are reporting live from the wilson building news 4. >> thank you, darcy. our transportation reporter was from home and he has done

will meet at the library going on at noon. on wednesday night, a 17-year-old was shot and killed in a double shooting on martin luther king,jr. avenue southeast. they are offering a $25,000 reward for information that leads to an arrest and conviction in the case. >> a few minutes ago, the chief was discussing the recent violence as leveragy leaders are gathering to review the shootings as well. local leaders are asking to stay vigilant until someone is charged in the crimes. they are scared to let their children play outside. just this week there were five shootings in a matter of 24 hours. a meeting will be held at 1:00 at the boys and girls club. >> all new, six former virginia attorney generals say former virginia governor bob mcdonald should remain out of jail while

paterno the winningest coach in major college football history. they stripped them of their wins following the child molestation scandal. authorities at the university of virginia look different this weekend. new rules went into effect for all fraternities and sororities. kegs are banned and they must be patrolled by security. all organizations have assigned the new agreement. two fraternities refused to sign. they pushed ahead with the policy after a rolling stone magazine report about a gang rape on ground. much of that has been called into question. >> sex assaults on college camps, there were 3900 nationwide but what is alarming is that it is believed 80% of rapes and assaults go unreported. northern virginia bureau chief

breaking news all new this morning. d.c. releasing their initial reports to d.c. mayor on the response to metro's underground emergency. >> person died in monday's accident and we have been reading the 37-page report since it was released. we want to check in with darcy spencer live with they are stuffed inside the cars not knowing if they would make it out alive. they show some of these callers and what they were saying to 911. one said okay but we need something to happen. people had to get on the floor because they can't breathe. a third caller hi we are on

the train and it's filling with smoke. we have been here half an hour. we are having trouble brothing. when are we getting help. later on in the transcript you can see where a metro officer was asked how long it's going to be. he said i can't tell you how long. frightening for the people in the tunnel not knowing if and when they would get there. there was a lot of confusion whether they were to the operators or metro police. as we have been reporting, there were communication problems among the responders trying to get in that tunnel and rescue the victims. that's what they want to do. they are hampered by communication problems. the evaporate shows that fire alerted metro days before this fatal incident and there were major radio problems at the metro station.

power to the third rail. wey ing spoke to a congressman who calls it unacceptable. >> the preliminary report is illustrated with images of what is described as severe damage to a section of the third rail that was about 1100 feet in front of yellow line train 302 when it came to a stop at throw3:15 p.m. minute. the report said that at 3:06 the breaker at one end of the trip and ten minutes later, metro operations activated lentvation fans in an effort to exhaust smoke from the tunnel. according to a fire department document when first responders arrived to evacuate riders nearly 40 minutes after the train stopped, after the third rail had been shut off, they said there was so much smoke in the tunnel they could not see the tail lights until they were

arrest. >> a pennsylvania man is facing emergency charges for charges for injecting a woman's butt with silicone. it took place in a capitol heights hotel room. the woman eventually died. the people who do these illegal procedures are hard to track down. >> no one knows who it is or the credentials of that person or more importantly what they are injecting. silicone gets into the blood stream and can result in a series of complications and many of them can be life-threatening. >> she charged with second-degree murder and assault and reckless endangerment. he was arrested in st. louis and brought back to prince george's county. how a long walk home by local children is fueling a debate about free-range parenting.

we continue to following that breaking news. d.c.'s mayor releasing the report on the deadly metro >> we have been going through the about 37 pages and a lot of information in here. >> we are joined by our reporter and a lot of members of our team skimming through the report. you have been on it since it came out and what is the most alarming thing you have been able to find so far? >> i think the most important thing is this is not good news for metro. it points to a lot of communication issues which we were suddenly aware of happening. you start to find out about the communication and the most damning and alarming thing that came out of this is included in this report that shows that personnel has been saying before

the january 12th incident that there were communication problems not only in the metro station, but the tunnel. that is a big, big finding in this report. >> adam, we saw the release or a preliminary report yesterday. how did that differ from what the mayor put out? >> the report is more based on what caused this whole thing. the communication issue while a big factor in response to this thing is not what caused the issue. what caused the issue still is everything is pointing to the massive situation and the arking he has been hearing about and getting pictures and seeing the damage that happened. still everything points to electrical cables and power and a major, major electrical event. you have to figure out why that happened in the first place. of the maintenance not done or

was the system not maintained? >> real quick, what is your general consensus. when it comes to safety and how the people feel when they get on the train. things are heightened but this adds to the problem. do people feel safe? >> i think that's up to each individual person. when they had the crash in 2009, they said is metro safe. her answer is they want metro to be as safe as it can be. i think that's up to interpretation. obviously this is going to cause more heightened fear but there a lot of people who will get back on the trains tomorrow. people will get on the trains today and say that's how they get from point a to point b. if you look at the statistics comparing metro to the highway,

clearly metro moves 700,000 people a day. and often times it delays and you never talk about injuries or death for that matter. this is the first death they had since 2009. more people are killed on the highways every day. it's a matter of interpretation but the bottom line is that this incident got out of hand and no one should get on a train wondering if they will make it alive. people put faith in metro. that's really what it boils down to. you have to be able to think you will get on a chain and get to where you need to go. >> the story from home this morning. we appng up early and helping us out. >> we do have that full report on our website. you can open it up on your phone inside the nbc washington app. we made it easy for you.
