back to you. >> another mishap inside the district virginia va medical center. surgeries delayed and operating rooms shutdown for three hours. news 4 has learned the feds found air system fill your. we told add to a growing list of problems at that medical center. it's on irving street. he's in our newsroom. >> since february, we've learned about a series of mishaps in this facility. this one shuttered one of the six operating groups late last week, eye surgery for patient and was down for about three hours. the news 4 i-team revealed agency record got a different problem of the surgical center in march that cracks in the surgical floor. elsewhere, they found holes and repaired those holes in walls inside the facility. and in april, inspection revealed medical supply shortages and unsanitary conditions in storage
