people died because of hurricane maria. the that means the death toll that actually is 20 times higher than we were first told. there was no fmal announcement about the new death toll but it appears in a report submitt to ngress this week as part of the island's reconstruction plan. the new death toll cludes the four months after the storm when widespread flooding cut off rural neighborhoods and power was out for almost all the island. we traveled toic puerto soon after hurricane maria hit our news 4 and saw the humanitarian effort led by military medical teams and we saw non-profits delivering clean drinking water and feeding crowds of people. recovery there has been happening y.ry slowl after a year the puerto rican government claims power has now been restored to all but 25 people. a sinister plot uncovered inside this isolated compound in the new mexico desert. some of the 11 children rescued

tonight a 4 i-team investigation reveals a series of major equipment failures inside the flagship medical center of the u.s. department of veterans affairs. scott macfarlane is here to explain the safety risks and he also talked to some frustrated patients. >> reporter: air conditioning and other refrigeration issues ruining vaccines and also causing a flood. the ridge federation failure forced the medical center to throw away more than20 000 flu vaccines that spoiled lastl. fal months later the fda found issues in the blood bank linked to warm room temperature. that was in march. in may a cooling pipe triggered a flood on multiple floors and foeved theuation of a
