and 23 are injured after a school bus crash in chattanooga, tennessee. the bus went off the road and crashed less than a mile and a half from woodmore elementary school. it's horrifying. let's walk through this. just before 3 o'clock monday afternoon, 35 children - kindergarten through fifth grade - loaded onto this school bus headed home and within minutes, the unthinkable. the bus flipped on its side, slammed into this tree and split it in half. people who heard the crash ran to help and

gruesome scene. the children who could, scrambled to get out, climbing through a roof hatch. parents rushed to the scene. you can see some walking away clutching their young children. reporters say some of the children they saw walking away had blood smeared on their faces. this rescue effort was intense. it took paramedics more than two hours to get all of the children off of this bus. investigators are still trying to figure out what happened. at this point -- they're pointing in part to speed. this road where this happened. it's residential, two lanes with curves. one boy told reporters that the bus was going really fast, the driver lost control, they hit a mailbox and then the tree. that driver is talking with police, they are not ruling out drugs or alcohol. the next step for investigators will be tough -- there are cameras on that bus. they're going to have to go through all of that video to see what led up to the crash and what happened in the minutes after. as for seatbelts, they aren't required in tennessee. for more on the florida rules and

our 10 news facebook page. simple fixes that could save your life are getting ignored by some people you pay to get around. 10investigates uncovers the alarming number of vehicle recalls in taxicabs, as well as in uber and lyft cars nationwide, "hazards for hire." while the fixes are easy, many don't want to lose business. investigative reporter noah pransky teams up with "tegna-owned television and online partners" around the country, to tell us: too many professional drivers are neglecting t fresh off the plane in tampa, sisters zetave young and evakay maluvac were anxious to get on the road...so they could start their 5-day cruise through the caribbean. they aren't really uber people... the older you get, you just do things you know. you just trust a taxi. and they figured a cab was the most reliable way to get across town.....any town! i just assume they're safe and get us where we're going, even though we don't know where we're going. but they also had no idea if the cab they hopped in...

been recalled by the federal government. parts like takata airbags, gm ignition switches that can shut-off mid- ride....or sticky toyota accelerator pedals. see, while almost every major city in america has some sort of oversight for taxis.... it seems almost none of them bother to check if those same vehicles have dangerous defects that need replacing. "safety recalls - they all do impose some kind of safety risk in many cases it can be very very serious." kelsey mays is with cars.com, who partnered with us and dozens of tegna television country... to examine how often the vehicles that drive you around.... had risky recalls that got ignored. recalls that may not affect every ride... but significant enough for the federal government to mandate postcards and letters go to every affected vehicle owner, urging immediate fixes. it's just that..... the taxi industry didn't get the memo! super 45-50 "for passengers it's really like playing the lottery. when you're getting into these cars there's a one in four chance basically that these taxi cabs might have an unfixed recall." super 124-140

biggest taxicab regulators and most admitted they did not include recalls in their required vehicle inspections. from tampa to houston to seattle .... a startling percentage of the taxis we checked were driving around with outstanding recalls some had been ignored for up to 7 years! and even in new york city, where the taxi & limousine commission told us they require recalls to be repaired before each vehicle's annual inspection.... they apparently just forgot to check. a lot. because 27% of the taxis we looked up never got the work done. yo builder just forgot to inspect your home for recalled screws? we took our findings to a representative for tampa's largest taxi company. "how does something fall through the cracks when it comes to engine shutoff issues, seatbelt concerns, loss of steering? these are things that everyone notices the car owners and they just don't get done." super 217-228 "you say they don't get done, but since we haven't seen the data, we can't trust what you're telling us. we don't even know what you're looking at you could be looking at a blank

"i'm sure it's probably not." it wasn't blank.... we provided yellow cab the list so they could get the issues fixed. while some issues were benign....others were potentially catastrophic. their defense: recalls get ignored by uber and lyft too...also known as transportation network companies, or, tncs. "let's make a level playing field for everyone. what inspections have there been for tncs out there operating without any inspections?" he brings up a good point. both uber and lyft told us they leave those risky recalls drivers. and we found plenty that needed repair. with risks that included exploding airbags..... engines that stall out... and steering columns that can disconnect! how widespread is it? well, we aren't sure exactly. uber took us to court to block the release of their vehicles' registrations in seattle. anecdotally, we found the outstanding recall rates on rideshares right around that same 25%. it's the same lousy lottery you hope to never win. but after we brought our findings to uber's attention, the

reminded them to check their vehicles for outstanding recalls. because without access to the vin..... passengers have no way to tell. nuclear plant. an earlier tsunami warning - said waves could be up to 10 feet tall. but the largest wave to hit japan's coast was recorded at just over 4-and- a-half feet. thankfully, there were no reports of serious damage or people hurt. new information -- about a police officer killed in san antonio right outside police headquarters. 31-year-old "otis tyrone mckane"-- was arrested monday after a "day-long manhunt." he's accused of "ambushing and killing detective benjamin marconi"-- sunday afternoon. it all started when detective marconi-- pulled over a driver in front of police headquarters. investigators say the detective was writing a ticket -- when the suspect walked up to the driver's side, and shot marconi twice in the head. the plumbing problem dangers. 10:26 they're not supposed to work on the lines while school is in session. they're not supposed to work on the lines without telling the principal. but crews from the city of tarpon springs. were working on the lines. apparently allowing the pressure to build out of control. when the toilet exploded. burson had no idea how badly she was injured.. 4:10 the people behind me said you need to sit down. and i looked behind me and there was a big trail of blood. the incident took place back in 2012. details just now emerging in a lawsuit filed by burson's attorney detailing the painful injury. which eventually led to infection. 8:04 it was a tuberculosis strain of bacteria. that got in there. burson is left with an ugly reminder of what happened on her leg. hopeful city crews will use more caution when working on water lines. 14:41 i kind of lost a year of my life.
