mary agnes carey. guest: she asked a lot of good questions. i want to talk about her point, you have insurers in rural areas and markets that have decided not to offer obamacare coverage. in particular, arizona, iowa, tennessee are having issues. it varies by county. it depends on the insurance company. the general theme is when the affordable care act came in, people like her brother, got coverage. it would help him get out of the er and reduce costs. because he comes into the system sick, it is expensive initially. insurance had people sick people coming in initially where the affordable care act did not attract the young, healthier people want to get. insurers getting in may have under price their products. now, they have to make up for the factors we are talking about, sicker people coming in and not healthier people coming in. insurances are trying to figure out what this means. in an urban market, you tend to have more competition with providers. can get a better deal. in the rural markets, you cannot do it as much with providers setting

for more, i am joined from washington by mary agnes carey of kaiser health news. how did we get to this situation? you know, the president likes to describe it as obamacare imploding. but we're clearly seeing that there's a disparity in how much choice consumers have when they go to these exchanges. >> insurers have been very concerned about the uncertainty around the affordable care act. as you mentioned, there's been a lot of discussion on capitol hill about repealing and replacing the law. that fell apart this week in the senate. there's been concern about whether or not the trump administration would fund these cost-sharing subsidies that help low-income people with their out-of-pocket costs. and there's also been concern from insurers about tough enforcement of the individual mandate, this requirement that most americans have insurance is or pay a fine. so when you look at that collectively, insurers are getting worried, and some are deciding not to go into some markets or leave other markets or raise their prices to try to compensate for that uncertainty. >>

our northern virginia bureau chief julie carey has been at the scene all day. she joins us with the latest. >> reporter: we learned the man accused of killing the trigger turned himself into maryland state police in fredrick this afternoon. his name is ernest stickell. this wild road rage encounter started up on the beltway and came to a head here in the left-hand turn lanes. that is when a witness tells me the victim and her passenger got out stickell's van shouting and threatening him and police say that is when he turned his gun on the victim. >> a white male suspect wearing a black polo shirt. >> reporter: it was a 911 call that brought rush hour traffic at this busy intersection to a stop. at the center of it all, the victim's suv, windows shot out, a trail of glass on the road. this is the security camera video of the moments just after the shooting. the suv driver hit in the neck and back does a u-turn loop. it is hard to see here but the suspect in the white van turns right in the other direction fleeing the scene. >> i saw the white transit to the righ

let's get to julie carey who has a look at the security camera video showing moments after the shooting. >> reporter: not long ago we learned the identity of the man accused of shooting that female victim. he is ernest stickell. he was tracked down and arrested in virginia. let me set the stage. it is believed this dispute started right up there on the beltway. a witness tells us she heard horns honking and shouting as the two white van exited on to the connecter here. it was right across the street in that left-hand turn lane where the suv swerved in front of the van, came to a stop, both driver and passenger got out and that is when things escalated. this is the security camera video of the moments just after the shooting. the suv driver hit in the neck and back does a u turn loop. it is hard to see but the suspect in the white van turns right in the other direction fleeing the scene. the suv's windows shattered by bullets. this woman who doesn't want her face shown saw it all happen. she was taking a cigarette break. she says the two vehicles got her attention right away as they exit
