craig boswell has more. >> reporter: the rtdepament of homeland security review of the fence jumping incident found alarms and radios didn't work properly and also revealed a secret service officer removed his ear piece and didn't have a required backup radio, and was taking on his -- talking his own private cell phone in a van when omar gonzalez went over the fence. two members of the team had the intruder in their rifle sights but they determined lethal force was not necessary because the intruder appeared unarmed. construction and trees blocked the view of other officers. one agent raised his weapon at gonzalez just outside the north portico entrance but didn't fire because he thought the doors were locked. by the time the officer realized the doors were unlocked. gonzalez was inside what is supposed to be one of the most secure buildings in the nation. a female officer tried to take him down but he was able to overpower her. another officer and two plains clothed agents eventually subdued gonzalez. they were staffing shortages and it blames lack of radio discipline for communicat
