here's dan noyes. >> it's going on three years since he died. i can now report he sought help from two d back in december during the last trial he would ever work. >> chief attorney in the san francisco public defender's office matt gonzalez is in charge after jeff adachi passed away on friday. he sought treatment for persistent cough in his last trial. he was referred to a specialist who ran a battery of tests. we don't know the specialist or the test results tonight. paramedics found him unresponsive in this apartment near be hoyt tower. 5:41 p.m. friday paramedics were called. paramedics arrived and started cpr and transported adachi to california pacific medical center where he was pronounced dead at 6:54 p.m. >> they left katerina in the apartment in what police consider a crime scene. police sources tell me officers are upset they were dispatched three hours after the 911 call. by then, katerina was gone. she has not been questioned by police about what happened that night. i reached a realtor for sotheby's, a key witness in the investigat

. >> dan noyes is here with that. dan? >> ama and dan, this bridge opened in 1956. it has had problems with chunks of concrete dropping from the upper and lower decks. but today's trouble may have been caused by very heavy trucks. a high level source who works with engineers on the bridge project tells me they believe they've identified the cause. it's an expansion joint on the upper deck at the eastern end, southern edge just above where the concrete chunks landed on the lower deck. the source says heavy trucks have been hitting that expansion joint hard, opening spaces between the concrete and rebar and allowing water intrusion. i spoke with the former president of the american society of civil engineers. >> that's one of the areas where you may have some problems, expansion joints take a lot of pounding from trucks. and they're also separation between two decks. so sometimes they're not supported as well as they can to take that pounding. and sometimes they do break up a bit. >> andy herman tells me the federal highway administration gives the richmond-san rafael

. >> the i-team reporter dan noyes is here with more. consumer advoeblths and lawyers for wildfire victims say pg&e was more interested in their bottom line than making safety improvements. in this july 2013 letter pg&e tells the public utilities commission it intends to replace six consecutive lattice steel towers with new towers on the caribou palermo 115 kilovolt power line that were damaged by a winter storm. the "wall street journal" reports those improvements were delayed year after year and never done. that is the same transmission line being investigated as the cause of november's camp fire in butte county that killed 86 people and destroyed 14,000 home. mark toney is a consumer advocate with the utility reform network. >> they promised to fix it in 2013. they promised to fix it in 2014. in 2015, in 2016. it is not fixed today. and that is the line that failed before the fire. >> reporter: on november 8th last year a pg&e worker reported seeing a fire started under the caribou palermo line near paradise. cal fire told me today the

dan noyes' exclusive reports this weekend had some of the most read and watched stories we've put on our website. dan will have more on this story tonight at 6:00. >>> oakland police have arrested at least two people in connection with an armed robbery and shooting of a television news crew security guard. the incident happened on 81 avenue near international boulevard at 5:00 sunday. it involved a crew from kpix television. according to oakland police, the guard and photographer were shot in the leg by one of those suspect, and police say the guard was able to return fire. officers recovered a stolen tv camera hours later after they chased the suspect's car. that vehicle crashed near 55th and bancroft avenue, and the driver was detained. >>> the morning drive came to an unexpected stop for drivers using the caldecott tunnel. did you get stuck in this? a car caught fire just after 11 thank morning and blocked boast eastbound bores of the tunnel. sky 7 shows you emergency responders stopping traffic. smoke could be seen streaming from the bores. both reopened an hour after the car fire

. >> let's bring in dan noyes who takes your own dogs? >> i have experience at that place for about 20 years of taking the dogs and the kids there, and it is a great place to take the animals and your dog there, and the path along the cliffs and you can go up along the cliffs and work your way down to the beach. i have walked on that same beach with my dogs and the kids along the years, and i have to say that you can sometimes see some slides, and you know, some smaller slides of course, and so at least in my mind, i am cautious about being closer to the cliffs coming off like that, and it is a wonderful place to take dogs and people out there, and it is a shame to see what is happening today. >> and who knows how familiar those women were with the area, and like you said, you have seen things coming down, and if you not familiar with this beach as not know to be worried about it. >> i know that you have talked to spencer about this, and the surfer in the area, and it is good news that the tide is going down, and the low tide at 7:00 toda
