gottlieb, i'm sorry you've seen situations in india at the moment.s that should worry us here versus population dynamics there or low vaccination rates? >> yeah, it's unclear right now. b.1.1.7 definitely got into india and that's accounting for some of the spread that we're seeing, rapid spread also they have a new varnlt, this 167 variant that has two mutations in it that concern us. as a 44k mutation that's found in the brads'llian variant and in the south african variant it also has a mutation that's found in the los angeles strain as well. it has two different mutations in it that concern us. that's probably accounting for some of the spread we don't have a good sense of how much of the infections are a result of 167 and b.1.1.7 but it's a lot of the infection as that we're seeing. the seral positivity in india is pretty high. there was a study out in december showing that probably about 40% of people in the metropolitan areas of that country had already been infected, so it must be much higher right now so they may be starting to reach a point

scott gottlieb. welcome back, dr.o on the question of younger adults, good news that pfizer is going after the teens now. when it comes to younger children, which is being tested, are there different safety concerns and different dosages being tested >> typically the fda would look at 12 to 15, which is the age group that pfizer or the company i'm on the board of filed for today as little adults and so they do look at children differently, of course but typically the kinds of considerations are similar when you get below 12, you start to think about reformulating a product like this, a vaccine, because below 12 you probably see more of an immune reaction to the vaccine itself so you might test a lower dose. that's what the companies are doing. pfizer is doing that, moderna is as well where they're doing dose de-escalation studies. basically testing lordosis in younger kids to see what's the lowest dose you need to elicit an immune response 12 and above you could use the same dose typically. this is true of a lot of va

scott gottlieb, also on the board of pfizer. we have to start with johnson & johnson being paused. look at the numbers. you have a better chance of being hit by lightning than developing blood clots from this vaccine. how concern are you this pause could erode public confidence in vaccines in general? >> look, i think if the process isn't handled well, it could erode public confidence. what you need is a clear path how to bring this vaccine back. i think it will come back with more language and label about who it should be used for and what doctors should be looking for for these remote side effects. what the fda needs now is a very clear process for how they're going to do it. the pause wasn't a regulatory action but recommendation from the fda to the states administering the vaccine to suspend use of the vaccine. so they never took a formal regulatory action. one option is take a formal regulatory action, but the vaccine on hold a short period of time while they review the data and bring it before the fda advisory committee. that would give them a clear end point when it could co

scott gottlieb. he's a member of the pfizer board and why don't we start right there, dr. gottlieb. the world progress on the vaccine and how much does that play into us being able to put covid behind us in the united states of america? >> there's no question that it's a challenge getting vaccine into low and middle-income countries, in particular. i think as we get into the later half of this year, there's going to be much more supply available to put into different markets. i think the problem and the challenges are still going to be the logistics. some of these vaccines need very complex culturing in storage and delivering them, especially in low-income countries that don't have the health care infrastructure can be a challenge. covax is working on that, the u.s. government, the biden administration has given a lot of money to support that. the companies that donated a vaccine to covax, including a company i'm on the board of, pfizer. so some of it's being addressed. something on the order of what we did under pepfar, massive resource to making vaccine is available. in terms of w
