as with most chronic illnesses, even though we have come so far with addictions and in mental illness we still are not at the same level of knowledge and expertise that we are with learning about diabetes and cancer and heart disease and even alzheimer's disease. so we have come a long way, but we have a lot longer to go to be able to get this to a level where all physicians, like dr. greene, who understands the behavioral health issues and understands addictions. so it becomes just one of the many physical health issues that we look at. and within the context of samhsa and the strategic initiatives, where is that going to take us to further the science? well, samhsa has eight strategic initiatives. we are focusing on everything from prevention right straight through to several different populations. we are looking at trauma and justice issues. we are looking at the military, because the military right now and the military families are really using the services that we have and are showing signs of both mental illness issues around depression and anxiety and suicide as well as addicti

the real scenarios that we run into is when our patients are ill and their medical illness is causing erratic behavior, confusion, delirium, agitation, things that comprise our ability to take care of them at a time when they do not have a decision-making capacity for the short term. examples may be infections or brain injury, that type of thing, where patients really need care. their behavior precludes them from getting the care. not only that, they can be violent and combative, not their fault, but the effect of the illness, and they can harm staff and harm other patients. in that situation there is not a legal hold and a legal avenue for a law enforcement officer to intervene. it's not a legal issue. this is a health care issue. if we have our own security, which frankly, most hospitals have. if you look around the city, they don't have mrpolice office. they have security who work as part of the health care team and abide by hospital policies and uphold our goal to maintain safety for our patients and for our staff and insure that we can treat patients who temporarily do not have c

20 years working in the tenderloin with poor and homeless people, primarily people who are mentally ill. the services before you are emergency services. what is interesting to me coming here today about the testimony i have heard is that emergency services are over represented. we have the neighborhood emergency response teams in their extremely stylish hard hats. we have the institutional police. the homeless outreach team and hospitality team are both equally emergence services. specifically, we work with people on the street who have no other option. typically, we look for people who have no other option, people locked out of the shelter system either because of high level metal health, substance abuse, or physical injury, or people who for whatever reason cannot it anywhere else. both of these agencies do the work of rescue, and if you cut the funds, no one will be able to pick up the slack. we do not have other support in the system. there is no other group during direct general street outreach in the community to help people get into shelter, and to care, into housing, and into ca
