this is firefighter and paramedic jason cortez. he was 42 years old. he had this accident around 10:00 this morning. and then he died an hour later here at the hospital. cortez started with the san francisco fire department in 2007 and was recently assigned to station number 3 in the tenderloin which sffd says is the busiest station in the country. you're looking at some more video of the incredible moving procession. i stood out here for hours. the folks who were here, the firefighters and law enforcement officers must have stood here for four hours waiting at the hospital before cortez's body came out and the procession began. there was an incredibly moving moment where a ladder truck drove down the street away from the hospital full of firefighters sitting on top of the ladder. and as you've been describing, they've been moving through the streets of san francisco to the medical examiner's office. of course cortez was a husband and father of two small young children and that's just been devastating for everyone here at the hospital to think about. th

its so-called green sheet, a preliminary investigation into the events that led to the death of jason cortez in a training exercise last wednesday. the green sheet is based on eyewitness accounts, from firefighters on scene, and expert analysis. the report concludes cortez was knocked off a fire escape when water shot out of a valve that he apparently opened to during the drill. the water knocked him backwards off the fire escape to the pavement below. the report says coronavirus safety protocols may have played a role in the incident. because multi company drills are suspended, and his engine company was conducting the pump operation alone, instead with another company. the green sheet will now be included as an internal report to learn from this incident and improve safety. as well as serving as an investigative tool for the official police and cal osha investigations. san francisco fire department lieutenant, jonathan, said they are able to utilize what our observations are, but it doesn't leave their investigation. it is another element that they are able to utilize. >>> the fire departm

i was very shocked and saddened by the loss of jason cortez.his tragedy brings to mind how important each of our members of the fire department are to each other as well as to the department at large, so i just wanted to personally express my condolences to his family and to the personnel involved in this horrific accident and wish them a speedy recovery from this tragedy as best i can. i had a quick question, a couple of quick questions for our fire marshal. you mentioned a junior fire marshal program, and i just wanted to get a little more detail on just what that meant and who's involved. and the arson report that commissioner rodriguez brought up, i, too, have brought this up in the past, and once they're available, that they can be presented to the fire commission so we can keep track of human arson incidents are happening -- track of how many arson incidents are happening. we never know what's happening with these fires in terms of the final result, so i'd like these shared with the fire commission on a regular basis. and my last question
