an update on the laguna honda gift fund and the true north metrics update. we also received a presentation on the new formatting of the regulatory affairs report and the facility assessment report. we had a discussion of the cms based three regulatory requirements. and at that time the committee discussed how these changes in the requirements will impact the quality work and the hospital overall star ratings. they're based on this requirement and there is change in the ranking, so it has some impact, so we want to follow up on that. the presentation on the changes to the star ratings is on your desk, commissioners. the committee also recommended that the full health commission approve the laguna honda medical staff by-laws and rules and regulations which were presented and approved by the j.c.c. and also approved the hospital-wide policies and procedures. in closed session, the committee approved the credential report. thank you, that's the entered of my report. >> thank you. is there comments or questions from the commission? great. commissioners, i didn't

you've seen the increased expense for laguna honda there.my last slide is maximum allowed overtime. the admin code says nobody can work more than 25% of their regular hours as overtime unless the department requests an exemption from department of human resources. the table shows there were a total of 17 depends who had add least one employee who exceeded the 25% limit. only four requests exemptions from dhr and those are emergency management, juvenile police and airport. >> commissioner: thank you very much. any questions or comments or questions for departments. seeing none, i have some questions. some of the overtime in our depends are mandatory? there's always a certain amount of required overtime? i believe so. >> commissioner: would you say there's a certain amount of mandatory overtime that's required? >> i'd have to defer to the department whether it's mandatory. there's some level of overtime which they generally expect because they use that in their budgeting practices. >> commissioner: when you say it's less expensive to use overt

with spokes at laguna, zuckerberg and san francisco general and ambulatory. we plan to provide four weeks of 24/7 support. i hope that answered your questions you had, commissioner green. >> i'm the chief information officer for the health department. i'll give you a very high level update on how we're organizing the project from the perspective of a business project not an i.t. project. these are the domain groups we have stood up over the past year. they're essentially haired by some of the most senior leaders in our organization including senior directors. spa they're supported by a project manager either an internal d.p.h. manager or external consultant. these bodies of the groups that includes frontline staff managers, champions that have participated and informed the adoption decisions and build decisions that we proceeded on and they're looking at how to get the organization ready and staff and training and super users are identified and ready to support the work in the next three months. this was the simplest way to organize a complex slide. i was try

it took me an awful long time to figure out, for example, in laguna honda, that project became zilch, if you will, so the funding for that project was redirected to other more pressing projects that could use the money. is there some way that you could actually list the name of the projects, the dollar signs, so that for example if you had another project that's kind of live, if you will, or a candidate, the next time we meet, that came -- that fell by the wayside since the last time we heard about the projects, i think that would give us a sense of the number of projects held in abeyance or abandoned so that we can follow some trail. i don't have a good way to do that, but it took me hours to read the report to figure out which ones and how many dollars. and at the end of the day, it is -- i got it because i put the pieces of the puzzle, but i -- you know, i'm just expressing frustration, and i know it's very difficult in the type of projects in this bond, they're not so neat, if you will, and you don't find things until you go there. but in the meantime, somebody else and the team h

at laguna honda hospital the patient census has been fairly stable over the last few years. what we're seeing over the last couple of years was an increased need for one-on-one coaches for some of our patients. these patients were presenting with issues that required beyond the regular staffing safety -- required additional safety needs to increase staffing to have one-on-one coaching to observe them. some of the things we were seeing was intrusive behavior, aggressive behavior, risk for falls, the need for additional support because perhaps they can't feed or toilet themselves. for clinic reasons it was determined it was best to have a one-on-one coach and that is beyond our staffing ratios. to ensure we are meeting the patient's safety and quality they enoccurred overtime to -- incurred overtime to ensure we're meeting their needs. in fiscal year '18-'19 we proposed an initiative to staff more accordingly. it's unpredictable we don't know who will be coming in to laguna honda but that's the strategy we've used today. were being accused sexually and physically by providers at laguna honda. there was a lawsuit that created the community living fund to help people live in the community and not be segregated. disabled people in san francisco, like myself, are segregated. the community living fund failed to help me in so many ways. it really was a horrible experience especially with my social worker, michael boyer and page usted, his supervisor. they told me i couldn't change social workers. in the time i was there, i was not able to get access to home delivered food. their phones did not work for months. my social worker would not follow up on things for weeks at a time. so it became so bad they wouldn't help me get any sort of batteries for my wheelchair or mobility equipment i needed for my power wheelchair. i was not able to get a ramp to access my home. i was not able to get in-home support services or a phone for accessibility needs. this program is really flawed. i would like oversight for it. fortunately, mod stepped in and helped me get the food i needed. they help
