the workers christopher shepherd and sheppard and lawrence daniels were killed at the walnut creek bart station in october of 2013. it happened during a bart strike so a trainee was operating that bart train when it hit the two workers, cal/osha gave bart three willful serious violations today. the first is that the workers were not qualified to do the work they were doing that day because they were not electrical workers and they were working near the third rail. the second violation is the trainee at the controls was operating the train by himself. cal/osha says the trainer was not with him. he was in a passenger car at the time of the accident. cal/osha also says that the trainee did not have adequate training. the last violation is that the procedure the workers were using known as "simple approval" in cal/osha's mind they believe "simple approval" is not adequate relying on a spotter to let others know a train is coming. the trainee didn't know how to sound the horn as the train was about to strike the workers. >> the operator in training saw the people attempted to stop the train,
