lisa amin gulezian joins us live. lisa? >> carolyn, investigators believe the problems may have started 15 years ago on an assembly line where the rivets were created incorrectly. abc news learned investigators are focusing their attention on rivets. the tiny pins that hold pieces of the plane together. sources say on that southwest 737 that ripped open, the rivets holding parts of the plane's fuselage together failed because they didn't fit perfectly. the rivet holes themselves may have been sized incorrectly back on the assembly line. >> they were, over time with the flexing of and expansion of compressing the airplane's few sill lodge, those holes would microscopically expand or change. >> and in this plane's case, the seem tore creating a gaping five-foot hole. oxygen masks dropped and the pilot dove to a lower altitude. >> the masks came down and it was frightening. >> as a result, boeing ordered sections of similar 737's worldwide. nearly 600 total. a third required inspections immediately. and of those, five have sli

lisa stark is back on the story tonight. she's live at reagan national. lisa? >> reporter: david, this, as you mentioned, this latest incident is in miami. it was at miami center, the controller falling asleep overnight. early this morning. luckily, other controllers on duty. no radio calls missed. it raises alarm bells. this may be the final straw. at this hour, high level negotiations are under way. between the government and the controllers union. they're working out new rules to ban scheduling practices that can cause fatigue. there have now been seven controllers believed to have fallen asleep on the job, on the midnight shift, since the start of the year. including one earlier this week in reno, that missed a call from a plane carrying a seriously ill passenger. controllers in a nearby facility tried to help. >> we're going the call them on the phone lines. >> all right. we'll circle some more. we have a pretty sick patient. we may have to land. >> reporter: the pilot ultimately had to land on his own. controllers bounce from morning shifts to afternoons,

>> thanks, lisa. let's turn to the man in charge of the nation's skies, the secretary of transportation, ray lahood. and mr. secretary, you call this incident outrageous, but it's at least the fourth, maybe the fifth in recent weeks. how could it happen again? >> george, i want people to know that we are outraged. this is just absolutely ridiculous that a controller could fall asleep in reno when a plane is trying to land there. and we have now five investigations going on. we'll learn a lot more about it. we've suspended these controllers and we may have to take more permanent action once these investigations are complete. >> you know, we checked in with our aviation expert, john nance, and he said that these controllers are being set up to fail. and it's time to grab the agency by the ears and make changes. are you prepared to make more changes? >> absolutely. i'll be grabbing the agency by the ears. we've done some grabbing today with the outrage that we've expressed and the suspension of this cont

lisa stark has been on this story from the beginning and has exclusive new details tonight. lisa? >> reporter: good evening, diane. well, abc news has learned that the problem that ripped a hole in that southwest boeing 737 may have started way back on the assembly line more than a decade ago. the question is, are any of the other nearly 600 planes like it at risk? it was a harrowing moment at 34,000 feet when the skin of the southwest jet opened up. a five-foot hole. passengers could see daylight as oxygen masks dropped and the pilot began to dive to a lower attitude. >> mask came down and it was pretty frightening. >> reporter: investigators found widespread cracking in the metal. the initial thought, metal fatigue, caused by the plane's 39,000 takeoffs and landings. but abc news has learned that, in fact, part of the problem may have actually been a defect when the boeing plane was manufactured in 1996. investigators are focused on rivets, thousands of metal pins that hold the pieces of an airplane together. the concern is that, in the area that failed, those pieces were not he

lisa stark joins us with more on this. hey, lisa. >> reporter: hey, good evening, george. the recall involves the model years 2004 through 2006. and the problem is the driver's side air bag. now, here's what's happening in some cases. the driver is getting into the vehicle. they're simply turning on the car. and then the air bag is suddenly popping out unexpectedly. ford says the issue is a wire right inside the steering column that can chafe and cause that air bag to deploy. there have been more than 300 complaints, 122 injuries. everything from burns and sprains, eye injuries, loss of consciousness. ford has been resisting this massive recall. it has argued that the air bag warning light comes on very, very early. weeks before the air bag would actually deploy. giving consumers plenty of notice. but the government has insisted on this recall. ford has now finally agreed. next month, more than 1 million truck owners will be getting those recall notices. george. >> boy, this is a huge recall. lisa, thanks very much. >>> still ahead on "world news" -- new questions today ab

lisa, good morning to you. >> reporter: good morning, dan. well, even as we speak, investigators with the national transportation safety board are right now leaving. they are heading to arizona to try to figure out what happened, what went wrong to cause this gaping hole to rip open in this southwest jet. as you mentioned, it happened at 36,000 feet. the pilot was forced to put the plane into a dive. it was a terrifying experience for passengers to say the least. when the passengers finally made it to their destination, an explosion of joy. >> just glad to be here. >> reporter: southwest flight 812 started routinely leaving phoenix at 3:45 p.m. local time bound for sacramento. 118 passengers on the boeing 737, but at 36,000 feet, loud pops, then the roof peeled back. >> well, when we could see daylight out of the plane, i go -- this is -- you know, you always think of people getting sucked out of airplanes when there was a hole in it. i was really -- it was really scary. >> reporter: oxygen masks dropped as the plane depressurized. >> the top
