chair of the ntsb which investigates plane extaccidents this is what she told the committee. >> what is happening from the staffing shortage is that air traffic controllers are being required to do mandatory overtime and what happens with mandatory overtime, it ends up leading to fatigue and distraction which is exactly what we're seeing as part of the incident investigations. >> do you agree with her, and also just to draw from your time, because you've been a part of these investigations of accidents, when human error is involved in a plane collision, how often does it come down to pilot fatigue? >> pilot fatigue,er a trir traf fatigue, equipment fatigue. we know that the office of inspector general and of course the ntsb have studied it and they say 60% of the time a near miss or if we have them collisions is caused by pilot error, often tired pilots. 20% is air traffic control. and 20% something else on the runway, equipment, confusion, runway gate confusion, et cetera. so about 60% for pilots, 40% for other reasons according to studies. >> yes, thank you for answering that quest

first up, this is the ntsb chair on the near misses so far this year. ntsb has opened investigations in to seven runway incursions this year alone. in over half the aircraft got within seven hundred feet of each other. these incidents must serve as a wake-up call before something more catastrophic occurs. >> dennis, is anybody waking up to this? >> well, we weren't asleep. these are near hits. the chairman has it right. she's been more forceful in her words recently. we have an aviation safety crisis going on. this study just affirms that the air traffic control system, particularly with staffing, is just not where it should be. the infrastructure has failed. now we have all of these airplanes flying this thanksgiving holiday and we know it's under pressure. the most concerning thing is with the air traffic controllers, they have my back, i've got theirs. when they're working overtime forced, 60 hour work weeks, they get fatigued. when you get fatigued, you make errors. the system is wired to trap that when it happens. if it's a constant threat, it's a
