the group endorsed the healthy aging model that's provided by the community services which provides a model that could be applied to various groups. provide for admittance to shelters where the clients go through a comprehensive needs assessment process. obviously, the availability of increased long-term, ongoing rent subsidies for seniors and persons with disabilities. as i mentioned previously, increased training for shelter and center staff, provide orientation sessions for shelter clients about case management, how to report. look at why seniors are moving out of s.r.o.s and utilizing shelters over navigation centers. the h.s.h. adult coordinated entry personnel resources hub. obviously greater access to assisted living facilities, skilled nursing and hospice. and universal case managers that could act as a team and work across the departments would be helpful. develop programs such as alcoholics anonymous and narcotics anonymous and other programs to address some of the behavioral needs. and that provides exclusive transportation because that often times becomes another problem.
