reiner.arlier this week, a louisiana doctor told cnn that every person who is infected with the original virus would typically infect 1 to 2 people. with the delta variant, he says 6 to 8 people. dr. reiner, if that's the case, where do you see us two weeks from now? do you think that may be the case with the delta variant, that is infecting more people than we saw before? >> yeah, it clearly is. we know a couple things about this variant. we know that it replicates very quickly. and the net effect of that is people who are infected with the delta variant have more virus on board. and we also know that the virus itself is more sticky. so you probably need less of it to become infected in the first case. so if you have a lot of virus on board, somebody who is spreading it and you don't need a lot of it to actually get infected, that's a prescription for an increase in transmissibility and that's what we're seeing around the united states. what we're going to see throughout the south and midwes

jonathan reiner from george washington university. good to have you on.of comparison, our sanjay gupta said that even with the numbers you're still more likely to get this syndrome from contracting the flu regularry and i wonder what your view is. is this a significant concern or not significant? >> well, first of all, what appears is that we have a vaccine safety monitoring system that works. and i think the take home message for the american public is that the fda just didn't issue eua's for vaccines. they are actively monitoring their efficacy and side effects. we do see gillan barre from vaccines. we see it from the flu vaccine probably 300 times a year. so the incidents with the j&j vaccine in the 13 million doses given in the united states might be a bit higher than what we've seen with the flu. i don't think it is alarming. it is still fairly unlikely. every year in the u.s., every week in the u.s., we see about 100 cases of gillan barre, including all kinds of -- and it has been reported with just covid, as you said, with just the covid infection

reiner, is that real? real life we could be facing these kinds of numbers again?the number of cases will continue to rise. as you said we're up about 51,000 average cases per day. a lot of that is asymmetric in the united states, focused in florida, texas and california. but it's going to rise everywhere. it's much slower and to lower levels in place like the northeast, which have very high levels of vaccination. as for deaths, we'll have to see. the excellent thing in the united states is that we have vaccinated almost 90% of people over the age of 65. the most vulnerable. about 80% of the deaths came in the first three surges in this country. i do not expect to see deaths approach anything what we were seeing in january when we were seeing sometimes 2,000 to 3,000 deaths per day. we will not approach that. >> if there's any glimmer of hope, that some good news. we were discussing kay ivey, being this voice for republican frustration. there have been others. you saw steve scalise this week getting vaccinated. that's good. the florida governor ron desantis telling

jonathan reiner. doctor, good evening to you.tration health officials are thinking people who are 65 or older who or who have compromised immune systems will need a third shot from pfizer or moderna. this is a shift from a few weeks ago. what do you think about this? what is going on? >> there is data coming from places like israel that suggest after a period of six to eight months, the efficacy of the mrna vaccines, particularly the pfizer vaccine may start to wayne and that's unclear if it maybe lower levels of neutralizing antibodies or whether it's just that we have the prevalence of these more transmissible aggressive strains like delta. we always expected at some point we needed to boost and i expect we'll start doing that with the folks that are most vulnerable who are holder or immune compromised in someway. probably, eventually, everybody in this country will need a booster in the next several months. that doesn't surprise me. >> let me give the numbers from what you said because you said this follows what we're learning

reiner says no mask if you're fully vaccinated. the cdc says the same thing. vaccinated people wear masks indoors? >> yeah, thank you so much, pamela, for having me. and obviously we agree both with cdc and lots of the medical experts across the country that are noting how much protection this vaccine gives fully vaccinated people. i do think you have to take into account local situations. and i want to point out that both the federal government and many state governments have actually asked fully vaccinated people to keep their masks on in other settings. and we all comply with that. we all wear our masks whenever we're on any public transit. we're wear our masks, regardless of vaccination status, when we're in health care facilities. we wear our masks when we're at schools in california. there are lots of settings where even know we know the vaccines provide powerful protection to those who are vaccinated, the slight risk that a vaccinated person could shed enough virus to infect somebody else, coupled with just creating less and less risk in those settings

jonathan reiner. you saw dr. fauci on tiktok this week.oblem this is. what's your reaction to young people remaining hesitant to get vaccinated after all of the information that's been provided by infectious disease experts like yourself? >> yeah, i think we need to be a lot better at messaging. i'd like to see all kinds of different ways of getting to people. i'd like to see nascar get involved. i'd like to see nashville and country music get involved. i'd like to see peer-to-peer discussions around the country. the truth is there have been about 2,600 folks 18 to 29 who have died in the united states from this virus. that's just about the same number of people who died in the world trade center towers on 9/11. so this virus can kill you even if you're young and pretty fit. as we saw in the piece leading into this, many people will get long haul symptoms that are really tough to beat. so there are all kinds of reasons to do this for yourself. the other really important community-based reason to get vaccinated is to prevent you from being

jonathan reiner, a cardiologist and professor at gw medical center. let's zero in on this.dc found this virus circulating in about 3% of cases. now less than two months later it's circulating in 83% of cases. is it most of the unvaccinated allowing this variant to spread so much and so fast in this country? >> what we really have is an asymmetric spread of the virus. and although every state is seeing an increase in cases, we're seeing it disproportionately in places where a number of folks who are unvaccinated is high. in fact, if you look at right now we're averaging about 37,000 cases per day in the united states. that's a 200% increase over the last 14 days, but a third of those cases is coming out of just four states, florida, louisiana, missouri, and arkansas. other parts of the united states where vaccination rates are high has seen very modest increases in cases. so it really has become a tale of two countries. >> when you look at the numbers you look at those states. what is your visceral reaction as a doctor? >> it's so unnecessary. and the word is maddening. i've
