when i tell people i work at san francisco general, people are really -- they express real surprise, that's a great facility, i've heard so much about it. and it's true. i really feel strongly that the city of san francisco has supported san francisco general and laguna honda and people who have been on the friend of the society now get very good health care. but it's becoming harder and harder every day. med surge is a fast unit. we admit and discharge patients. many times these are patients that are discharged that have minimum family support. they need to go home. they have dressings to do. they need medications. they can't do it on their own. this has become a trend where people are being discharged early and they're not ready. we don't have support. we do have nurses that come to the homes to help but that area is now being cut. so, the support for some of these people who need additional help is not there. it's dwindling and it's quiling very fast. i think the san francisco residents have shown over the years by supporting the bill and the money to staff these places is there. s

i worked at san francisco general approximately 15 years and now i work in the community. and i really wanted to address some issues pertaining to the changes structurally and otherwise at san francisco general and how it's impacting those of us in the community, meaning that the department of psychiatry has been cutting back on beds and shortening the length of stay. and, so, releasing people who are not doing very well into the community. and i think it's nothing short of a miracle that nothing more significant has happened thus far. i can give an example of a young man i've been working with over 10 years now, both on the acute side as well as the outpatient side who had tried to get hospitalized and treated for the last 2-1/2 years. well, he he finally managed to get successfully hospitalized. he's now in the jail system being treated by jail psychiatry. and that's the way it's been going more so and now he's looking to go to wi health court who is going to mandate him into treatment. that's after he assaulted somebody in the mission right in the parking lot of the miss

i practice at san francisco general hospital. i'm a professor of medicine and epidemiology at ucsf. i've been in san francisco for a long time. i've done all of my postgraduate training here, raised my family here. this is an issue that i want to give you the context of what motivates me about this issue, and that is really my experience in clinic taking care of patients with type 2 diabetes and thinking about the challenges ahead. so, type 2 diabetes, when i was in school, we were taught what's called adult onset diabetes and that distinguishes from juvenile onset diabetes. and the reality is that type 2 diabetes, adult onset diabetes as well as diseases of children and disease of adolescents and we've really shifted that hep dick i can ~ epidemic to one that happens when you age to becoming more common among teenagers and children. so, this is the statistic that's quite compelling to me. so, one in four adolescents have pre-diabetes or going to be predisposed to becoming diabetic when they become adults. 10 years ago that number was one in 11. so, i'm not somebody who is usually p
