sanjay gupta. sanjay, great to see you.ew year. >> happy new year. >> everybody's so hopeful that we'll be able to turn a page, but of course coronavirus has followed us into this new year. so how different do you think 2021 will be? >> i think it's going to look a lot different. i guess it's hard not to. 2020 was such a unique and tragic year, you know? i mean, we've all lived through this, physically, psychologically. these vaccines, i think, are -- they're here. they're going to be more widely distributed. and it is incredibly hopeful, not just because of the vaccines, but i think also because of what they represent. i mean, you know, we are able to respond so quickly to this pandemic by creating something that typically takes years, if not decades to make, a vaccine. and they're going to make a significant impact, but it takes a while, still. so by this time next year, i think we're going to be in a very different position. the vaccines, the testing, things like that that will happen over these next few months here in 2

sanjay gupta joins me now. so grim looking at these numbers. talk about the hope ahead of us. that, of course, is the vaccine that people are relying on, hoping this will get us out of the pandemic. cdc has issued new guidance, claiming a person could take one dose of the pfizer vaccine and another of the moderna vaccine in exceptional situations and that those could be up to six weeks between if necessary. do you think they're loosening the guidelines because production has fallen so far behind? >> i think is probably driving this here. uneven distribution, the administration obviously, these vaccines were not where we should be. and this question comes up a lot. it's one of the most common questions i get, pamela. just got the first dose. what if i can't get the second dose of that same vaccine? people are quiet concerned about it. in the uk, they loosened these guidelines quite some time ago. hopefully, people don't need to do that. scientifically, it seems to make sense. it should still work. these are very similar vaccines and 42 days or s

sanjay gupta. sanjay, you were listening as the president said by next week they'll increase the amount of vaccine shipped by 16%, that this summer -- by this summer, they want to purchase 200 million more doses of vaccine. what's important here when you look at this and see what the administration is doing? what's your most important takeaway? >> i think the most important thing is we, obviously have to have enough vaccine, but the pace at which these vaccines actually get to administered, meaning shots going into arms, i think is really critically important. i guess we all know that. but it's become even more important as we discussed this in the context of these variants. it's important to remember that when the virus spreads, the more it spreads, the more mutations it continues to accumulate. many of these mutations will not be that consequential but you'll get mutations that aggregate and could potentially start to escape -- develop what's called escape from the vaccines. and you want to do it

sanjay gupta. sanjay, how would half doses work?they come in viles of ten doses, and doses are each 100 micro grams. this is the trial dose they set it on. they would basically give half the volume to each person. instead of 100 micro grams, they would get 50 for both doses, first and second dose. it sounds simple, it is more logistically complicated. you may need different syringes and needles to make this work. but this is precisely what the fda will be talking about. moncef told me in terms of a possible strategy to try to address getting more vaccine out there. >> do we know if moderna tested using half doses? will it still be as effective? >> what we don't know, even the data moncef was sharing is not really published data. the way the trial should work is basically in phase three, you get a certain dose, you see does it prevent covid illness or not. they don't have that sort of data on the 50 micro grams. what they do have, jake, this is interesting, they found in phase two trials people under the age of 55 when they got that

sanjay gupta. sanjay, great to see you. i think we can all agree, more vaccine is better than less vaccine. but in terms of what president biden announced yesterday, that his administration is buying 200 million more doses of these first two vaccines that are available, just tell me, is that wise, given that we hear that j&j, the single-dose vaccine, may be right around the corner, which would be easier to administer for everybody? >> right. i mean, you know, we don't know for certain if that's going to be what the data is going to show on johnson & johnson, whether it's going to be authorized. but as you point out, it does look pretty promising. and as we start to do the math there, we can show all the various doses from all the various companies that the united states government has now purchased. but it is well more than what would be needed to vaccinate everybody and certainly to get to that point of herd immunity. 300 million doses of pfizer, 300 million of moderna. remember, they also have 300 million of astrazeneca.

sanjay gupta. what went through your mind as both a doctor and a journalist as you watched the first official biden administration covid-19 briefing earlier today? you've been calling for more transparency. are you satisfied with what you saw and heard today? >> yeah, wolf. i mean, i think there was a lot of information that was given. this is the sort of briefing that i think we have been hoping for, for some time. very specific details. what are the adverse event rates with the vaccines? we talked about those allergic reactions. dr. walensky said somewhere between two and six in a million. we got a better idea on that. how effective are these vaccines against the variants? big question. dr. fauci addressed that today, and the news was encouraging. where are some of these vaccines? we hear about the numbers that have been distributed but only half have been administered. why is that? andy slavitt sort of addressed that. there's still going to be questions. obviously there's still a lot of work to

sanjay gupta. do i have it right? >> yeah, it's a more transmissible strain.doesn't appear to be more deadly. but if it's more transmissible, it can more easily get into vulnerable populations, that's a problem. it's not surprising it would become the more dominant strain. it probably doubles its proportion every week. so, if it's 5% this week, it'll be 10% next week, 20% the week after that. that's when it becomes the more dominant strain. big question, chris, do the antibodies either generated by the vaccine or in people who previously had the infection, will those antibodies work against this strain? don't know for sure, but pfizer thinks they will. that's going to be tested now. these viruses will probably continue to mutate, and it is probably that the vaccine will be something that either needs to be given every year, every couple of years or a booster shot or something because of that, chris. >> what do you make of this not enough vaccine situation? is this just about the reality of distribution, or is this poor planning? and how do we fix it? >> well,

sanjay gupta. a rare treat to have you here in studio, sanjay. thank you so much.h officials say the two people who tested positive for the new south african variant that worries fauci and others have no known travel history, no connection to each other. that seems really troubling. >> yeah, i mean, you know, there's so far the contact tracing hasn't revealed anybody else that's actually had contact with these people or people that may have actually spread it to them, but almost assuredly, without the travel connection, history, it's out there. to give you context, i was looking into thiss, the uk variant, first time it was discovered will be one month tomorrow. first case. now it's in 30 states here in the country. even now if you go back and say, okay, before december 29th when it was first found they're now finding new cases even earlier than that. so it's here. these variants are here. that's not really a surprise given how contagious and how much travel there still is. >> so we know that the uk variant was more easily transmissible. it was more -- it wasn't wo

sanjay gupta for some analysis. sanjay, what kind of difference could it have made if we'd been getting these kinds of regular medical science-based briefings from the white house throughout this entire past year, the entirety of the pandemic? >> i think it would have made a big difference, wolf. i mean, clearly just having a consistent message, taking this seriously, i mean, as we've talked about so many times there's obviously countries around the world that have not suffered nearly as much as we have in terms of people becoming infected and sadly dying. i also think, wolf, we just spend so much of our time as journalists after those briefings previously fact checking. i mean, it was kind of stutter stepping our way through the reporting because there was so much that we had to go back and just check the facts and correct misconceptions that came out of those briefings. this was factual. we learned a lot about the variants. we learned a lot about how the vaccine rollout is going and what still needs to be done. >>

sanjay gupta who is a neurosurgeon gets his own vaccine today.t-elect biden plans to release every vaccine instead of holding back half ensuring that everybody gets a second half of the vaccine and he is out with his book "building a better brain at any age, keep sharp." do you think it is the right idea? >> well, you do see it would be a demonstrable impact of getting people vaccinated, but the big questions are can the manufacturers keep up to giving the second dose to people on time. i spoke to the ceo of pfizer and they anticipate that they would be be able to manufacturer 1.3 billion doses by the end of 2021 and most of the doses are going to other countries around the world, and 200 million total are coming to the united states, but what is the manufacturing capacity of these other companies, and will johnson and johnson and oxford astrazenica come on line and if they do, it is a good move, but it is a little bit of a gamble, because if you are starting to push people's second doses back too far, we don't have the data in terms the of how t

sanjay gupta. open enrollment is over. it's over, but what the president will do is reopen the exchanges so people can go in and buy health care they need. and the thinking is that people have so much increased need because of the pandemic, what would this do? >> yeah, so that's exactly right. it's tough to know exactly how many people have lost employer-based health care through this pandemic, but it's exactly to address that potential need. according to kaiser family foundation, the number could be 2 million to 3 million. a lot of people have, obviously, lost their jobs. how many of those people had health care insurance through their employer. how many were able to get things like cobra to extend their health care insurance? regardless, there's obviously a need here, and i think that's what this executive action is sort of intended to address. so six weeks typically is what open enrollment is near the end of the year. they say maybe they'll keep this option, these exchanges open for a few months to try and let people bu

sanjay gupta. when do we have a free pass?at point will we, you know, what number can we start to return to normal, sanjay? >> some of that was pretty sobering in terms of how long this is still going to take. >> the answer to that i think sort of gets back at this shall of herd immunity. when are enough people protected, you are protected and enough people around you are also protected? once you get to that 75%, even if it's not 100%, they say you have enough of a herd of protection within the country that the virus really shouldn't cause people to be getting sick or significantly sick anymore. but that could take a while. and, you know, there's all these other things that are sort of, you know, potentially impacting that, including the variants that many people have been talking about a lot. so herd immunity, right now, maybe summer. we were still saying, that is likely the point where we get to that. and that is when many of these types of activities become safer to do. >> we talked about the variant within sanjay, and we a

sanjay gupta. sanjay, just to state it one more time.ans dead in a single day from coronavirus than south korea has suffered the entire pandemic. than japan has suffered the entire pandemic. we did it in one single day. and i've got a feeling this is not the last time we're breaking that record. >> no, i can't believe it. we're coming up almost on the one year anniversary of the first patient that was confirmed to have the infection in this country. that was late january of this year. here we are, breaking all kinds of records. several states breaking their records over the last day. and you're right. i want to show these graphics. these models that we've been following for close to a year now. these come from the imhe. basically looking at what the moderate case scenarios are and the worst-case scenarios. if we have it, we can put it up. basically, what we see when we follow this is a couple of things. one is that we're sort of on the worst-case scenario trajectory at this point. that shouldn't surprise people. you can see at that point

sanjay gupta. sanjay, the top line that sticks out to a lot of people is that the johnson & johnson vaccine is 66% effective in a global trial, which is less than the two other vaccines that have already been approved, but you think the number waeshd be focused on is 85% instead. explain why. >> we can put up these two numbers so people can see what the trial showed. 85 perks this is basically the protection against serious disease. the type of disease ta might land someone in the hospital or have severe symptoms. it's the number of infections, number of hospitalizations, number of deaths. if you can bring down hospitalizations and deaths, that ends up being a key point. i think we'll look at these numbers and also see how these vaccines compared to the variants. you see, obviously, in south africa, it's the lowest efficacy, almost all the patients in that trial had the variant now that they know. so you're getting an idea of what the efficacy is against moderate disease. regardless severe disease,

sanjay gupta are both here. good evening to both of you. sanjay, i want to start with you. breaking news about new cdc guidance that came out on vaccines. what do you know? >> this is a question i get more than just about any other, the two vaccines, the moderna and pfizer one, both mrna, can you interchange them? you get the first dose of moderna vaccine, could you then get the second dose of a pfizer vaccine. the guidance has been no and frankly is still no except in exceptional situations when you have no other options. could you potentially interchange them. you have to wait at least four weeks between the two doses but you could interchange them if you no other options. this is in response to the shaky rollout of the vaccines, but the other thing is you can wait up to six weeks, 42 days into between the two doses as well if you have to. you can interchange and possibly extend the time frame. >> six weeks and you can mix and match but only under certain circumstances. people ask you a lot and i don't want them confused so say it again just so we get it, doctor. >> excep

sanjay gupta. know, we knew january was going to be bleak, we talked about it a lot, but the thing that we held out hope for, the thing that gave us the light at the end of the tunnel was the vaccinations and by now today more than 20 million americans were projected to be vaccinated by the trump administration. we are nowhere near that. we are at 4.8 million americans have been vaccinated, 17 million doses have been distributed. is it time to change our outlook for what this spring and summer are going to look like? sanjay, hold on a second, we're having a hard time with your audio. just one second. >> alisyn, can you hear me? >> now i can. thanks, sanjay. >> i don't know how much this will change the overall timetable for return to normalcy or changing of the overall trajectory, all those things that we're looking for. we'll see. you know, but obviously this has been a disappointing rollout. what has sort of struck me now reporting on this just since the beginning of the year is that we're not hea

sanjay gupta. sanjay, great to see you. the states are ill-equipped to have -- to meet this aspirational goal. they don't have the infrastructure, sometimes they don't have the manpower. so, have you heard how president biden plans to change that? >> well, we're starting to hear some specific plans. we're going to hear more today. yesterday was sort of larger framework, but they talk, first of all, about a lot of money going towards this. you know, there was, as part of this $1.9 trillion package you're hearing about, $400 billion is going specifically to vaccines, ramping up testing, and reopening schools. so a lot of it towards vaccines, talking about things like these large vaccination fairs, community vaccination clinics, things like that that would allow people to easier access vaccines. we need to hear people about actually increasing manpower, just personnel, to be able to administer these vaccines. that seems to have been a rate-limited step. and we also need to understand how the private sector will be a part of th

sanjay gupta. sanjay, give us your reaction to this new plan laid out by president biden. >> well, you know, he's shittin all right nou ntes. the press briefing, by the way, the first covid sort of focused press briefing since november 19th i thought was very factual, very evidence based, very honest. there are some specific things that president biden talked about ahead of that press briefing that are significant. i mean, you know, as phil mattingly just mentioned, ten executive actions. that's significant. specific role for fema in terms of getting these community vaccination centers out there. 100 of them. they say within a month. specific role for cdc in terms of getting retail pharmacies by february 7th or february 8th according to president biden, being able to satisfy vaccination needs in certain communities through your pharmacy. a specific role for hhs in terms of creating more vaccinators because that was a rate-limiting step. you remember, wolf, just not enough people to actually be pushi

sanjay gupta. sanjay, i was very interested in where we are today, right? with hospitalizations going down, with dr. fauci very pleased with. i mean, he's optimistic that maybe we have plateaued. but i wanted to dig into the concerns about these new variants and whether or not that means we could see a new wave in the united states. he didn't rule that out, but i don't think he seems to think it's a giant cause of alarm this mor morning. >> right, that's what i took away from it, as well. i think that these variants are something we certainly have to pay attention to and there's a couple of them out there. one way to think about it, and i was speaking to some people who really study viral dynamics, even last night. these are obviously -- this uk variant, for example, much more transmissible. so what does that mean. there are several things that you have to pay attention to. one is, if something is more transmissible, it can become the dominant strain. we saw that before. the italian strain in the spring of last year was the dominant strain -- or the more tr

sanjay gupta. he clunky nature of the vaccine distribution plan, but to hear from biden administration officials that there's more or less no plan and they have to start from scratch is stunning. >> it really is. and it follows this pattern, you know, where the -- when you don't have a specific national plan and you hand things off to the states, you see these problems happen over and over again. we saw this with testing, going back to early last year. we saw this with personal protective equipment, even with ventilators, when we thought ventilators were going to be a bigger part of addressing this pandemic. now with the vaccine, we spend so much time reporting on the scientific achievement, which was tremendous in terms of actually developing these types of vaccines, the idea that operation warp speed would then hand the vaccine off to the states and say, that's basically hit, we're done with this, that is stunning. and in some ways, you could have anticipated this. you find in some places, supply

sanjay gupta. sanjay, let's start there.cinations into arms in his first 100 days. you just heard mj exclaim there, there are complications. so, for instance, not having enough people to administer the vaccine and, of course, thus far, the vaccinations haven't gone as well as projected and so do you think it's still realistic? >> i really still do, alisyn. and i know that there's been a lot of concern about this. i've been talking to people within all these sectors at the federal level, at the state level, but also the private sector level as well. it's going to come with bumps and hiccups and it's going to feel uneven. the way the system was set up was that "operation warp speed" was getting these doses to the states. they were tracking it through the software platform tiberius. after that it was left up to the state to handle it. we've seen what's happened so far. one thing to point out is that the major pharmacy, the major pharmacy organizations have been largely responsible for taking care of long-term care facilities. a

sanjay gupta. sanjay, first of all, give us your overall assessment of the new plan president biden and his aides are laying out today because there is a lot of specific detail there. >> yeah, i mean, it's pretty comprehensive, wolf. we've been talking about this for so long, to hear some of these potential strategies to move and address this pandemic, actually starting to be discussed like this i think was pretty significant. ten executive actions that were signed. as you point out a lot of specifics, fema saying we'll have a hundred area clinics in areas particularly hard to reach by the end of the month. the cdc tasked with making sure pharmacies can also start to deliver vaccines if they have enough resources and staffing and all that by the beginning of next month. hhs tasked with trying to recruit more vaccinators, people who can actually administer the vaccines, pulling people maybe from the military or retired health care professionals. whatever it may be. there was a lot of detail there, wo

half the dose twice instead of the full dose twice that's what moncef slaoui told my colleague, sanjay gupta that the fda is going to consider. they did that in phase 2 or something along the lines. and when they went in the lab and looked at the blood of folks vaccinated it did give an antibody response but it's different from saying it prevented people from getting infected with covid-19. you only know that in a phase 3 trial. i'm going to say it again because it's so important. you only know if you're protecting people once you do a phase 3 trial. a phase 3 trial was not done with this half formula, and so what the doctor said is i can't imagine why in the world we'd want to put that into play. i asked dr. fauci what he thought of it and he said he wanted to see the data before he gave an opinion. >> thank you for that update, elizabeth. the president and also the president-elect, joe biden are both going to be in georgia today. this is after the president's stunning phone call with georgia's secretary of state over the weekend. what effect will it have on the runoff that's tomorrow? when
