steve centanni is live at reagan national airport in arlington, virginia, where this all happened earlier this week. steve? >> reporter: yeah, jon. it was a question of miscommunication, and it could have been very dangerous, but, of course, there was no problem here, and nobody was injured. all the planes got to their destinations safely. what was happening was there was a direction of traffic from south to north on the runway you see behind me over here, and because there were thunderstorms in the air, the faa, the local controllers were trying to reverse the direction to go from north to south for all landings and takeoffs. well, one of the planes, at least, didn't get the word, and that's the one that was coming in for a landing from the north when he shouldn't have been. at the last minute, controllers diverted him to the west, away from the airport. we have a graphic that shows what happened. we had the one plane landing from the north who really shouldn't have been there and two planes on the opposite end of the runway, the same runway trying to take off heading from south to north

steve centanni is live in washington. what did you find out?> reporter: this report from the inspector general found 154 different mailing addresses where multiple returns were filed resulting in millions of dollars of fraudulent refunds. from one single address nearly 24,000 returns were filed and refunds were issued totaling $26.3 million. an address in oxnard, california where 2,500 refunds were mailed. the report says the i.r.s. has not established control over those submitting returns with numbers issued no non-resident workers. it all came to light because of complaints to congress by i.r.s. employees who saw this going on. wanted congress to know about it. the report says the agency needs better training and documentation so they can spot these questionable applications and stop fraud. the agency says the i.r.s. has moved quickly and aggressively to address issues identified. we have taken major steps to strengthen our documentation standards and significantly increased our scrutiny of applications. congress likely to be taking a look at

steve centanni live at reagan national with more. >> reporter: this happened yesterday afternoon aboutit was first reported by "the washington post." their normal pattern was planes were coming in from the south on the runway heading towards the north. all takeoffs and landings going in the same direction. there were thunderstorms in the area, the wind was shifting. but they want to make it so everybody can come from the north, all on the same runway. but not everybody got the message about that very important switch. so what happened was here is a graphic we can show you, we had two planes take off from the south, they were the last two to take off under the old pattern and we had a plane landing from the north, it was the first to come in for a landing under the new pattern, and apparently there was some miscommunication between the tower and the regional traffic controller and they came too close to each other, 1.5 miles or 800 feet vertical separation. it's still well within the comfort zone and the faa says these two planes were too close but they were heading in a somewhat differ
