two women were killed when their parachutes failed during a tandem skydive. investigators say the instructor-- a california resident-- was very experienced... with more than 4,000 jumps under her belt. witnesses say when the women jumped out of the small plane-- everything looked normal. but soon they realized something was very wrong.
