teresa garcia tells us, they simply walked out the front door escorted by the guards. >> florida police are looking for two convicted killers who escaped prison, essentially by presenting a homemade get out of jail free card. >> these two individuals are out, they shouldn't be. >> on two occasions, joseph jenkins were released from this franklin county florida prison. guards walked him out the front gate. why? because prison officials were duked by forged court documents. the documents contain the signature of belvin perry. >> if you have some fundamental knowledge, not a great deal of knowledge, but some fundamental knowledge concerning the computer system, it's easy to go on and lift that signature and fix it on something else. >> it's not hard to find judge perry's signature. he was the presiding judge in the casey anthony murder case and his name is signed on dozens of court papers available online. prison officials say guards follow the rules. >> we initiated our typical procedure for an inmate release. the department followed our procedures and protocalls to the letter. >> jenkins

as teresa garcia reports that teacher is being hailed as a hero. >> reporter: the deadly shooting at a nevada middle school was fast, furious and frightening. >> i saw the people running, i thought that was going to be my last day. >> he pointed to us and then said, you ruined my life, and i'm going to ruin yours. >> reporter: the 13-year-old shooter opened fire around 7:15 a.m., just before classes started on the campus of sparks middle school. >> a male student came to the school with a semi-automatic handgun. he shot two students, one male teacher, before turning the gun on himself. >> reporter: law enforcement arrived within three minutes, but it was all over. students say 45-year-old math teacher michael landsberry had approached the boy and told him to lower his weapon. instead, the teen fired into the teacher's chest. >> to my information, we don't know everything he was doing yet, but we do know he was trying to intervene. >> reporter: landsberry was a military veteran, having served tours in afghanistan. he coached the school's soccer team and was a beloved figure on campus.

as teresa garcia reports, this may be part of a larger problem. >> three days after escaping this florida prison with forged paperwork, joseph jenkins and charms walker walked back into an orange county jail to register as felons. ironically they were following florida law. corrections workers took these mug shots of the men and fingerprinted them, but once again, jenkins and walker walked out free men. >> they showed up and signed documents at that time so we know that they were here in this area. >> jenkins and walker who were serving life sentences for murder were mistakenly released after prison officials were fooled by these fake documents bearing forged signatures. the orange county sheriff's office is working with the corrections department to investigate who created these forgeries. >> we're looking at the system's breakdown and i'm not standing here to point the finger at anyone at this time. >> cbs news has learned corrections officials had received phony papers once before for general kin's release. that time they spotted the counterfeit. pugh is terrified knowing her husband's
