anne makovec is in san bruno, where we are just now hearing from the head of the national transportationety board. anne? >> reporter: she says this is the kind of information she would have expected to hear months ago. pg&e just revealed a few weeks ago that the pipeline in this same area sprang a leak back in 1988. they only told investigators about this a few weeks ago. the chair of the national transportation safety board, deborah hersman, and congresswoman jackie speier were touring the site of the blast just within the past hour in part to announce some new safety recommendations. hersman also talked about pg&e's faulty recordkeeping and says the leak revelation shows just how inadequate it is and that the nation has a lot to learn from what happened here. >> no need to tell you anything about the tragedy that occurred here in this community. you all know about that all too well. what the ntsb's role is, is to find out what happened and how it happened and to make recommendations to prevent a similar accident from occurring in another community. >> reporter: of course, the blast occ

on tuesday, the national transportation for america released a report called " dangers by design." it highlighted the fact that in the last decade, 47,700 people were killed by being hit by cars, which the report points out, is the equivalent of a jumbo jet crashing every month but right now we are not seen the kind of public attention and funding that would go to a catastrophe like that. the report also found a majority of deaths occurred on arterial streets, the wide, fast streets, like the ones south of market and elsewhere in the city, including lombard, where that young man was hit. here in california, the report also found seniors, people of color, low income communities are most at risk of pedestrian injury and death. this is true in san francisco and even more so, our pedestrian fatality rate -- the rate at which pedestrians are a percentage of all traffic fatalities. it is more than half year, which puts us at no. 3 in the country among counties. for all these reasons, this is an opportunity -- this bond provides the opportunity to give funding to this issue that the report

. >> reporter: investigators with the national transportation safety board were back at crash site. ooking for evidence to help them figure out why the driver slammed in to the train. they say visibility on that day was excellent. the crossing gate and warning lights were working. should have been seen from well over a half-mile away. >> at this point, the team is still working on the vehicle. that was part of the tires and brakes are a focus. >> reporter: investigators will also visit john davis trucking the company that employed the driver. the ntsb says since september, the company has had seven violations from 19 random roadside inspections. in one case, the tires on a tractor-trailer were so bald it was immediately taken off the road. but lee's children say they haven't been focussing much on the investigation. their thoughts are with their mother. >> she was a very big person, and a very big voice, and a heart about three times bigger than her voice. she looked out for everyone. >> investigators are looking at a number of other factors that may have played a role in the crash.
