nbc bay area's vicky nguyen joins us with the details. vicky? >> reporter: we reviewed months of inspection records for the valley transportation authority. they reveal a list of critical fixes that vta has been slow to make. state inspectors found vta violated safety procedures and state regulation, failing to maintain its trains, leading to situations where trains could collide head-on or potentially derail. now vta trains carry 140,000 riders a day. they share the roads with thousands of cars. records reveal vta has failed to fix signals and to get its fleet into compliance with state standards. now time having run out as levy stadium opens in august. inspectors say they are still finding problems. when your inspectors found these problems, what did they instruct vta to do? >> not run those vehicles. and we found that they did run them after that. >> vta is a very safe operation. we have very professional operators, and i encourage everyone to ride vta. >> reporter: tonight more on what inspectors found, and why insiders say vta has been slow

goode evening, i'm vicki liviakis. this is *not the first story we've done about the oakland mayor behind the wheel. she's been caught on her phone twice in the last month. and we have the photo evidence to prove it. tonight -- just after the accident. our reporter was the only one to talk to the mayor. first. alecia reid has the latest from the scene of the crash in oakland. it's a story we brought to you *first on four. mayor jean quan was escorted from the accident by opd. the driver of the other car involved remained at the scene for hours. witnesses tell us the mayor was on her phone and blew through the red light at the 26t inters. we're told the mayor did get out of her car to check on others involved in the accident. opd says there was an independent witness that tells them the mayor had the green light. investigators say it's hard to determine who's at fault. they're hoping surveillance footage from the market across the street will help in the investigation. reporting live in oakland. only on kron-4. we have

forensic plant ecologist vicki trammell looked for any plant material that was in the hair. >> the hair five years outside most likely being drug around by scavengers like coyotes. so my question was not so much where is the body, because i had no way of determining that, but where had the hair spent five years? >> the lack of soil in the hair suggested the body had not been buried. >> the hair wasn't muddy and there was bleach marks. in other words, bleached by the sun in the places that were exposed. and that would not have happened had the hair been buried for five years. >> trammell also found pine needles and shards of bark in the braids. the bark was from an aspen tree which tends to grow in cleared areas. she also found needles from two coniferous tree species, the subalpine fur and the engleman spruce. subalpine furs grow in elevations above 9,000 feet. and engleman spruces grow in wet areas suggesting the body was somewhere on the mountain's north slope, which doesn't get much sun. trammell identified an area matching all these parameters just one mile from where the braided ha
