reporter: it happened the day of june 2 during the house fire that killed veteran firefighters vincent perez and tony valerio in san francisco's diamond heights. they died after a flashover, a surge of searing flame when everything in a room ignites at once causing the room's contents to explode. in cal-osha's report, issued monday, the agency doesn't blame the san francisco fire department for the firefighters' deaths but it points out serious infractions cal-osha said it found during the investigation. in addition to the unresponded radio calls, cal-osha says a battalion chief entered the house alone and that at one point, only one firefighter was outside the house to monitor the situation and handle the pumps, both against the rules of. the san francisco fire department has a report of its own coming out but in the meantime has a message. >> the public needs to know that our members are always ready to help our citizens and at a moment's notice we are there to save them orr do what we have to do to protect them. >> reporter: the fire department said cal-osha didn't give them a chance to r
