wen, let me ask you about schools. i've got a superintendent in iowa who's coming up a little later, where iowa has banned mask mandates for those schools, most grades are not eligible or a lot of kids are not eligible for the vaccine because they're under the age of 12. what should be the testing regimen for these schools? should they start with a baseline of every student being tested? should there be testing every week, every two weeks, month, what do you think? >> yeah, i mean, i think it's too bad that these schools are not requiring masks because they are a really effective layer of protection. we can think about this as if you're going to remove the layer of masking, what can you replace it be w? ideally it's with vaccination but if that's not possible, twice weekly testing for every student and teacher that's unvaccinated can be a replacement for masks. not completely, though. and so that's why i really hope that parents and these schools will still consider wearing -- having their child wear masks but i also t

wen, i know you received the johnson & johnson vaccine. require a booster to protect against variants. there seems to be a lot of concern over boosters. i have this sense that boosters are looked at as a bad thing. we've all had a number of different boosters in our lives with the various vaccines we've had. >> we always thought at one point one of two things might happen. one is that immunity might wane over time. two is there may be variants developing for which the vaccines we have may be less effective. in the johnson & johnson vaccine, there is contradictory evidence. there are two different studies, one by the manufacturer and one by independent researchers that has not been peer reviewed that show different things about whether people with the johnson & johnson vaccine may benefit from another dose. we're in this gray zone where our federal health authorities are not able to make a recommendation because we don't have this data. i will tell you as a clinician enough a patient who is particularly vulnerable, elderly, chronic medical

wen?ed on the science that we have, that getting the vaccine protects you very well from getting ill, reduces your likelihood of transmitting the virus to others. we also know that it reduces the amount of virus that you carry even if you get infected. the problem, though, is with the delta variant, it seems that you get more of the virus in you if you're infected. so does that somehow counteract the effect of the vaccine, and to what extent? we don't know, and again, these questions need to be answered to advise a lot of people. these are not undermining the trust in the vaccines, it's giving people who are vaccinated a better understanding of what is it that we should be doing still to protect ourselves and our loved ones. >> and again, still 93% efficacy against severe illness and hospitalization, which really is the most important thing. if anything else, it points to the reason to still get vaccinated, even more important than ever. dr. fauci told nbc yesterday, when he's in areas of

wen, you heard liz wedge reporting on the updated guidance for schools. i'm curious what your impressions of it are? >> based on draft guidelines i think that the cdc did the right thing here. because they are doing two things at once. they're saying that schools should be open for in person, full time instruction come the fall. we do that and we could do it safely if we have the right mitigation measure ms in place. i'm glad that they said masks are required for unvaccinated children over the age of two. that is essential. especially when we have the more cont contagious variant yet in the u.s. i think it is important that they mention testing. testing is a great measure, especially if you're not going to be doing distancing. if you're bringing back kids full time, a lot of schools could not do distancing so in a sense testing is replacing distancing. the cdc could do more to encourage vaccination to make explicit if you're vaccinated, you don't need this weekly testing. and so we want to you be vaccinated to opt out of testing and masking. but i was ple

leana wen, who is also a cnn medical analyst. dr. wen, great to see you.of this back and forgth? the messaging was not clean on this. >> i do think there was some unnecessary confusion here. pfizer is smith an emergency authorization application to say if at some point we need a booster shot, they want to have that on hand. we may need a booster, one, if immunity from the vaccines begin to wane over time, or two, if there are variants that develop to which the vaccines may work less effectively. so i think it's fine for them to submit the emergency use authorization. then that way we have the booster in case it's needed. what the cdc and fda are saying is, right now the booster is not needed. but let's look at the data that pfizer is going to present and it may well be that for a segment of the population, for example for patients who are severely immunocompromised, they might benefit from a booster shot first or older people may need a booster first. but for the general population, the bottom line message is that you do not need a booster right now so ple

wen would think of this. just anecdotally, i know we all know people who are vaccinated and feel safe who have gotten covid. >> yes. >> and haven't ended up hospitalized but have ended up pretty sick, compared to how we thought we would be when we're vaccinated. so, what i wonder, just from a public-health perspective and, frankly, a political perspective because it's about expectations, is whether you say if you, the unvaccinated, don't get your vaccines, you are going to continue to allow this virus to mutate. and for variants to be as -- as -- as infectious as they are. and it will make it impossible for all of us to be -- to be safe. maybe, it's more nuanced than he can be but that is the reality of what's happening right now. >> yeah. let's play what he said about sort of the altar call. >> one of those other networks is not a big fan of mine. one you talk about a lot. but if you notice, as they say in -- in the southern part of my state -- they've had an altar call, some of those guys. all of the sudden

wen, what do you think? how much does this move the needle?panies saying you have to get vaccinated and president biden saying federal employees, you've got to get vaccinated or we're going to make your life pretty annoying with a bunch of testing and mitigation measures? >> this is huge, and i think we need to talk about why it's being done. i mean, imagine if you're being asked to go back to work in-person, and you know that you're going to be surrounded by potentially unvaccinated people who could infect you. and even if you're vaccinated and then you know that you're not going to get very sick yourself, what if you live at home with young children who are too young to be vaccinated. what if you live with immuno compromised people. this is basically saying that workplaces have an obligation to their employees. either you have testing or you have proof of vaccination. and i think this is such an important move. i wish the biden administration, of course, did this a while ago, months ago, but better late than never. i think this will have a h

wen, thank you for joining us. i really appreciate you helping us out with this story.nts to know about, what about kids, kids under 12, can't get the vaccine yet. the delta variant is really contagious. are kids in more danger now than they have been in the past? >> yes, because we have this variant here that's more contagious. what that means is that the activities that we thought were previously lower risk are now going to be higher risk. things that we got away from before and just did not get affected because of those activities now there is a higher chance of getting infected because of it. and so i do think we need to change this narrative. there is this narrative floating around that somehow kids are immune from getting coronavirus. that's just not true. more than 4 million children have already been affected during this pandemic, and if we have something that's even more contagious around, schools should not be dropping their mask mandates. if you have lots of unvaccinated children together with one another in indoor, cramped settings, not great ventilation, it

i can fully appreciate that the people living on waun wen road, and the surrounding areas, this wouldnse, we will look at all of these aspects. what calls were made to us and over what period of time we responded. but that will become apparent in due course. of course one of the allegations made against the police is that the police response that night was wholly inadequate. and that for the space of two or three hours there wasn't a sufficient police response. do you accept people have reason to be disappointed? i'll look at all aspects of it. so, mobilising resources, notjust from the immediate area but from across the force, which is something obviously we can do. i'll be looking at that, mobilising resources, command decisions. and decisions as to when, where and how resources were deployed to the scene. seven police officers suffered minor injuries as they tried to confront the mob, with reports of up to 200 people at the scene. while the police were forced back by the crowd, some locals took the matter into their own hands. as the police withdrew — a number of local men, includi

wen, that goes on that people are not vaccinated, gives the delta variant to do more work or the next variant to come about. because the next step the cdc said in this document is the way they put it is acknowledge the war has changed. what does that mean? >> well, think the cdc really needs to change their messaging here. because it is been really confusing for people to be reading these leaf documents and try to match it up with what they said earlier this week. my takeaway from all of this is yes, the delta variant, as doctor said is extremely concerning. it is very contagious and makes people sicker and it makes people sicker faster and it is affecting younger people. but at the same time, we also know that the problem is actually not primarily with the vaccinated. the problem is mainly with the unvaccinated. when we look around the country to see where are surges occurring and people ending up in the hospitals, it is concentrations of unvaccinated individuals. so yes, it is true that if you are vaccinated, you should know there is a chance that you could still contract covid-19 a

leana wen, dr.nice to have you back. >> great to join you. >> so didn't reach 70% the goal, just above 67%, not bad, for adults 18 and above. given estimates of how many others in the country were exposed to this, though not vaccinated might have some immunity from exposure, do we have any sense of how close we are as a country to herd immunity? >> i don't think we're that close to herd immunity, and the reason i believe that is we're seeing surges in so many parts of the country. if we actually were at herd immunity due to, as you said, vaccination or recovery from coronavirus, then we wouldn't have these huge surgers we're seeing in missouri, arkansas, wyoming, nevada, so many parts of the country. i think at this point we need to acknowledge them. we're at a plateau when it comes to vaccinations. a lot of people who have wanted to be vaccinated, they're vaccinated. there are people who are hesitant for whatever reason. of course we need to work on the ground game of doctors, community health work

wen?ay it's almost certainly -- let me go to something else because we're running out of time. this could end one of three ways. that we won't get it under control, there'll be ongoing severe disease or there will be transition to an epidemic seasonal like the flu, or we'll transition to an endemic that has lower impact than the flu. which has more impact here for you? >> i think this really depends on what it is that we do at this point. so now we have this delta variant that is much more contagious. because it's more contagious, it's going to be even harder for us to reach herd immunity. we're going to have to vaccinate an even higher proportion of people to get there. what happens, then, if we end up having another variant developing that's even more contagious, that could cause more disease, that could evade the protection of our immune system? so how quickly we can get this under control and which way we go depends on what we do now, to overcome disinformation. what we really need to

leana wen. good to have you. let's start with john left off. masks, at this point medically, would it be your advice that vaccinated people should wear masks inside? >> well, i am really relieved that the white house is considering to revise their guidance because the circumstances on the ground have changed. the realities have changed. the science has changed. you would expect that the policy changes accordingly. initially in may when the cdc came out with their guidance saying if you're vaccinated you don't need to wear a mask, first of all, they didn't take into account the honor system was not going to wear and unvaccinated people were taking off their mask as well. but also we are now seeing that we have 40,000 new daily infections per day compared to 11,000 a month ago. we have the delta variant that's surging and actually we don't know at this point whether if you are fully vaccinated, whether you're able to transmit the virus to others if you're carrying the delta variant which is the dominant variant here. so i think it makes a lot of

leana wen, former health commissioner for baltimore. so dr.rms that senator paul is making? >> i first want to distinguish between gain of function and what is being discussed here. gain of function actually is a type of research that commonly occurs because if we're talking about, for example, if you have bacteria that you're trying to make to digest plastics better, that's also gaining a new property, gaining a new function. in this case, when we're referring to covid-19, the allegation here is senator paul is saying that somehow the nih provided funding to this lab in wuhan, which is true, that the funding was provided, but he's specifically alleging that the funding was to create a virus that is going to be more transmissible and more lethal and actually led to covid-19, that somehow also leaked from the lab, whether intentional or not, that then led to a global pandemic that's killed millions of people. and so the implication actually by senator paul is something really nefarious and incredibly insulting to dr. fauci because it's saying,

wen, you tweeted something interesting earlier today.vaccinated and then going around un-masked, in crowded areas, is not like eating unhealthy food. no one should have the right to spread a potentially deadly disease to immunocompromised individuals and young children. why should the most vulnerable pay the price? in your mind, should more communities be following what los angeles county is doing? reinstating masks, if people won't get vaccinated? >> yes, frankly, i think that the mask mandates were lifted prematurely. i had said so, at the time. i think a lot of us in public health were very worried about what we are seeing, now. i mean, we are seeing virtually every state have a surge in cases. we are seeing 19 states that are having doubling of the infection rates of covid than they did two weeks ago. arkansas, having a fivefold increase from early june. we are seeing this because the public misunderstood the cdc guidance for fully-vaccinated people, as we can now do whatever we want. even if we are unvaccinated, we can now behave, a

wen, what do you think of this move, first just looking at the new move in l.a. county.saying once you lift these restrictions, you're not going to be able to put them back on. >> think it is hard to put the jeanne back in the bottle. they're going to be a lot of people who will resist that but i think l.a. is doing the responsible thing. we have been saying all along this is not an on and off switch. we have to use indoor masks again if we see surges. i wish the participants of the country seeing high surges, that they would follow the lead of l.a. county. the problem is not with the vaccinated individuals. if you're vaccinated and you're around other people who are also fully vaccinated, there is virtually to risk to you. the problem is the unvaccinated are behaving as if they're vaccinated. they are a risk to other unvaccinated people, including young children. but they're also a potential risk to vaccinated people. because of the risk of break through infections. so do think that a proof of vaccination will be checked. it makes sense to be reinstated indoor mask mand

leana wen. good evening, dr. wen, great to see you. so, here we are. it is a holiday weekend. i remember what the discussions were like, last fourth of july. now, the discussions are different. obviously. when it comes to coronavirus. medical officials are saying that, look, if you are vaccinated, there is almost-no risk. but should we be celebrating with family members or friends that are not vaccinated? and do we need to wear a mask? >> it's a great question. and first of all, i do think we have to have knowledge that we are in such a different place from where we were last fourth of july when the worst was yet to come, versus right now. i think the worst is behind us, especially if we are vaccinated. here is the thing. if you are fully vaccinated, and you are around other people who are, also, vaccinated. you are very safe. on the other hand, if you are unvaccinated and people around you are unvaccinated, too, that's not safe. if you are vaccinated and your family members are not vaccinated, you are probably safe, yourself. especially, if you are not immunocompromised. but

leana wen, cnn medical analyst. she's author of "lifelines, a doctor's journey in the fight for public health." dr. wen, with 24 states seeing cases up at least 10%, how concerned are you about this pfizer news? according to the cdc, more than 84 million americans have received both doses of the pfizer vaccine. >> i'm actually concerned about the pfizer news confusing people at this point. i think it's important to put it into perspective. as you mentioned, it's not as if they have new data. they're actually going off of what the israeli ministry of health reported earlier this week. the key take-home from that is that the vaccines that we have are still very effective against protecting against severe disease. so no one should be listening to this and saying i need to go out and get a booster shot right now. now, it does appear that immunity might wane or the vaccines are less effective against the delta variant. so i think that's something we need more information about when it comes to mild or asymptomatic illnes

lena wen. sanjay, as you wrote just last week, we got very close, tantalizingly close in the race against covid. now, as i mentioned, we find ourselves in this unsettling, confusing time. from a medical perspective, where are we in the course of the pandemic now? >> well, you know, if the country came in for a checkup right now, anderson, i think the doctors and nurses would tell them they're concerned. i mean, you have about a third of the country now that has high levels of viral transmission. we're in july. this is supposed to be the best time of the year as far as viral transmission being low. people are outside, the virus doesn't like to be outside. this could be as good as it gets, at least for a period of time. and yet a third of the country can take a look at the map. viral transmission is high. at the same time that the viral transmission is high, vaccination rates are low. i mean, they're really remarkably low. i have to tell you, i'm here in tokyo. we have been talking a lot about at

leana wen, the former baltimore health commissioner.r, more than 2,000 people are still dying per week here in the u.s. from covid, and we know the new variant is in every corner of the country. talk to the unvaccinated people who have made the calculation that they are willing to risk getting covid versus the vaccine. what do people need to know about the delta variant? >> people need to know that if you are unvaccinated, you are at high risk for contracting covid-19, in particular the delta variant. this is more transmissible. it is already becoming dominant here in the u.s. the five states with the highest rate of covid-19 infections, the delta variant is more than 50% of the cases. it is 70% of all of the new cases, for example, in missouri, 60% of all the new cases in utah. people who are unvaccinated should know, yes, it is possible that you are not going to get severely ill. actually, most people do not become severely ill or hospitalized or died, but you could still be a long hauler. you could end up having long-term symptoms of
