in the united states, behind heart disease, and cancer. >> health reporter stephanie stahl is here with more in a new report and pennsylvania group that is looking out for residents. >> yes. >> reporter: trying to help on 24 would. pennsylvania safety patient authority has said there has been a noticeable increase in reports of, series of events and deaths at hospitals and medical facilities around the state. there is a similar situation, a cross the country. medical mistakes have become the new, third leading cause of death in the united states. >> we're talking about patients dying from the care that they receive rather than the disease or injuries for which they seek care. we're talking about things that happened that shouldn't happen in a sound health care system. >> reporter: medical errors include mistakes in diagnosis, inadequate discharge instructions, and preventable complications. such as infections, pick up in the hospital. >> patients just don't die from heart plaques and bacteria, they can also die

from communication break downs and medication errors. the system level problems are almost ubiquitous in health care. >> reporter: researchers say most medical errors are not due to bad doctors, they a say system wide failures are responsible for putting patients at risk causing 250,000 deaths a year. >> patients falling through the cracks, a fragmented health care system, diagnostic errors, medication dose goerors and preventable complications. >> reporter: pennsylvania pennsylvania safety authority's annual report says serious events at medical facilities are up 9 percent with a 22 percent increase in patient deaths. that could be due to implementation of the new standardized reports. so the question everybody is asking is what can patients do to protect themselves. welshing a couple things. be informed? know about potential complications, ask questions, especially about medications or treatment options, and get second opinions if you can. you know, if you are in the the hospital though you are out of it sometimes so get a family member to be involved asking questions.
