i would observe and in listening to zeke, it reminded me that california has been in this value-based care game for quite some time. i think it's fair to say more so than the rest of the country. having been in california since 79, going back to the mid- 80s and moving on from there. the affordable care act certainly turbocharged the movement of value -based care and frankly, only in the way that big movement by the federal government ten. when it comes to issues like healthcare in america given the role that the federal government plays in this arena. the american healthcare act which is being perhaps voted on or is being voted on as we speak, frankly, has elements to it that raise question about the momentum around it. maybe we can touch on that later on. finally, when i think about value-based care i think about in three ways. value -based care at the patient level, along the lines of what wyatt and i have already described. there's the issue of value -based care at the organization level and there's the issue of value -based care as it relates to the system itself. whether that's

if you were to take the same countries that zeke had up on the slide, and if you were to examine their domestic woulditures for what we refer to as the social safety net and support systems that go into a population's health status, i think you would be stunned at the difference between the united states and those countries in terms of what they invest. i am not bringing that up to suggest that one is right or one is wrong. but i do bring it up to suggest that, when it comes to the question of health status, those of us in the delivery of care, whether it is the ambulatory side or the inpatient side, we are kind of the end of the final. the funnel. we are involved to the extent that we can impact prevention of disease and the avoidance of it. but in terms of the impact on that last point, what we can do pales in comparison to the other things that drive an individual's socioeconomic status that in turn contributes to their health status. dr. decker: i would just underscore, tom, your comments. when you look at health, -- health outcomes in the united they are driven far more by individ
